FELL OFF DION DOZER WHILE HE WAS CHECKING OIL LEVEL,INJURING HIS LEFT LEG AND KNEE.IT WAS RAINING AND HE HAD MUD ON HIS FEET. THE METAL ON DOZER WAS ALSO WET CAUSING HIS FFET TO SLIP.

EMPLOYEE WAS CLIMBING DOWN FROM THE D-6 DOZER & HIS FOOT SLIPPED ON THE CAT PAD & EMPLLYEE ALLEGEDLY FELL STRAINED HIS BACK
EMPLOYEE HAD GONE TO THE UNDERFLOW ROOM TO BACKFLUSH THE SLURRY PUMP, AND AS HE WAS LEAVINGTHE UNDERFLOW ROOM, HE SLIPPED ON SOME MUD AND FELL ALLEDGEDLY INSURING HIS BACK. THE EMPLOYEE WORKED THE FOLLOWING DAY, BUT ASKED FOR ACCIDENTREPORT TO BE FILED. THE EMPLOYEE WAS SCHEDULED TO BE LAID OFF ON 9-25-92.
THE EMPLOYEE WAS GETTING OUT OF AN 800 MACK TRUCK WHEN HE ALLEGEDLY SLIPPED AND FELL, INJURING HIS BACK, HANDS AND KNEES, HOWEVER THE EMPLOYEE WAS AWARE THAT A REDUCTION IN THE WORK FORCE WAS SCHEDULED FOR 9-25-92, AND THAT HIS JOB MIGHT BE AFFECTED BY THE REDUCTION IN THE WORK FORCE.
EMPLOYEE WAS WALKING DOWN THE STEPS AT THE SCALEHOUSE AND SLIPPED AND FELL AND INJURED HIS BACK.
EMPLOYEE STEPPED DOWN FROM LOCOMOTIVE & SLIPPED & FELL INJURING HIS BACK EMPLOYEE STATED THAT HE ALREADY HAD A SORE BACK FROM AN INCIDENT ON 04-07-92 WHILE WORKING AS A WELDER
