Miner was attempting to pull a piece of conveyor skirting from a chute when EE lost balance and fell backwards onto the catwalk. Miner sustained a lacerations to EE's left hand requiring 4 sutures to close.
Injured employee had just completed rigging a load on the 300 ton Link belt crane. EE made eye contact with the operator and asked if EE could come and talk to EE. While up on the deck talking to the Crane operator EE fainted and fell to ground and broke left clavicle. 03/17/14 Miner reported falling while descending screen house stairs, pain to right knee. 04/03/14 Miner requested to see doctor, released full duty. 05/02/14 Miner had follow up doctor apt. 06/12/14 Miner saw orthopedic doctor. Company was not notified of specialist 06/12/13 until 08/04/14. Injury meets reportable status on 06/12/14.
Miner was dismounting Bobcat 130 skid-steer loader when he lost his footing. Miner attempted to catch himself when he heard his shoulder pop, and he went to the ground. Miner was treated and released for a dislocated shoulder at the Heart of Texas Memorial Hospital ER.
