Injured was standing on the belt rail of the lowlow when he lost his footing and fell, striking his shoulder on the low low rail. The injury was not reportable until he had surgery on the shoulder on 9/16/05.

Injured was moving CM cable out of roadway when he twisted his knee
Handling miner cable when his feet gangled in wire screen the continuouse miner had just cut down. He tripped and fell striking his left knee on the mine floor. Did not go to a doctor until 6/30/05. Dr. diagnosed as bursitis and prescribed one month off work.
Injured was moving cables and curtain out of the travel path of the mobile bridge carrier when he stumbled over some rocks lying along the rib which had just been removed from the top of the continuous miner. He fell, breaking his fall with his right hand and knee. RTW on 1/16/05. Missed work 1/1905 and 1/20/05

EE WAS WALKING AROUND END OF SHUTTLE CAR WHEN EE SLIPPED AND FELL, TRIED TO CATCH SELF FROM FALL AND HURT RIGHT SHOULDER. ACCIDENT OCCURED IN #7 ENTRY AT POWER CAR WHERE SHUTTLE CAR WAS ANCHORED. 1ST RIGHT PANEL
Injured was dismounting a rail mantrip when he felt his knee fail. There were no tripping hazards or loose material involved in the incident, only body motion and possibly poor body positioning.
Injured stated while he was pulling belt with channel locks, he slipped on a wet, muddy surface, feeling a pain in left lower back and hip. This accident happened at the old 13 belt. The injured started to miss work on Oct 12, 2004.
EE WAS BENT OVER TO PICK UP A SHEET OF ROOF MAT AND STEPPED IN A HOLE IN THE MINE FLOOR. HE TRIED TO CATCH HIMSELF FROM FALLING AND FELT PULL AND BURNING SENSATION IN LOWER BACK.
Injured was crossing a belt tail piece while the belt was shut down when he slipped on the metal structure and fell landing on his posterior. The distance of the fall was approx. 18 inches.

DURING THE PROCESS OF SETTING THE NO. 2 BELT TAILPIECE THEINJURED TRIPPED OVER THE ANCHOR PIN STICKING OUT OF THE MINE FLOOR (APPROX. 4-6 INCHES) THAT WAS INSTALLED TO ANCHOR THE TAILPIECE WITH CAUSING HIM TO FALL TO THE MINE FLOOR STRIKING HIS LEFT KNEE ON A ROCK.
The injured was climbing down the short ladder in the elevator and closing the access hatch above him when his foot became caught on the bottom ladder rung causing him to fall approximately two feet to the elevator floor. NOTE: THE INJURED DID NOT BEGIN MISSING WORK UNTIL 06/14/2004 WHEN HE HAD OTRHOSCOPTIC SURGERY ON THE KNEE
MINER REACHED OUT TO GRAB THE HAND RAIL TO MOUNT A FRONT END LOADER AND SLIPPED ON A PATCH OF ICE. HE FELL TO THE GROUND AND FRACTURED THE MIDDLE FINGER ON HIS LEFT HAND.
WHEN THE INJURED WAS CLIMBING OUT OF THE SCOOP HE WAS OPERATING HE TWISTED HIS RIGHT KNEE CAUSING PAIN AND SWELLING. THE ORIGINAL DATE OF THE INJURY WAS 9-7-03, HE DID NOT BEGIN MISSING WORK UNTIL 10-10-03.
INJURED WAS LOADING BOLTING SUPPLIES ONTO THE BOLT MACHINE IN 41" OF HEIGHT. HE SLIPPED CAUSING HIM TO TWIST HIS KNEE. NOTE - INJURED BEGAN MISSING WORK ON 10/14/03.

THE INJURED WAS HELPING TO PUT BELT STRUCTURE ( TOP STRUCTURE FOR 48"BELT)IN THE #2 BELT LINE. WHEN HE TURNED TO PICK UP THE NEXT PIECE OF STRUCTURE TO BE INSTALLED, HIS FOOT SLIPPED ON THE WET MINE FLOOR ALLOWING HIS RT. KNEE TO TWIST RESULTING IN PAIN AND SWELLING TO HIS RT. KNEE.
EE WAS WALKING DOWN A BANK TO CHECK THE WATER QUALITY OF THE DISCHARGE FROM A POND. HE SLIPPED ON A WET ROCK AND FELL, FRACTURING A BONE IN HIS LEG.
THE INDIVIDUAL WAS GETTING OUT OF THE MANTRIP, WHEN HIS FEET SLIPPED FROM UNDER HIM, CAUSING HIM TO HIT HIS LOWER BACK ON THE EDGE OF THE MANTRIP. THIS CAUSED HIM TO SPRAIN HIS LOWER BACK. IND IVIDUAL DID NOT HAVE TO BE OFF FROM WORK UNTIL 02/04/02.
EE STEPPED OUT OF CABL OF LUBE TRUCK, FOOT SLIPPED ON ICY GROUND AND EE FELL TO GROUND. AS HE FELL, HE TRIED TO BREAK THE FALL AND LEFT HAND STRUCK THE GROUND. FRACTURE TO BONE IN LEFT HAND.
WHILE WALKING ON DRILL PLATFORM, THE INJURED LOST HIS BALANCE AND FELL TO GROUND LEVEL INJURING HIS RIGHT SHOULDER.
INDIVIDUAL WAS DRAGGING 3 PIECES OF WIRE SCREEN BEHIND HIM WHEN HE LOST HIS FOOTING. HE FELT HIS KNEE POP AND BECOME TENDER. BEGAN MISSING WORK ON 5-28-01.
INJURED WAS EXITING SCOOP AND FAILED TO OBSERVE THE UNEVEN BOTTOM. AS HE GOT OUT HE LOST HIS FOOTING, DOING A SPLIT, ONE LEG IN THE BACK AND ONE LEG OUT.
MINER OPERATOR WAS ON HIS KNEES WHILE OPERATING A JOY 14-CM15 BY REMOTE CONTROL. WHEN EE STARTED TO STAND UP HE SLIPPED AND STARTED TO FALL FORWARD. WHEN HE TRIED TO CATCH HIMSELF HE FELT A SH ARP PAIN IN HIS LOWER BACK.
VICTIM SLIPPED WHILE DESCENDING STEPS TO CLEAN COAL TUNNEL. FELT PAIN IN BACK.
EE COAL TRUCK DRIVER WAS EXITING HIS MACK CH 612 COAL TRUCK IN THE PARKING LOT AT THE TRUCK SHOP LOCATED AT THE PREPARATION PLANG AS HE PLACED HIS LEFT FOOT ON THE GROUND HE SLIPPED ON SOME IC E AND TURNED HIS LEFT ANKLE FRACTURING IT.
EMPLOYEE WAS CLIMBING INTO THE VESSEL REFUSE SCREEN. HE SLIPPED AND FELL 24" ONTO A 10" TUBE. THE FALL RESULTED ITNO A BRUISE TO THE CHEST.
WHILE STEPPING DOWN OFF A SCREEN TO AN UNEVEN SURFACE, THE INDIVIDUAL TWISTED HIS RIGHT ANKLE.
WHILE WALKING THE EE SLIPPED ON ICE AND SPRAINED HIS RIGHT ANKLE.
