Mostrando los 49An employee was standing on a step and the v belt guard was facing the screen. EE turned to get a new screen rail and stepped off the v belt guard. EE grabbed the shaker with EE's right arm as it was still over the top of the shaker and caught self preventing falling to the ground.
Employee was exiting a motor grader, EE was facing the steps of the grader while holding onto both handles. As EE was completing the dismount to the ground, EE's right foot slipped on uneven surface, and EE twisted right ankle which caused EE to fall.
Employee arrived at work around 7am and parked EE's vehicle in the employee parking lot near a loader that EE was going to use for the day. EE exited EE's vehicle and walked over to the loader to check the fluid and start it. As EE was walking around the loader EE slipped and fell on some ice that was hidden by some fresh snow.
Employee was descending a set of metal stairs from a screen deck when they tripped causing them to land awkwardly on the next lower step. The sudden jarring caused discomfort in their lower back.
Employee was stepping off the D8D7 dozer and as employee reached the ground with left foot first, employee rotated to the left and felt a pop in left knee. The ground was just graded flat.
Employee was standing on the platform next to his entry door on 980M Cat Loader to clean the 45 degree window off when his foot slipped off the platform; he was holding onto the mirror as this happened so he caught himself with his right arm on the mirror causing a strain to his bicep.
Employee was walking from the office to the parking area when she slipped and fell on some ice on the sidewalk. Dislocated right shoulder.
Employee had just finished loosening the tail pulley. When he moved from the sitting position to standing up, he felt a slight pop in his knee, spraining his left knee.
He stepped off the back of Michigan CAT's lube truck while facing forward and missed the blocking on the ground that was used for steps and stepped down 2 feet twisting his left knee causing a strain to his knee.
Employee was lifting himself out of the crusher housing, when his feet came out he turned his body to the left and felt a pop in his left rib cage.
Employee was walking onto Dredge from floating conveyor, he stepped onto Dredge and stepped into man hole for pontoon. This cover was removed by trespasser over the weekend. Employee fell onto his left side and lower ribs, contusion to left ribs.
Ice was covered with snow and employee didn't see ice. Employee slipped on ice injuried right arm & shoulder. Employee was hurt on 12/11/09 and reported incident to management that day, at this time employee did not feel he needed to see doctor. On 1/4/10 reported pain in his shoulder, was sent to clinic and was put on restricted duty along with prescription medicine.
Employee stated that as he was exiting the truck his foot slipped off the step and he landed on the ground in an awkward maner. He later stated that his knee was sore. He was taken to the clinic as a pre caution where it was diagnosed as a minor strain. Return to work regular job.
Employee was in Scale office before shift. Stood up from chair to walk to fax/copier, took one step and knee gave out. (Did not hit anything, twist or turn). Employee said she had been exercising a lot using "WII Fitness" program.
EE missed last step coming down from ladder and fell 2' to the ground. His ribs landed on a frozen dirt clod, causing rib contusion.
Employee had mud on his boots. As he dismounted his loader, his foot slipped off the bottom rung. He fell to the ground spraining his right ankle.
Employee slipped off ladder while climbing to get on top of a compressor room to get fluorescent light bulbs. As he slipped, his right wrist snagged on garage door rail cutting approx 2" of skin back which required 20 stitches.
Employee was putting a wiper on grizzly conveyor when it slipped out of cradle hitting him on the chin and then hit ladder, knocking him off. The ladder hit him below the knees on both legs. Originally this was a first aid only. Employee's second check up with doctor (10/22/07) revealed swealing of his left knee at which time he was put on restricted duty.
While climbing down the ladder on loader, the bottom step broke causing employee to bump her left knee on the step above. She was given Motrin 600 mg. #20 one q6h as needed for pain at the health center, although she did not take them and threw them in the garbage. Lead coding clerk from MSHA told us that we should report this as a first aid incident only.
Jumped off of conveyor - inguinal hernia on right side.
Loader operator climbed down from cab of loader, he turned around, once he got to the ground & started to walk away from the loader he stepped into a hole & fell forward. His left knee struck a 5"rock that way laying on the ground.
EE was walking around fueling station to do some clean-up & twisted his right ankle. He said no specific cause & the ground was level. Our Safety Director spoke with a Coder at MSHA Office of Injury & Employment Information on 4/6/06 about incident & was told it would not be reportable. Site was inspected on 4/25/06 & MSHA Inspector told Plant Manager that 7000-1 must be completed.
Employee was stepping down from truck onto the ground. He lost footing and landed awkward onto the ground injuring his ankle.
While walking up hill in soft sand, employee's left knee buckled. Clinic gave employee a RX for #21, 800 mg Ibuprofen tablets as well as other restrictions that are not necessarily inhibiting him from doing his normal job.
While exiting front end loader, employee was climbing down ladder and he fell backwards hitting his head on the ground. Employee had multiple objects in his hands which caused him to loose his grip.
WHILE CLIMBING DOWN BACK OF LOADER HIS FOOT CAUGHT ON STEP, CAUSING HIM TO PUSH UP WITH HIS OPPOSITE LEG STRAINING HIS ABDOMEN WALL.
While climbing out from underneath the Pioneer hopper, employee slipped on loose material. He caught himself with both hands and injured his left wrist in the process.
AFTER INSPECTING WASH PLANT SPRAY BARS, EE LOST HIS FOOTING AND CAUGHT HIMSELF WITH HIS LEFT HAND. THIS CAUSED SHOULDER AND UPPER ARM STRAIN.
CHANGING SCREENS ON PRIMARY SCREEN, WHEN EE FELLOFF OF TOP OF SCREEN AND LANDED ON WALKWAY ON HIS BACK.
EMPLOYEE WAS HELPING CARRY A TOOL CABINET & HIS FOOT STRUCK A SHELF SUPPORT LYING ON THE GROUND CAUSING HIM TO TRIP. THE CABINET LANDED ON HIS LEGS, PUSHING HIS ANKLE INTO ONE OF THE SUPPORTS, CAUSING A PUNCTURE WOUND TO HIS ANKLE. THIS INCIDENT REQUIRED 3 STITCHES TO THE INSIDE OF THE LEFT ANKLE.
EE WENT TO STEP ONTO WET STEPS AND HER FOOT SLIPPED. SHE FELL ON HER RIGHT KNEE AND CUT IT AS WELL AS SWELLING THAT STARTED TO OCCUR. SHE WENT TO THE CLINIC THAT DAY AND WAS PUT ON RESTRICTED DUTY.
EE WAS REMOVING BOLTS ALONG HF-6 CONVEYOR. HE WAS USING A PUNCH & A 3 LB HAMMER TO DRIVE OUT THE 3/4" BOLTS THAT HOLD THE 20' SECTIONS TOGETHER. WHEN HE HIT THE PUNCH WITH THE HAMMER, HIS LEFT FOOT SLID DUE TO THE SNOW ON THE GROUND & HE FELT SOMETHING GIVE IN HIS LOWER BACK. HE CONTINUED TO WORK & HIS BACK GOT WORSE. THE SUPERVISOR WAS CALLED @ 10:20 AM AT WHICH TIME HE COMPLAINED
EMPLOYEE SLIPPED GETTING DOWN FROM LOADER & FELL BACKWARDS, HITTING HIS HEAD ON THE TIRE OF ANOTHER PARKER LOADER.
EMPLOYEE WAS CLIMBING UP THE RIGHT REAR LADDER OF E1-91 LOADER AND LOST FOOTING BECAUSE OF THE DRIED MUD. THE MUD BROKE AWAY, CAUSING EMPLOYEE TO FALL BACK, HITTING HIS HEAD ON THE OPEN TOOL BOX DOOR OF HIS SERVICE TRUCK, WHICH WAS PARKED NEXT TO THE LOADER.
WHILE STEPPING DOWN FROM PICKUP TRUCK, EE SLIPPED & HIT HER RIGHT LEG ON THE EDGE OF THE TAILGATE. SHE DIDN'T GO TO THE CLINIC AT THE TIME OF THE INCIDENT & RETURNED TO WORK ON 1/25/02. ON 3/1 4/02, EE REQUESTED TO GO TO THE CLINIC DUE TO A SORE LEG. NO RESTRICTIONS WERE ISSUED BUT KETOPROFEN 75 MG #30 WAS PRESCRIBED, ALSO AN ELASTIC KNEE SUPPORT. CLINIC SUGGESTED MRI OF KNEE.
EE WAS CLIMBING UP ONTO LOADER WHEN HIS FOOT SLIPPED BECAUSE OF FROST. AS SOON AS HE CAUGHT HIMSELF, THE SUDDEN JERK ON HIS BODY CAUSED AN ABDOMINAL WALL STRAIN.
GROUND UNEVEN TURNED ANKLE (LEFT).
PRE SHIFT EQUIPMENT INSPECTION, SLIPPED INTO NEARBY DITCH.
CLIMBING OFF CONVEYOR, FELL ON A CONCRETE BLOCK.
WHILE STANDING ON A PALLET EMPLOYEE STEPPED OVERA BOX AND HIS LEFT FOOT SLIPPED OFF THE PALLET &ONTO A BOARD WITH NAILS STICKING OUT.
EE SLIPPED ON ICY AREA WALKING TO HIS LOADER. THAWED CONDITIONS THE PREVIOUS DAY RE-FROZE OVERNIGHT. AREA WAS SEMI-DARK THAT EARLY IN THE AM BEHIND THE MOUNTAIN WHERE THE SUN HADN'T SHONE YET. WHEN EE SLIPPED HIS RIGHT LEG WENT FORWARD CAUSING A SPLIT. HE WENT DOWN ON THE LEFT LEG AND FRACTURED HIS LEFT LEG FROM IMPACT AT THE KNEE AND WEIGHT OF BODY.
WHILE EE WAS WORKING ON A PIECE OF EQUIPMENT (CR-10), HE SLIPPED ON THE ICE AND FELL ON HIS SIDE, JERKING HIS BACK. EE DID NOT VISIT DR/CLINIC UNTIL 2-15-01.
WHILE UNLOADING TROUGHING IDLERS OFF THE BOOM TRUCK DECK FOR INSTALLATION ON THE ROHR DREGE #1 CONVEYOR, EE SLIPPED ON THE 1 INCH OF FRESH SNOW AND FELL TO THE GROUND FROM GROUND LEVEL. THIS C AUSED THE IDLER HE WAS CARRYING TO STRIKE HIS RIGHT ARM AGAINST THE GROUND.
EE WAS ACTING AS HOT SAND PLANT OPERATOR. ON THIS PARTICULAR DAY, ALL AREA SCHOOLS WERE CLOSED DUE TO ICY CONDITIONS. THE AREA AROUND THE ELECTRICAL CONTROLS, FEEDER AND WHERE HIS VEHICLE WAS PARKED HAD BEEN SANDED. DURING THE SHUT DOWN PROCESS, EE WENT TO SHUT OFF THE PROPANE CONTROL AND FELL ON HIS LEFT SIDE SINCE THE AREA WAS NOT SUFFICIENTLY SANDED.
EMPLOYEE SLIPPED ON ICE WHILE WALKING AROUND IN PLANT AREA. HE LANDED ON HIS BACK.
EE WAS CLIMBING THE LADDER ON THE ROHR DREDGE WITH TOOLS IN HIS HAND. THERE WAS ALSO SILT FROM BAD MATERIAL ON THE LADDER THAT HE WAS CLIMBING. EE SLIPPED AND FELL BACKWARDS ONTO DECK.
CLIMBING DOWN FROM WATER TRUCK - SLIPPED ON STEPS.
THIS IS A RE-OCCURENCE OF ACCIDENT ON 1-17-00, WHEN EMPLOYEE FELL OFF TRACK AND HIS RIGHT BUTTOCK LANDED ON A BOLDER CAUSING PAIN IN HIS RIGHT LEG. ON 8-4-00 AND 8-5-00 EMPLOYEE WAS STRINGIN G CABLE ON DRAGLINE CAUSING IRRATATION TO RIGHT BUTTOCKS AND LOWER BACK. THIS IS THE SAME AREA FROM ACCIDENT ON 1-17-00.
