EE, at the beginning of the shift was climbing the mounting ladder on the 789C Rock truck and as EE went to grab the grab rail on the front of the cab, missed the rail with hand and as a result fell backwards and landed on back & right hip.

EE is a Continuous Miner Operator on 39B Section, right side. EE was tramming the miner down the Number 7 Entry, when EE stepped into a hole and twisted left ankle.

EE was carrying a can of welding rods, when EE fell, causing the palm of EE's hand to strike the sharp edge of the can and EE suffered a laceration to EE's left hand. The injury required stitches to close the wound.

EE was installing a rib bolt when EE heard a "pop" in the roof at the ATRS, as EE turned to move away EE felt pain in right knee. EE worked full duty until 9/11/20 when EE needed medical attention. EE was placed on restricted duty at that time.

Employee was climbing up the access steps of the 777C rock truck to clean the mirrors. EE lost footing and fell backwards to the ground. Employee landed on back and began experiencing pain in the neck, back, and hips as a result of the fall.

EE was dismounting the 777B rock truck to refuel. When EE stepped from the bottom step to the ground, (approximately 30"), left ankle rolled over when foot contacted the ground. EE began to experience pain and swelling in the ankle.

Employee was walking to Pre-Shift a pump, when EE stepped on a loose rock, slipped and fell to the ground. This resulted in a fracture to the tibia just above the ankle. This was diagnosed on March 7th, when EE visited the ER.

Employee was descending loader ladder using three points of contact and when left foot contacted the uneven ground they sprained an ankle.

EE was dismounting the D11T cat dozer when EE lost footing and step to the ground twisting the left knee.

Employee was changing a screen panel in the plant when employee slipped and twisted an ankle, causing a fracture.

EE was cleaning a coal spill at the 5A tailpiece with a fire hose. EE attempted to step back, when EE tripped and fell on EE's left shoulder. The area was wet, muddy and the pressure of the hose caused EE to slip and fall.

Employee was operating a shuttle car and as employee was attempting to move the C/M cable out of the roadway. Employee slipped in the wet muddy conditions and rolled ankle.

Employee was taping the clean coal chute from an elevated position when they lost their footing and fell. Employee was standing on a dryer bracket (approx. 6" wide) and slipped falling approximately 4 feet to the ground. They landed on their right shoulder and face. Employee received 5 stitches to the right eyebrow area, to close a small laceration.

Employee was walking up an incline on the punchout section and tripped falling to the ground on right side. Employee went back to the roof bolter and finished bolting the cut. A few minutes later felt a burning sensation in leg and had a knot on the side of right leg.

Employee slipped and fell from a service truck as ee was unloading wear strips for a dozer blade. EE suffered a laceration to left hand that required stiches.

Employee slipped on a piece of conveyor belt and fell on EE's right hand/wrist.

Employee was pulling pump cable through a stopping, when EE slipped on the cable and twisted EE's left knee. Employee went to the Dr on 7/23/2018

Employee was traveling up the fuel tank steps, when they lost balance, foot and hand slipped, body then twisted and impacted the truck bed and they fell to the ground. During the fall employee scraped their right side on the truck bed.

Stepping down out of the truck, slipped & fell to the ground, hurt back & elbow

EE was working was working on a dozer when loosing footing and falling to the ground.

Employee was walking towards mobile piece of equipment, with hard hat in hand, when employee tripped and fell forward striking forehead on the frame of the mobile equipment.

Employee was carrying two cans of hydraulic oil, slipped and fell backwards.

Loading holes on a shot, slipped in mud, hurt back.

Employee was walking across the parking lot when he twisted his ankle. (No tripping hazards observed)

Employee was getting on personnel carrier when he slipped and struck his lower leg against the frame of the ride.

Employee was getting off dozer when he slipped causing him to twist his left knee.

Slipped on ice while carrying box of boosters.

Employee was walking to the face in the #2 entry when he stepped between two rocks and lost his balance.

Employee stated that he stepped in a drill hole that was hid by a small amount of dirt on top of the hole. Employee reported accident on 4-12-10.

Employee was hanging miner cable and his knee twisted. He fell and hit his knee on bottom.

EE states he was walking by the draw down tunnels when he slipped and fell on snow/ice walkway. EE fell onto corner of a concrete slab on left hip and bent left thumb backward catching himself. EE missed 8 days and is currently working modified duty as of 3/1/10.

EE states he was caring a box that had blown away due to high wind and while walking with the box a high gust of wind caused the ee to lose his balance and fall. EE fractured his left wrist when he pt his hand out to brace himself for fall. EE on modified duty.

EE was walking to work area on flat ground and slipped and fell. He landed on his right hand and was complaining of pain in his thumb.

EE stated that while getting off a 777 Rock Truck, he fell from the right front ladder.

Employee states he was checking levels in the flat tank in the belt press building when he slipped and fell, fracturing his left knee cap and spraining his right wrist.

The ground was muddy around the track, the employee said that he slipped on the track rail and hurt his leg. Right thigh, there were no wittnesses and the accident was not reported until midshift.
