EE was climbing the ladder to hang a chain fall when the ladder slid from underneath him and he fell to the floor hitting his head on a piece of metal that resulted in a laceration to the back of his head.

Individual was completing a pre-operational inspection on the feeder, he slipped and struck his right forearm on the rib, injuring his right shoulder.

The injured employee was exiting his vehicle to enter the office when he slipped and fractured his right ankle.

EE had attended a safety meeting and was getting ready to board the man trip to go to his assigned work area, when he slipped on some ice and fell. EE continued to work his shift and went to his family Dr. the next day.

While working on a tractor and climbing down missed a step falling about 6 inches and landing hard on his right leg. His right knee gave out and he felt a sharp pain. X-rays revealed no fractures or dislocations and he was released to the care of his own physician. Began missing work 1/24/2011 after an MRI revealed a medial and lateral tear of the right meniscus.

While working out of a scoop bucket slipped and fell landing on his right shoulder resulting in pain when pushing, pulling or lifting. He did not seek medical attention until after his shift. Initially a first aid only became a lost time after surgery to repair the shoulder. Began missing work 04/02/2011.

EE was attempting to climb onto the 850 John Deere dozer. He had placed one foot on the step. When he tried to place the 2nd foot on the step, that foot slipped in the snow. He was holding onto the handrail. When his foot slipped, it caused him to twist and fall against the tracks of the dozer, pulling a muscle and pinching a nerve in his back.

EE was getting off his dozer at the end of his shift when he slipped on a rock that had became wedged in the step that was located on the push arm of the dozer.

After bending down to place a magnet stepped through a hole in floor grating puncturing his left shin. Transported to St. John's ER Springfield for treatment including sutures and staples to close the wound. He returned to his next scheduled shift and has not missed any work.

Employee fell while getting down from grader. Foot slipped, causing fall. Tried to break fall by putting hands out, causing strain on left hand. He reported to supervisor, refused medical treatment. Went to ER next morning 11/30/10, and had hand X-Rayed. Employee came to work that day, missed no time. Hospital notified him 12/5/2010 with the results, confirmed his hand was broken.

While parking Grader near end of shift, employee noticed the small door on the right rear at radiator was opened. Holding to hand rail, employee walked to door and while bending over to shut door, his feet slipped and all of his weight was on his left arm and shoulder. He went home and next morning went to Dr. He was taken off work.

Individual was standing on the bolt machine hanging line curtain. When he proceeded to get down, he slipped and fell. He landed on a piece of coal and injured his right ankle.

EE was working inside the #1 scalping screen, upon exiting the screen, he twisted his left knee while trying to make it from the discharge lip of the screen over to the crusher chute then onto the breaker building floor.

Individual was advancing a rock dust hose along the belt line when he stepped into a hole and cracked his left fibula

Individual was walking the secondary escape Way as he was crossing a overcast the step broke and he fell.

Employee had just finshed routine maintenance on a drill and as he was walking away from drill he slipped on a berm of dirt. When he reacted and tried to catch himself he devloped a pain in the left side of his ribs.EE finished his shift and went to the Dr. on 09/09/2010 and was placed on light duty work for five days.

EE was helping pull the trailing cable of the continuous miner off of the cable reel in an effort to locate the reason that the circuit breaker was knocking. While doing this his foot became entangled in that trailing cable causing him to fall; and his head hit the ground. As the result the side shields on his safety glasses caused a laceration that required 7 sutures to close.

Individual was getting out of a scoop. He fell and his foot was still in the scoop cab. He twisted his ankle in the process.

While walking tripped and landed on his left heel. He continued working and completed his shift. Initially reported 6/3/2010 as a Minor First Aid became a Lost Time Accident 7/13/2010 after being diagnosed by his physician as a left calcaneal fracture although X-rays were inconclusive and directed to perform "no work until further notice".

The injured, a contract employee, was in process of servicing a Caterpillar de-watering pump generator. While performing the task, his foot slipped on the trailer step causing his foot to slide in the space between the trailer and the house of the generator. As a result, he twisted and bumped his knee.

EE stated that as he was dismounting a D-11 Cat Dozer his knee locked up and made a popping sound. EE finished shift then went to his family DR where X-Rays were Negative. EE returned to Dr. and an MRI revealed some torn cartlidge. EE's first day off work will be 07/05/10 as the mines were shut down from 06/26/10 thru 07/05/10 due to vacation.

Individual stepped in a hole and twisted his knee. He continued to work his regular schedule and on 6-10-10 he had outpaient surgery and was placed on light duty

Employee stated that as he was checking tires, he tripped and fell. As he fell, he grabbed tires and twisted lower back.

While attempting to pull down a rib that MSHA had deemed a hazard and using a slate bar a small piece of the rib broke off allowing him to fall backwards striking the back of his head on an unknown object. The impact caused an open laceration approximately 1.5" long that required sutures to close. He returned to work after treatment without restrictions.

Individual was walking backwards while rock dusting when he stepped in a hole twisting his hip. He continued to work untill seeing his doctor again. This was a minor incident untill the 2nd of May he returned from his Dr. appointment with a restricted duty notice.

EE was tramming the continuous mining machine across the 003 section, when he stepped in a hole in the mine floor and twisted his right knee.

Employee was walking down road from drill bench to service truck and slipped causing a sprain to his right knee. On the 26th didn't report to work & contacted doctor. Reported back to work on the 27th. Went to emergency room on 28th due to pain and swelling.

Injured was standing in an upright position and was reaching above his head to hang a ventilation line curtain, when he fell back against the continuous miner which caused a pain in his back. Injured continued to perform his normal duties, without restrictions, and began to perform restricted work on 12/28/2009 making it reportable on that date.

EE was preparing to exit the steps at the back of the launch vehicle to do a welding task on a hydraulic jack end. When his right foot contacted the frozen ground he twisted his right ankle, which has been diagnosed as a sprain.

The employee was assisting in the removal of a panel cover for a continuous mining machine. He was crossing the conveyor on a disabled continuous miner when he stepped on a piece of rubber belting and slipped. When he did this he twisted his left ankle which has been diagnosed as a sprain.

Employee was in an upright position operating a Fletcher DDO-15 Roof Ranger Roof Bolter, when he slipped on a piece of rock on the mine floor. In an attempt to break his fall, he extended his right arm out and when he landed, he received a strain to the right wrist.

Mine Supervisor asked EE to clean up area where he was working to see why water was dripping on them. EE hosed down area and there was a hole. When water broke the hole open, EE was dumped in to the chute and covered up.

EE started going over the hill to seed hollowfil when he stepped on a rock and fractured his little toe.

The employee lost his footing when he reached for a roof bolt plate from the bolting machine tray. Laceration to his left arm.

While working employee stepped into a hole and fell striking his right arm, shoulder and head and felt pain. Pain worsened and he called an EMT. Transport by ambulance to St. John's ER Springfield. No fractures were indentified and he was scheduled to undergo a series of physical therapy sessions making this a medical reportable event.

Employee stated while working from a 2-foot step ladder and placing boxes of filters in the appropriate bins, she lost her balance and fell to the floor. In doing so, she twisted her left knee resulting in a sprain. Note: Employee started missing work due to injury on May 31, 2009.

Injured was standing on elevated pad, washing out discharge chute on #1 belt. As he stepped off pad, he slipped. Hand caught a piece of metal and cut finger. Some skin was later removed and a skin graft was done on 5/21/09.

Employee stepped in between the grader blade and tire to start up the ladder to the cab of grader. He twisted left knee and fell to the ground. He stated that he heard a popping noise and was unable to put any weight on or stand. He was transported to ER and X-ray showed nothing broke. He seen Family Dr. on May 1, 2009 and returned to work on May 4, 2009. LTA began on May 26, 2009.

Injured was carrying a joint of 8" waterline and stepped in a hole, twisting his right ankle. When he fell, the waterline landed on his right foot. X-rays negative. Injured given few days off work to heal.

Employee was dismounting the D9R #2 Dozer with an oil sample bottle in his hand, when his foot slipped, causing him to strike the blade push bar with his left rib cage.

EE was walking down the belt line entry to the surface and stepped in a hole and twisted his right ankle.

Helping the mechanic work on push out lever on a Ram car, while prying down on the arm, he lost his balance and fell into the Ball hitch on a 4 wheeler buggy striking his right knee and the right side of his back on the hitch. He started missing work 2/19/09 and missed 2/20/09 and reported back to work 2/21/09.

EE stated he was feeling ill and decieded to get some fresh air and when he opened the door on the dozer he passed out and fell off the dozer causing him to recieve several cracked ribs.

The employee was in an upright position walking down the mine pit access road, when he stepped on a piece of ice, causing him to slip. He was carrying a plastic dinner bucket and when he fell, his left chest area fell upon the dinner bucket, causing him to fracture the 5th and 6th ribs on the left side of his chest.

End loader operator had got off the loader to move the coal stacker. When he was walking back to the loader he fell on the frozen, slick ground. When he was uprighting himself, he struck his head on a metal storage rack causing a strain to his neck.

Contractor was dismounting Cat D-11 dozer. As he got down to the lift arm, he slipped, fell backwards, struck the ground, injuring back and right wrist, sustaining a fractured thoracic vertebrae and right wrist.

After parking the Fuel/Lube Truck employee walked behind the truck where he slipped/tripped on the frozen ground resulting in a sprained ankle.

At the beginning of day shift on November 18, 2008 EE was attempting to enter his dozer to begin the pre-shift examination. The door of the dozer was frozen due to snow and below freezing temperatures. When he pulled on the door handle and it broke causing him to lose his balance and fall backward off of the dozer striking his head on the ground.

Injured was helping install a French drain through old dam in King Branch; upon exiting the area he stepped on a rock. The rock twisted from beneath his foot resulting in a fall injuring his left leg. First aid was administered by two M.E.T.s.

While descending a Cat 988F Loader, the miner slipped and caught himself, causing a strain to his right shoulder.
