Emp was coming down a 5 foot ladder (used to access haul truck eng compartment). While coming onto the ladder emp put their foot on the 3rd rung. Emp's foot was wet from having (a few minutes ago) gone to the truck for a tool. When emp's foot slipped (wet) the ladder leaned hard to one side and fell over. Emp fell 3 feet to the floor and fractured (hair line) right radius.
Emp. slipped and fell on 6-25 - diagnosed as a sprain. Emp off on 6-26 (working 4 10's) on Saturday 6-27 Dr called emp to see ortho Dr on 6-29. On 6-29 Emp - diagnosed with hairline fracture and told to be out of work. Emp became a lost time accident on 6-29-2020 with a fracture to the tibia of rt leg.
While getting off the loader, the employee, slipped on step (did not fall) and strained back. Employee went back for a follow up on 11/22/2019 and P.T. was recommended... on 11/25/2019 Employee signed up for P.T. --- making this injury reportable.
Employee slipped climbing off fuel truck ladder and fell 2.5 feet to the ground. Employee bruised left elbow and swelling soon followed. Employee went to clinic and relieved treatment and physical therapy (2 sessions) Employee was released to full duty with no lost time or restrictions.
Employee tripped over guide wire along ramp going into a trailer. Employee fell onto left knee.

Getting off Excavator employee (who is over weight and has arthritis in both knees) fell on EE's bottom and had trouble getting up. Employee used 3 points of contact and when EE's rt foot touched the ground EE released EE's hands and EE's knee went out.
Employee injured knee when employee slipped climbing up an excavator to fuel it. This slip / injury happened on 6/2/17. Employee continued to work (after receiving 1st Aid) and worked without issue until employee woke-up on July 5th in pain. On July 5th employee had knee drained and was back to full duty on the 10th of July.
EE was walking toward a pump station when ee tripped over a 2" water hose, causing ee to stumble and land on wet sand awkwardly with left foot causing a six inch crack in left fibula.
Employee was climbing on to the 1st step of haul truck and went to place wheel chalk in place....ee slip and fell 18 inches and fractured left wrist
Inspecting the Dozer, Employee was dismounting the Dozer and slipped, spraining EE's left knee. Employee reported the sprain to management immediately. Employee and management decided in house 1st aid was enough. It was later decided (on 3-25-16) to seek professional help.
Employee was standing on unstable ground...lost balance and grabbed for near by pipe for support. Employee's ring caught the angle iron that was supporting the pipe and almost tour his ring finger off.

EMPLOYEE WAS STANDING ON A CRUSHER FRAME (APPROXIMATELY 3"-4" HIGH) ATTEMPTING TO REMOVE A ROCK FROM THE HOPPER. HE MOVED ONE FOOT TO RELEASE THE ROCK WHEN HIS OTHER FOOT SLIPPED CAUSING HIM T O FALL ON HIS RIGHT SIDE.
EMPLOYEE STEPPED OUT OF HIS TRUCK AND HURT HIS KNEE.
EE WAS ATTEMPTING TO WASH OUT A SCREW CONVEYOR AND SLIPPED STRIKING HIS RIGHT KNEE ON A SCREW FLIGHT.
EE WAS CLIMBING UP A LADD4ER & HIT HIS KNEE
