Tech walked into sample building to remove bag and stepped on loose floor grating and fell through with left leg up to shin below knee. The grating returned to position trapping leg. No skin damage; ankle sprain resulted. Grating may have been loosed due to vibration from sampling system or not replaced during recent construction. Grating in walkway path.

Mostrando los 40While walking to the Briquetter Building, employee stepped off of the pugmill concrete pad, which is approximately 6-8 inches above ground level and rolled his right ankle. This resulted in a sprained ankle and time off work.
Employee was attempting to remove a large lump of coal from the raw coal feeder with a jack hammer. The employee stepped on the handrail resulting in him falling onto the jack hammer.

Ee was climbing up stairs of forklift when his left foot slipped off of top step his left leg went under forklift. He was holding onto both handles when slipped. Right knee ligament was strained.

EE WAS WALKING DOWN FROM LIGHT HOUSE TO MINE PORTALS AND SLIPPED ON FROZEN HAUL ROAD. HIS LEG WENT BACK UNDER HIS BODY - BROKE RIGHT ANKLE.

Employee tripped over a 3" toe plate while walking backwards giving hand signals to a crane operator. Employee fell on the existing catwalk bruising his knee, ribs, and shoulder. He was OK until a few days ago, decided to go back to the doctor on 9/15. Doctor put him on modified duty, but no modified duty is available, therefore it's a lost time.

EMPLOYEE HAD POURED A BAG OF CHEMICAL INTO A FEEDER BOX. HE WANTED TO SEE HOW MUCH CHEMICAL WAS IN THE FEEDER. HE FOUND A METAL BOX TO CLIMB ONTO LOOK INTO THE FEEDER. HE LOST HIS BALANCE AND FELL TO THE FLOOR STRIKING HIS HEAD AND RIGHT HIP.
EE STATES HE WAS CLEANING THE GUARD SHACK, SWEEPING THE FLOOR. HE HAD TO MOVE A SMALL COOLER TO SWEEP UNDER IT, HE STEPPED ON THE FLOOR & IT GAVE WAY W/HIM. HIS RT. FOOT WENT THROUGH THE FLOOR WHILE HIS LEFT FOOT/LEG WENT UNDERNEATH HIM.
INJURED WAS IN STOOPED POSITION ALIGNING SPADS WHEN THE SHUTTLE CAR APPROACHED WHERE HE WAS WORKING WHEN INJURED REALIZED OPERATOR WAS NOT GOING TO STOP, HE GOT OUT OF THE PATH OF THE SHUTTLE CAR. HOWEVER, HE WAS CAUGHT BY THE SHUTTLE CAR CABLE AND THROWN AGAINST THE MINE ROOF AND CAUGHT AGAINST THE RIB.
EE FELL DOWN GOING TO MAN TRIP AND CUT IS RIGHT 5TH DIGIT ON SHARP ROCK, REQIRING 6 STITCHES.
EE BECAME ENTANGLED IN A WATER HOSE THAT HE WAS USING, LOST HIS BALANCE, FALLING ON LT. ELBOW.
EE BECAME FRIGHTENED BY LOUD NOISE NEAR HIM HE THEN JUMPED FROM STEP LADDER PLACED NEAR COAL HOPPER & BELT LINE. HE WAS APPROX 21/2 FT FROM GROUND HIS RIGHT FOOT COMES IN CONTACT WITH A LUM P OF CAOL & SPRAINS ANKLE. THIS INCIDENT WAS NOT REPORTED TO MGMT UNTIL 10/29/01
EE WAS DESCENDING STAIRS FROM SECOND FLOOR WHEN HIS FEET SLIPPED, CAUSING HIM TO FALL. HE STRUCK HIS LEFT ELBOW, RESULTING IN A LACERATION REQUIRING SUTURES.
EMPLOYEE WAS INSTALLING V-BELT AND SLIPPED AND FELL 7FT. OFF BELT.

EMPLOYEE WAS EXITING THE BINDER PUMP CONTAINMENTAREA WHEN HE SLIPPED & FELL ON A PATCH OF ICE. ACCIDENT NOT REPORTED BY EMPLOYEE UNTIL 1/8/01.
