Employee lost his footing and fell onto a metal rib strap resulting in a cut that required 5 stitches to repair.

EE was climbing down out of water truck and when he stepped on the ground his foot slipped and left knee struck step on truck. Knee started hurting after a few days and he went to the doctor on 1/14/13 Doctor put him off work pending MRI.

Stepped off stairs onto concrete pad that had a little patch of ice on it. Slipped causing him to fall and twist his knee

Employee was climbing down ladder on caterpillar 789 rock truck at quitting time and when he got to the bottom and stopped on the ground, his foot slipped and he twisted his right knee ** has been off work waiting for workers comp to approve MRI **

Coal sample operator slipped while exiting the auger control room and strained his lower back.

Employee lost footing in a mud puddle and fell backwards onto right hand. Finished shift, missed next day to have hand examined. Bruise was determined to be only injury.

Operator slipped while dismounting and fell injuring his left shoulder

Fueler was attempting to dismount a Komatsu 300 excavator when he lost his footing and fell approx. 24" to the ground landing on his back. He was transported to the hospital and released with bruising.

Mechanic was climbing onto a Cat D11 Dozer when his foot slipped off of the foot step and he fell landing on his right foot slanted inward.

Operator was dismounting from equipment to use the restroom. While dismounting he lost his footing and fell backward landing on his elbow. The impact caused a jarring motion and created pain in his shoulder.

Operator was dismounting from Komatsu Excavator when his foot slipped and he fell resulting in a bruised left buttock

Fueler was removing fueling hose on a 992 loader when he felt a sharp pain in his left calf and felt pressure on it. He fell to the ground for a minute and then continued to work.

Operator was operating a loader on the upper stockton coal pit, when he became light headed and hot. The operator stopped the loader and stepped out onto the cat walk to get some fresh air. At this time he briefly lost consciousness and fell, striking his lip and cheek on the railing. Operator was taken to Logan Hospital for Dr. evaluation.

Climbing down from hauler, made it down to 3rd step of ladder & then fell to ground.

Employee slipped off track of dozer while changing fuel filters, jarring lower back.

THE MAINTENANCE EMPLOYEE ATTEMPTED TO NOTIFY THE LOADER OPERATOR OF HIS APPROACH TO PERFORM MAINTENANCE ON THE LOADER. THE OPERATOR DID NOT HEAR HIM AND WHILE MAINTENANCE EMPLOYEE WAS CLIMBING UP THE LADDER OF LOADER, THE LOADER OPERATOR PULLED OUT CAUSING HIM TO LOSE HIS GRIP.

Climbing off of left side of 992G Loader and fell from steps.

Employee was stepping down from grader. He said his right hand gave away and let him fall onto the corner of blade and circle of the grader.

EE WAS CLIMBING DOWN LADDER FROM LOADER - LEFT FOOT ON STEP AND WHEN STEPPING DOWN TO UNEVEN GROUND, FOOT WAS CAUGHT AT STEP AND TWISTED RIGHT KNEE WHEN LOST BALANCE.

Operator was climbing down from D9 Dozer when he slipped off step, cutting left leg.

After working on fender lights of equipment, employee stepped from fender back onto deck, hitting and slightly cutting his face above the right eye. He then fell to the ground.

Employee stated he was cleaning windows on equipment and stepped back too far, fell to ground.

Sitting on ground, reached up with left arm to pull self up, lost balance fell with full weight on left shoulder. Completely dislocated left shoulder.

Ladder slipped and employee fell while fueling equipment.

EE slipped and fell on drill bench injuring lower back. There were no witnesses to the accident.

Employee had gotten off dozer, attempted to go back up and got to the second step and fell to the ground.

Employee was changing filter on dozer. He went to throw filter on the ground, oil came out of the filter & on to the step, EE fell from the step to the ground.

Employee was climbing up the ladder of a 785 Hauler when the weld on the bottom three steps broke, causing him to lose his grip and fall to the ground. employee landed on his lower back.

EE was walking behind welding truck, stumbled over a chain, fell and hit his head on a piece of triangled metal that was sticking up on a piece of flat metal.

EE went to step off the steam truck he slipped and hit his leg on the bottom step of the truck.

EE was filling up fuel truck. He climbed up ladder to see if truck was full. He was climbing down and he slipped on ladder and fell on ground. He said his right shoulder and neck were hurting. Sent him to hospital (A.R.H.)

EE WENT OVER AN EMBANKMENT TO WARN THE TREE CUTTERS THAT THEY WERE READY TO BLAST. HE FELL ON HIS BACK AND SLID DOWN THE HILL. THE SLOPE WAS STEEP.

EE WAS DESCENDING A LADDER ON THE BACK OF A JOB SITE ANDO BULK TRUCK, WHEN HIS FOOT CAME IN CONTACT WITH THE BOTTOM (1ST STEP) STEP, HE LOST BALANCE & FELL TO GROUND. THE RIGHT SIDE BRACE OF T HE STEP WAS BROKEN. THE UNSAFE CONDITION OF THE STEP WAS NOT REPORTED OR CAUTION TAKEN BY EE DURING HIS DESCENT.

EE WAS WELDING ON THE RADIATOR FRAME OF THE 992 END LOADER WHEN HE FELL ONTO THE FRAME OF THE END LOADER. HIS GRAB BAR OR FLANGE WAS HOT SELVRE.

WALKING UP HILL SLIPPED AND FELL ON ROOT BRUISING HIS RIGHT KNEE.

EE WAS CLIMBING UP THE LADDER TO HIS TRUCK. HE SLIPPED ON THE STEPS AND FELL BACKWARDS FOR ABOUT 12' LANDED ON HIS FEET AND BACK. STOVED UP LEGS AND BACK.

COMING DOWN STEPS AT THE PARTS TRAILER & HIS FEET SLIPPED ON THE TOP STEP. HE FELL AND LANDED ON BOTH ELBOWS.

EE WAS STANDIG ON THE LEVER THAT CLOSES THE DOORON THE POWEDER TRUCK. HIS FOOT SLIPPED OFF THE LEVER AND STRUCKHIM ON THE KNEE.

EE STEPPED OUT OT THE POWDER TRUCK, AND STEPPED INTO A DRILL HOLE, WHICH RESULTED IN A SPRAIN TO THE RIGHT ANKLE.

OPERATOR WAS CLIMBING OFF THE DOZER WHEN HE SLIPPED AND FELL TO THE GROUND.

CLIMBED ON THE GUARD RAIL TO CLEAN THE WINDOWS ON THE HIGHWALL DRILL. LOST HIS BALANCE & GRABBED THE DOOR & FELL TO THE GROUND. INJURYTO RIGHT SHOULDER

HE WAS MOUNTING THE FRONT END LOADER. WHEN HIS FEET SLIPPED, DUE TO THE WET MUDDY CONDITIONS, HE LOST HIS BALANCE. FELL ON HIS LEFT HAND AND INJURED HIS WRIST.

EMP BACKED HIS SERVICE TRUCK UP TO TE REAR PORTION OF A 777CAT TRUCK TO CLIMB FROM DERVICE TRUCK TO THE BED OF THE TRIPLE 7 TO RETRIEVE SOME PARTS. WHILE CLIMBING BACK DOWN HIS FEET SLIPPED CA USING HIM TO FALL ONTO THE DECKLID OF SERV TRUCK

FELL OFF DION DOZER WHILE HE WAS CHECKING OIL LEVEL,INJURING HIS LEFT LEG AND KNEE.IT WAS RAINING AND HE HAD MUD ON HIS FEET. THE METAL ON DOZER WAS ALSO WET CAUSING HIS FFET TO SLIP.

OPPERATOR WAS GETTING OFF LOADER, WHEN HE STEP OFF LADER HIS LEFT FOOT TRUNED BREAKING HIS ANKLE

EMPLOYEE WAS CLIMBING DOWN FROM THE D-6 DOZER & HIS FOOT SLIPPED ON THE CAT PAD & EMPLLYEE ALLEGEDLY FELL STRAINED HIS BACK

EMPLOYEE HAD GONE TO THE UNDERFLOW ROOM TO BACKFLUSH THE SLURRY PUMP, AND AS HE WAS LEAVINGTHE UNDERFLOW ROOM, HE SLIPPED ON SOME MUD AND FELL ALLEDGEDLY INSURING HIS BACK. THE EMPLOYEE WORKED THE FOLLOWING DAY, BUT ASKED FOR ACCIDENTREPORT TO BE FILED. THE EMPLOYEE WAS SCHEDULED TO BE LAID OFF ON 9-25-92.

THE EMPLOYEE WAS GETTING OUT OF AN 800 MACK TRUCK WHEN HE ALLEGEDLY SLIPPED AND FELL, INJURING HIS BACK, HANDS AND KNEES, HOWEVER THE EMPLOYEE WAS AWARE THAT A REDUCTION IN THE WORK FORCE WAS SCHEDULED FOR 9-25-92, AND THAT HIS JOB MIGHT BE AFFECTED BY THE REDUCTION IN THE WORK FORCE.

EMPLOYEE WAS WALKING DOWN THE STEPS AT THE SCALEHOUSE AND SLIPPED AND FELL AND INJURED HIS BACK.

EMPLOYEE STEPPED DOWN FROM LOCOMOTIVE & SLIPPED & FELL INJURING HIS BACK EMPLOYEE STATED THAT HE ALREADY HAD A SORE BACK FROM AN INCIDENT ON 04-07-92 WHILE WORKING AS A WELDER
