Employee was leaving their car in the parking lot and twisted their right ankle. After an x-ray it was found that the employees fibula was chipped due to this.
An employee was climbing rope ladder to get into the vessel when the screw clamp that was holding it in place came off. The employee fell 46 inches and hurt their knee and bumped shoulder. This resulted in a shoulder sprain.

Employee turned to the right and walked away from chained doorway when right foot slipped on ice. Left leg bent underneath employee, left arm slipped out over employee's head trying to stop fall.

The worker was part of a construction crew pouring at the mine as part of a new facility addition. The worker was walking across steel rebar matting that was part of the concrete form being poured. As the worker walked across the matting EE lost footing and fell forward. As EE fell, left forearm struck the edged of a section of rebar, caused cut to the L forearm, 10 stitches.
Employee was walking in an aisle way of the raw material storage building and slipped on ice causing a strain to left knee. The injury was not initially reported until 9 days later. After consultation it required surgery to repair a torn meniscus, performed on 6/15/16.

Employee walking to the job through alleyway. The walkway was icy and sloped. Knee went out from under him and he fell. Loss of strength in a bent knee. Can't crouch to work. Left for knee surgery on 11/24/2009.
EE was walking across the plant to go to lunch. Stepped right foot into a small pothole in the yard area under the kiln. His ankle rolled and he tightened up to catch himself. Felt immediate pain in his lower back. Went to the doctor the next day in the a.m. Doctor put him on restricted duty for 3 days.
On 7/30/08,EE slipped on mill feed floor where drag conveyor comes through floor.EE braced fall with right hand by placing on cement cooler, which injured hand.EE reported injury at start of shift on 7/31/08.Employee went to doctor on 8/13/08 and received physical therapy referral. Employee started physical therapy on 8/25/08.
