Tech walked into sample building to remove bag and stepped on loose floor grating and fell through with left leg up to shin below knee. The grating returned to position trapping leg. No skin damage; ankle sprain resulted. Grating may have been loosed due to vibration from sampling system or not replaced during recent construction. Grating in walkway path.

While walking to the Briquetter Building, employee stepped off of the pugmill concrete pad, which is approximately 6-8 inches above ground level and rolled his right ankle. This resulted in a sprained ankle and time off work.

Employee was attempting to remove a large lump of coal from the raw coal feeder with a jack hammer. The employee stepped on the handrail resulting in him falling onto the jack hammer.

Employee was performing maintenance on the A side briquetter unit and while performing this task tripped over a crib block. As a result, the employee fell into the belt structure and fractured his right arm (humerus).

Employee was moving miner cable out of roadway and stepped into a hole and felt pain in back and hip.

Employee was walking across the storage yard and twisted his right ankle, which caused a sprain. Employee's doctor put him off for 3 days, therefore resulting in a lost time accident.

Employee was dismounting D9 dozer when his hand slipped off the handrail causing him to fall off the dozer cab access platform,employee landed upright on to the ground below and fractured his right ankle.

Employee was taking air reading on top overcast. Coming down steps on overcast he midjudged last step causing him to fall backward hitting on his lower back and tailbone.

Employee walked between line to hang halo spray bar. He started to rehang halo spray. When entering spot to reach chain, caught right boot toe on toe plate. Felt some knee pain but no medical attention needed at that time.

Ee was climbing up stairs of forklift when his left foot slipped off of top step his left leg went under forklift. He was holding onto both handles when slipped. Right knee ligament was strained.

Employee was helping roof bolt machine operator move to another location and strained lower back (slip & fall). (Employee starting missing 03/13/06.)

EE WAS WALKING DOWN FROM LIGHT HOUSE TO MINE PORTALS AND SLIPPED ON FROZEN HAUL ROAD. HIS LEG WENT BACK UNDER HIS BODY - BROKE RIGHT ANKLE.

Employee was geting out of repairman's buggy, slipped and fell in mud.

EE WAS TAKING BOLTS OUT OF PLATE ON CRUSHER. HE WENT TO MOVE OUT THE THE WAY TO POSITION HIMSELF TO PULL THE PLATE. THE BOTTOM OF HIS SHOES CAUGHT ON THE BASE OF THE CRUSHER, RESULTING IN TWISTING OF LEFT KNEE. FIRST DAY OFF WORK 1/31/06.

Employe was climbing off dozer, felt sharp pain in lower back.

MISSED STEP ON STAIRS IN CONTROL ROOM. FELL AND HIT HEAD.

Was getting out of deck of scoop. Area was muddy, twisted his left knee.

Employee was getting out of mantrip, felt pain in lower back, starting missing work 12/09/2005

Employee was climbing onto drill when his foot slipped on step resulting in a pulled muscle to the left shoulder.

Employee tripped over a 3" toe plate while walking backwards giving hand signals to a crane operator. Employee fell on the existing catwalk bruising his knee, ribs, and shoulder. He was OK until a few days ago, decided to go back to the doctor on 9/15. Doctor put him on modified duty, but no modified duty is available, therefore it's a lost time.

Employee was installing belt hangers with Fletcher drill when he stepped across T-bar and twisted his left knee.

Employee was shoveling tailpiece and fell backwards and landing on a piece of rock. Started missing work 3/17/2005.

EE was cleaning in the draw off tunnel by the feeder #2 walkway above tunnel floor. After cleaning was complete, EE was preparing to climb down the fixed ladder access. Before climbing down the ladder, the EE needed to reconnect the safety chain and while doing this, slipped and fell against the tunnel wall, injuring his right shoulder.

Employee was walking down steps from safety office and fell, cutting his right arm.

Employee was walking across section and slipped and fell on loose material. (Started missing work 10/18/2004).

EMPLOYEE WAS STANDING ON REAR BUMPER OF PICKUP PUTTING COAL SAMPLES IN BED OF TRUCK. HIS BOOTS WERE MUDDY WHEN HIS FEET SLIPPED OFF BUMPER FALLING ON TOP OF TAILGATE, STRIKING HIS CHEST.

EE WAS WALKING TO HIS TRUCK WHEN HE STEPPED ON A BOARD WHICH CAUSED HIM TO TRIP. HE FELL AGAINST HIS TRUCK AND HIS LEFT ARM WENT BETWEEN THE RUNNING BOARD AND THE TRUCK BODY FRACTURING HIS SHO ULDER.

EMPLOYEE HAD POURED A BAG OF CHEMICAL INTO A FEEDER BOX. HE WANTED TO SEE HOW MUCH CHEMICAL WAS IN THE FEEDER. HE FOUND A METAL BOX TO CLIMB ONTO LOOK INTO THE FEEDER. HE LOST HIS BALANCE AND FELL TO THE FLOOR STRIKING HIS HEAD AND RIGHT HIP.

EE STATES HE WAS CLEANING THE GUARD SHACK, SWEEPING THE FLOOR. HE HAD TO MOVE A SMALL COOLER TO SWEEP UNDER IT, HE STEPPED ON THE FLOOR & IT GAVE WAY W/HIM. HIS RT. FOOT WENT THROUGH THE FLOOR WHILE HIS LEFT FOOT/LEG WENT UNDERNEATH HIM.

INJURED WAS IN STOOPED POSITION ALIGNING SPADS WHEN THE SHUTTLE CAR APPROACHED WHERE HE WAS WORKING WHEN INJURED REALIZED OPERATOR WAS NOT GOING TO STOP, HE GOT OUT OF THE PATH OF THE SHUTTLE CAR. HOWEVER, HE WAS CAUGHT BY THE SHUTTLE CAR CABLE AND THROWN AGAINST THE MINE ROOF AND CAUGHT AGAINST THE RIB.

EE FELL DOWN GOING TO MAN TRIP AND CUT IS RIGHT 5TH DIGIT ON SHARP ROCK, REQIRING 6 STITCHES.

EE BECAME ENTANGLED IN A WATER HOSE THAT HE WAS USING, LOST HIS BALANCE, FALLING ON LT. ELBOW.

EE BECAME FRIGHTENED BY LOUD NOISE NEAR HIM HE THEN JUMPED FROM STEP LADDER PLACED NEAR COAL HOPPER & BELT LINE. HE WAS APPROX 21/2 FT FROM GROUND HIS RIGHT FOOT COMES IN CONTACT WITH A LUM P OF CAOL & SPRAINS ANKLE. THIS INCIDENT WAS NOT REPORTED TO MGMT UNTIL 10/29/01

EE WAS DESCENDING STAIRS FROM SECOND FLOOR WHEN HIS FEET SLIPPED, CAUSING HIM TO FALL. HE STRUCK HIS LEFT ELBOW, RESULTING IN A LACERATION REQUIRING SUTURES.

EMPLOYEE WAS INSTALLING V-BELT AND SLIPPED AND FELL 7FT. OFF BELT.

EE FELL TO GROUND WHILE DESCENDING STEPS LOCATED ON COUNTERWEIGHT ON 988-F LOADER, IN THE ACT OF REFUELING.

EMPLOYEE WAS GOING TO HIS CAR, SLIPPED ON SOME ICE ON SOME STEPS, FELL INJURING HIS BACK.

EMPLOYEE WAS STEPPING UP ON CHECK A DOOR AND HURT HIS RIGHT KNEE.

EE WAS ON AN EXTENSION LADDER TRYING TO FREE AN OVERHEAD ROLL-UP TYPE DOOR THAT WAS STUCK. THE LADDER SLIPPED CAUSING HIM TO FALL. HE LANDED ONHIS FEET BUT THE IMPACT CAUSED A FRACTURE TO THE LEFT HEEL.

EMPLOYEE WAS EXITING THE BINDER PUMP CONTAINMENTAREA WHEN HE SLIPPED & FELL ON A PATCH OF ICE. ACCIDENT NOT REPORTED BY EMPLOYEE UNTIL 1/8/01.
