Employee was using the elevator to go up to the 12 pack when the elevator stopped due to a limit switch failing.

An employee attempted to break a rake handle by stomping on it. During the attempt, the handle recoiled and struck the employee in the side of the stomach. The pain from the handle caused them to lose consciousness while walking back to the break area. The employee fell down approximately 10 stairs, sustaining two lacerations to the left knee requiring 19 staples.

The employee removed the calibration weight and it dropped onto the employee's hand, pinching it between the top of the guard and the weight. As a result, the employee sustained a cut that required 8 stitches and a hairline fracture to left hand ring finger.

Employee lifted 80-170 lb liners for mill #3 outage and developed right-hand pain after a popping sensation. On May 8 the tendon tightened again; diagnosed with trigger finger. Surgery occurred 1/8/26, with no use of the right hand until 3/11/26. Employee has returned to work with restrictions on 2/2/2026.

While pulling liners, the liner broke free hitting the employee in the face. Requiring the employee to get stitches.

Tripped on lip of grating and fell forward onto left knee.

employee was cutting vac hose with a knife and it inadvertently slipped into their right index finger requiring 6 stitches.

Contractor was going up in the finish and adjusted backpack resulting in the safety trip to engage in elevator.

Employee was working on driving wedges between liners when a chunk of liner broke and cut EE's left index finger. Employee was seen by the onsite first aiders and received cleaning and a bandage and was sent back to work. On 09/25/24 the employee received medical attention to clean the cut out due to an infection.

A fire occurred on the 5th floor of the pre-heat tower where Alternatives Fuels enters the process.

The fire originated from the kiln pushing material after the calciner was lost. This loss of the calciner enabled unprepared hot material to pass through the kiln and cooler without sufficient cooling time. Consequently, when these materials reached the belt without proper cooling, they ignited and caused the fire.

Contractors accidentally pressed the e-stop and contractors where stuck in the west Alamac for 2 hours before talking to the elevator mechanic. The mechanic told us to try the e-stop which fixed the situation.

The door lock on the alamac elevator was stuck due to a electrical part that failed. This was replaced and the elevator returned to service.

The employee, wearing glasses and a face shield, was in the process of removing the kiln shelf feed door for inspection when dust unexpectedly puffed into both eyes from the opening on the feed shelf. The employee was prescribed eye drops on march 4th, 2024.

On September 25th, 2023, at 1:00 PM, an incident occurred at Finish Mill #3 during a repair operation on the water lance. The employee sustained an injury to EE's right ring finger when EE accidentally pinched it between a pipe EE was carrying and a handrail. The injury was assessed as requiring medical attention and received two stitches.

Employee was leaving their car in the parking lot and twisted their right ankle. After an x-ray it was found that the employees fibula was chipped due to this.

An employee was cutting vac hose and cut themselves. The employee received 2 stitches on right thigh.

The employee was installing new buckets on FM#3 discharge elevator. The bucket weighted roughly 25lbs and was lifting the bucket at chest level. Employee has been doing physical therapy from 5/25/2023 until 1/31/24. The doctors diagnosed EE with a torn rotator cuff and went into surgery 1/31/24.

Employee was lifting cribbing to set the mill motor in place on a flat bed trailer. When the employee was lifting the cribbing (approximately 100lbs) employee felt knee hyperextended backward resulting in pain. Employee was seen on January 6, 2023 by the orthopedics and was given a cortisone shot.

The safety switch on the elevator was activated. Two contractors were entrapped in the elevator at the finish silos until Otis arrived to bring the elevator back to the bottom floor.

The #3 boom boat loading sock counterweight was stuck in the up position and needed to be freed up. The employee decided to pull on the cable to free the counterweight. When the counterweight broke free it pulled employee's hand towards the pulley. This resulted in cutting the tip of the pinky off.

Employee aggravated lower back / Left buttock from operating the truck over time. Employee was looked at by our occupational doctor on 8/30/2022 and given time off, incurring an LTI.

Two contractors were using the elevator to head to the top of the finish silos when it stopped in-between floors. Otis elevator was called to free the miners. The contractors made it out of the elevator at 11:50am.

Employee was swinging a sledge hammer to close a swing gate in the finish silos. The employee missed the target resulting in hitting their pinky and causing a fracture.

The employee was walking out of the doorway in the finish silos. The door that was propped open closed due to a gust of wind and pinched employee's finger between the door frame and the door. The employee was diagnosed with a fracture and a laceration to the left index fingertip.

An employee was swinging a sledgehammer to knock out pins on the clinker cooler conveying elements. The employee noticed a sharp pain in the left hand. The employee was then diagnosed with hook of the hamate fracture.

An employee was climbing rope ladder to get into the vessel when the screw clamp that was holding it in place came off. The employee fell 46 inches and hurt their knee and bumped shoulder. This resulted in a shoulder sprain.

Employee slipped on a patch of ice while getting out of personal vehicle in the employee parking lot and landed on left hand causing a broken wrist.

Employee was assisting in moving an intake pipe for the 1302 fan. The intake pipe tipped over and pinched the employee's finger between the fork of the telehandler and intake pipe. Employee received 6 stitches and fractured the upper part of middle finger.

Employee was cutting the head of a bolt to remove a machine guard with a portable grinder. The grinder kicked and struck the employee in the lower left thumb causing an injury that required six stiches.

The employee was instructed to go up to the 316 belt and clear debris. The employee proceeded to the platform to perform the function. The employee went to place the shovel on the platform. While putting the shovel up on the platform, the shovel made contact with the hand rail and bounced back with the flat part of the metal shovel pan hitting the employee in the nose.

EE was installing buckets on a bucket elevator. One of the buckets pinched EE's left ring finger causing a cut through EE's leather gloves. EE received stitches to the finger.

Employee smashed right middle finger with sledge hammer. Employee was hammering #1 slide gate on the 12 pack.

Employee was working on the M-1 belt and pinched the left third finger between the chain and the belt.

The employee operates as a lab tech, was out walking, they took a step in the main yard and twisted left knee. The event occurred on 1/3/21 but not reported until 1/14/21. After multiple follow-ups, surgery was scheduled on 8/1/2021. First lost day of work occurred on 8/2/2021.

The employee was carrying a 170lb coupler up 5 flights of stairs in the PH tower on rt. shoulder to repair the kiln feed elevator. The employee reported a shoulder strain injury and received first aid on 12/20/20. Employee requested on 1/19/21 to be evaluated by medical professional and placed on restrictions. Employee received surgery on 6/30/21 and experienced lost time.

Removing bottom pan of airslide, leg in wrong area, pan came down and hit leg and bent foot causing a sprain to right big toe and ankle.

1 contractor descending in the PHT Personnel Elevator attempted to open the door to stop the car, instead of riding it to the floor that was called. This incorrect use of the elevator trips a micro switch in the door latch and stops the elevator from working. Elevator company was called to free the miner. Contractors signed a document highlighting accepted uses the next day.

A miner with almost 14 years experience was helping reattach expanded metal to guard framing at #2 Kiln Pier when a piece of sensory flat iron rotated into the work area and hit EE's forehead. The miner continued to work for 2 hours before requesting treatment, was then taken to the hospital, treated and brought home.

Two miners were ascending in the preheat tower personnel elevator when it stopped between the 5th and 6th floors. Miners were entrapped at 8:55am. Doors were checked and power cycled before a service call was placed with OTIS Elevator. The Service Tech arrived and the two miners were able to get out at 10:07am. A faulty Safety Overflow Switch was the issue.

On February 3, 2020 around 12:10p, our mine contacted MSHA regarding a single contractor, working for TIPS, stuck between the fifth and sixth floors of the Finish Mill building. Otis was called immediately and the contractor was freed from the elevator at 12:49pm.

Miner was descending in finish silo personal elevator when car stopped. Elevator service call was placed, technician reset governor overspeed switch and miner was out elevator at 1/9/20 3:00am

Miner was descending in preheater personnel elevator when it stopped in between floors 8 and 9. Miner was entrapped at 11:35, doors were checked and elevator power was cycled. Service call was placed with Otis Elevator, service tech arrived and miner was gotten out at 12:46. On inspection, a wire was found around sheave of positioning tape that caused tape to come off.

Miner was descending in preheater personnel elevator when it stopped in between floors 2 and 3. Miner was entrapped at 18:45, doors were checked and elevator power was cycled. Service call was placed with Otis Elevator, service tech arrived and miner was gotten out at 19:50. Tech adjusted excessive slack in positioning tape.

Wire from wire wheel poked right hand.

Finish mill silo elevator stopped on the way down. Otis came out on an emergency call and reset car controller and repaired safties

Employee was descending from top floor of preheat tower. A guide rope from swing scaffold for siding work got into lower cable sheave. Tension switch stopped elevator approx. 40' above ground. Used man lift to rescue employee.

Impaled right hand on guard hangar.

While unhooking a choker, the choker swung back and hit employee in the mouth. Causing a false tooth to come loose.

Employee went to the maintenance shop to pick up welding rods. When employee picked up the welding rod can (without gloves), a sharp piece of the can cut EE's right finger. The laceration required stitches.

Miner was walking up a stair way and tripped at the top falling forward onto the concrete catwalk. EE put arms down to catch self and cut left forearm just below the elbow resulting in 2 sutures to close the cut.

Miner was in the cement silo elevator when it unexpectedly stopped. EE was unable to get out of the elevator, waited for the service maintenance company to arrive. EE was in the elevator longer than 30 minutes prompting an immediate notification call to MSHA. The call was made in the allotted time and the miner was shortly after able to exit the elevator.

A personnel elevator in the preheat tower stopped unexpectedly and 3 miners in the elevator were unable to get out for 48 minutes. The supervisor on duty called the immediate reporting hotline within 13 minutes of notification. A report was sent to the district office in Lansing.

Miner was walking and tripped on a step falling onto grating. EE was carrying a sledge hammer and the hammer landed on EE's left hand resulting in a fracture to the left index finger.

Miner was cutting corrugated vacuum hose with a box knife and struck right hand with the knife blade cutting through the glove and causing a cut to the hand. The cut resulted in the miner receiving 7 stitches.

Cutting a 2x4 not clamped down. It slipped and employee grabbed for it, cutting left index and middle finger.

Miner stated that ee slipped on refractory on the deck. Employee was interviewed after the incident and reported no injury and completed a statement and returned to work without further incident.

Employee was riding the bus into the plant from contractor's parking lot when EE's left eye started watering, took EE to eye doctor, removed foreign body from eye.

Miner was greasing a finish mill bearing with a portable 3 wheeled greaser. The hose did not reach by 6 inches so the miner tried propping the greaser on a block of wood. While lifting the greaser the front caster wheel spun and pinched miner's finger causing a cut to left finger. The cut was serious enough it resulted in restricted job duties until healed, no stitches.

Employee got too close to a propane space heater and overalls caught fire on right leg, burning through and melting undergarment which was made of polyester. Fire caused 2nd degree burn on right leg.

Per MSHA instruction - walking to tool trailer after safety meeting. Slipped on ice and snow twisting ankle.

An ignition source within the kiln coal dust collector created a change of pressure inside the baghouse causing the compression doors to blow open and the fire suppression system to activate.

Employee stopped grinding and leaned forward. Dust/debris that had built up on hard hat or faceshield bracket, fell down EE's face and into EE's right eye. Eye was washed out & employee felt eye was okay. On 12/18/2017, employee reported that eye had gotten worse; red from irritation, causing pain and blurred vision. Employee was taken to Quick Care for foreign body removal.

Employee was walking backwards, tripped and fell over own feet. Employee used right hand/wrist to attempt to break the fall. Right distal medial radial head fracture.

Crane Operator was climbing the ladder at the front of the crane and stepped on to the crane deck. After Operator placed first step Operator slipped and shin struck the second step on the deck. Operator sustained laceration on lower left leg requiring stitches.

Employee was using a sleever bar to fit stack piece. Pressure on bar caused bar to fly out and strike employee above left eye. Employee received stitches above left eye.

Miner was fueling a haul truck and was walking from fuel station around the truck, slipped on loose gravel causing miner to lose their balance and strain their back. Miner refused treatment until 7/24 when aching didn't recede, restrictions placed then.

Shredded plastic used for alternate process fuel that was stored in a covered containment caught fire. Unknown if the cover or plastic ignited first, cover was burnt off the structure and surface of plastic's was burnt. Source of ignition unknown. Fire was extinguished but had exceeded 30 minute time frame, call was made to MSHA hotline. Report to be filed with area MSHA office.

Shift started at 6pm prior evening. During the working shift the employee was clearing raw kiln feed from a transport drag by vacuum method. The hot feed was being cooled by water application to prevent vacuum hose from collapsing. The hot material sloughed into pooled water creating a steam rush that burnt the miners legs. Incident investigated by mine inspector.

Miner was experiencing tingling and numbness in left hand and fingers. Upon evaluation it was determined that the miner had carpal tunnel syndrome. EMG scheduled to determine if surgery is needed.

Employee received injury to finger while using rigging equipment. Resulted in five sutures to right ring finger.

Employee was setting a spool piece into a coal transport line when the spool pinched the tip of ee finger (right 5th digit). The pinch resulted in a fracture of distal tuft.

Employee was engaged in steel erection activities and was installing tube braces with two other employees. They attempted to push the brace into location and it shifted back and pinched the employee's finger between the steel.

EE felt pain in right shoulder while traveling mortar product overhead in a newly constructed cyclone plenum. EE didn't seek medical treatment believing it was just a sore shoulder. EE returned to Wear Con facility on 5/8/17 where EE sought medical attention and was diagnosed with a right shoulder muscle and tendon strain of the rotator cuff and place on restrictive duty.

Miner was clearing hard buildup from an air slide flow gate when the gate closed while hand was under it. The gate pinched fingers causing a cut to right 4th digit that resulted in stitches.

While turning over a wall panel for the coal mill bag house with a forklift, the wall panel slid off the fork and pinned employee against a tool box. The panel hit around the lower side of employee's left leg.

Mobile mechanic was reaching in changing a hose on a 992 CAT loader. EE felt a pain in the side. EE reported the pain and refused treatment as to the assumption it was a strain. On 4/18 EE told doctor about the pain and an x-ray showed EE had fractured ribs. Note: employee is being evaluated for another condition possibly contributing to injury. No missed work.

Employee was hammering a pin out of a screw conveyor and missed the punch striking left thumb with the hammer.

Miner had a flare up of muscle tissue from a pre-existing condition resulting in current time away from work. The miner had surgery on knee in 2013 that was reported as the initial accident. submission # 1693071

Employee was leaving site for the day. Employee stepped out of the transport van, and as employee hit the ground felt right knee twist and pop.

The construction crew worker used a pry bar to remove the wooden form work from the concrete. As the worker applied pressure to the pry bar, it slipped from underneath the form at an angle creating a pinch point between the pry bar handle and a nearby piece of concrete, crushing the workers left ring finer causing a cut that required 4 stitches.

Miner was climbing down a ladder on a Cat 914 G end loader and lost balance falling backward striking head on the ground resulting in a contusion that required 2 staples to close.

Employee was setting horizontal rebar (size 11) and to help maneuver it, grabbed the end of the rebar with right hand. When sliding the bar, right index finger was caught between the horizontal bar being placed and a previously placed vertical bar, causing a small fracture to the distal phalanx and a laceration, which required sutures.

During a retest of the employees hearing it was determined that there was a shift of 35db at the 3k range from a 2009 baseline in the employees left ear. Note; the employee refused testing from 2009 to 2016.

The worker was part of a construction crew pouring at the mine as part of a new facility addition. The worker was walking across steel rebar matting that was part of the concrete form being poured. As the worker walked across the matting EE lost footing and fell forward. As EE fell, left forearm struck the edged of a section of rebar, caused cut to the L forearm, 10 stitches.

Occupational Illness; Bilateral Carpal tunnel

Dispatched tradesman from the Iron Workers Hall out of Detroit was working for Strand Constructors, Inc. at a field bench, grinding a steel pin when a piece of metal fell from overhead, striking the worker in the back , shoulder, neck region resulting in a stable fracture to the transverse process at C-4. Worker was evaluated and released from the hospital.

Employee was walking in an aisle way of the raw material storage building and slipped on ice causing a strain to left knee. The injury was not initially reported until 9 days later. After consultation it required surgery to repair a torn meniscus, performed on 6/15/16.

Miner noticed swelling of right knee from climbing in and out the of haul truck that is used for waste dust transfer.

Employee tripped at top step of stairs and fell forward onto a platform resulting in injury to ribs upon impact.

Employee was walking in an icy area, slipped and fell. EE landed on EE's side causing a shoulder injury.

Contract laborer was cleaning up a gypsum rock spill on grating when EE was struck by a piece of gypsum. A single gypsum rock fell from a beam just as the employee bent over to pick up a piece of material and fell between the adjustment strap and brim of EE's hard hat resulting in a small contusion that required a stitch to close. The employee was given full return to work status.

Worker was walking and stepped off curb, hyperextending his left knee. Felt fine the day after but knee pain came back in end of January. Was given rest restriction to heal.

Repairmen was cutting a piece of conveyor skirting to size with a box knife. The knife slipped and the blade came into contact with the employees left forearm causing a small 1/2" incision. Treatment resulted in 3 stitches.

Employee was assisting in adjusting cutting knives of plastics shredder. The drum head was being turned manually to check for clearance, the employee stumbled and reached to gain balance resulting in his finger entering the shredder screen through one of the approximately 2 inch holes of the screen and coming in contact with the turning drum and a chipping tooth.

Employee was lifting an electric operated cable hoist for installation and strained back resulting in muscle pain in lower lumbar region of back.

Employee was entering the above grate section of the Clinker Cooler through the south access door during a kiln outage for grate repair. The employee stepped through the access door and his right foot caught while his torso was turning and caused strain on his right knee.

Employee has carpal tunnel syndrome from years of working as a maintenance repairman.

Took plant down on Sunday March 1st for outage. On March 7th, 2015 saw glowing coal embers inside coal dust collector hopper/screw area. Called fire department and reported to MSHA.

Employee alleges hand swelling from jackhammering. Sent to doctor who diagnosed a hereditary condition. However, filing w/c due to swelling from jackhammering.

Employee was instructing forklift operator in rebar placement. He stepped back from load and instructed operator to shake out rebar bundle to ease handling. Operator shook bundle and 2 pieces of rebar fell off. As they struck ground, they bounces and struck bent over employee in shoulder.

Employee is a machinist. He was cleaning a bolt on the wire wheel machine. The bolt pulled into the machine and employee's finger was pulled in.

Employee was using a pipe wrench to loosen a fitting. As he pushed hard on the wrench he felt a pop in his chest.

Stepped off of step ladder onto a piece of wire. It rolled causing him to fall to the floor. Twisted knee when he fell.

Occupational illness. Employee scraped leg when rock came out of headbox. First aid treatment at home--no reporting. 5 days later, employee had infection in leg. Taken to hospital where he received IV therapy and hospitalization. First reported 10/24/2013. Still did not know if it was work related until 10/31/2013

Snow and ice everywhere. Employee pulled off the main road to a storage area right after the loader had gone through plowing snow but before any sand had been spread in this area. Loaded his truck with material, turned and his feet went out from under him--he fell.

Employee working under cooler with group of employees stringing the drag chain. As he was moving the chain across the sprocket he slipped and fell hard on his knee on a piece of castable.

Employee was standing by the door frame. He and his crew had been lifting and placing a piece of table liner through the door and into the mill. The liner became stuck in the door frame. The hoist operator backed it out. As it backed out it turned unexpectedly towards the employee. He backed away but it trapped his arm between the load and the door frame.

Employee had clocked out to go home. Leaving plant driving a scooter. Still on plant roadway--near parking lot. Lost control on a curve due to sand and dust on smooth road. It had been windy and material had accumulated. Employee tipped scooter over, fell and fractured thumb on left hand. This was originally assumed it was not work related. Comp found it to be work related.

Employee inside gyp silo 21 breaking down the job. Standing on scaffolding--was handed a ladder from below. Went to turn with ladder. As he did his foot caught between scaffold planks. They were tied down and did not move. He fell sideways wrenching his back on the lower left side.

Employee working with 55 gallon drum on handcart felt sharp pain in elbow.

Employee holding a 30# coupling to be removed. They were replacing a seal. As it came out it fell suddenly and unexpectedly. It hit his pinky finger.

Employee hardface welding and a piece of metal went into his boot. Burn of left ankle.

Employee was using a utility knife to cut a small piece of conveyor belt that was torn. His hand slipped and the knife struck his other hand causing a puncture injury. The wound required 3 stitches to close properly.

Employee walking down outside staircase and slipped on ice at the bottom of the staircase. He did not go all the way down--caught himself but his right knee twisted causing injury and pain.

Employee was shovelling. Hit a hard lump of material that jarred his wrist and elbow.

Full investigation was sent to field office. We had a fire in the kiln coal mill. The CO2 suppression system activated. All was well until 14:06 when CO rose quickly in the dust collector. The concern was fire. We manually activated the CO2 fire suppression system and the CO went to "0". No injury. No property damage.

Employee sent to work on airslide that had gotten water in it. They had to pull the airslide out and re-skin the air slide. To get to the airslide they had to climb over an airslide wall. As he climbed this 4 feet angled wall, slid and fell against wall. Injured shoulder tendon.

Employee changing screens in crusher, standing on platform, facing crusher box using an impact gun to tighten screws. He walked off the end of the platform and fell landing on a hopper box wall.

Employee walking down staircase. Foot caught on grating on bottom step. Knee was twisted.

Employee removing door from bucket elevator. Attached come-along to hold door but forgot to attach the hook to the door. Took bolts off of door. Pulled door off and it fell striking employees foot.

Employee lifted a spool of wire in the warehouse, turned, stepped on pallet and foot slipped off pallet. Big toe was injured

Employee working on mill liner job. Lifting a liner, foot stepped on a stray mill ball on the floor. Knee twisted. Strain and Sprain of right knee.

Employee inside ball mill, turned to leave, ball rolled under his foot and twisted right knee.

Employee alleges carpal tunnel syndrome has occurred due to his work with an electric grease gun--trigger on gun. Employee went out for surgery on wrists on 11/17/09.

Employee needs shoulder surgery from degenerative changes over time. He alleges that he was working on welding a bucket of quarry endloader when he took a large block of cribbing and moved it around to set under the bucket which caused pain. Surgery scheduled 4/2/2010

Working on maintenance crew installing a heavy object at the roller mill. Using come-along to lift object. Had to push object to straighten it to go through an opening. Felt pull in groin area. Not going away. Seeing a doctor. On restrictions.

Employee working on airslide. Instead of walking around to the other side--decided to step over it. As she did, she lost her footing and fell. As she fell her little finger on the right hand caught a bolt head and was cut. Stitches were required.

Employee has experienced carpal tunnel syndrome in both wrists aggravated over time by his work as a repairman at the plant. He left today to get the first wrist taken care of. In two weeks he will have surgery on the second wrist and then return to work.

Heavy lifting of elevator buckets and other material with twisting, turning and bending, caused a strain of the abdominal muscle and strained lumbar muscles.

Employee walking to the job through alleyway. The walkway was icy and sloped. Knee went out from under him and he fell. Loss of strength in a bent knee. Can't crouch to work. Left for knee surgery on 11/24/2009.

Employee stepped off small staircase to floor. There was a stray clinker ball on the floor. Employee stepped on it, twisted ankle and knee. Ankle started to swell. The next day employee went to the hospital where it was determined the ankle was fractured.

EE was walking across the plant to go to lunch. Stepped right foot into a small pothole in the yard area under the kiln. His ankle rolled and he tightened up to catch himself. Felt immediate pain in his lower back. Went to the doctor the next day in the a.m. Doctor put him on restricted duty for 3 days.

Employee entering ball mill. Climbed down ladder, as he left ladder and stepped onto balls, he slipped and fell and landed directly on his right knee.

Slipped and fell on wet CKD dust at the South Pug Elevator. Hand landed on sharp rock. Sharp deep pain in right palm.

Working on the #3 discharge elevator. Lifted a bucket that was stuck. Felt a pull in the back. Ignored it. Kept working. He is now feeling symptoms in the left leg and went to the dr. yesterday. He has been placed on a 10 lb lifting restriction and anti-inflammatory prescription. Seeing doctor next week.

Employee alleges that he was welding inside a chute and received a couple small welding burns on his legs. He did not report it. One week later he stood up from a desk and felt a sharp pain in his left knee. The next day he was admitted to the hospital with an infection in his knee. He alleges the infection came from the small welding burns.

EE was welding on the 807 elevator housing. two ee were vacuming with industrial vacum hose. The hose whipped suddenly and hit him knocking him down. He was unconscious for 30 seconds-2 minutes.

Employee was checking equipment outside. Snow was falling heavily. As he was walking down staircase he slipped on last step and fell heavily on shoulder. LTA 1/16/08.

Employee, shovelling spilled material off belt line, felt groin pull. No treatment. Wrote up groin pull again on 12/26/07. Still no treatment. On 1/10/08 employee decided to go to doctor. LDW was 1/11/08. LTA as of 1/14/08.

Employee walking down steps. Had oil on the bottom of his shoe - slipped on step - caught himself and felt pain in his lower back. Losing time from work as of 12/3/07.

Plant was starting up. There was trouble with the indirect firing system and had to go back down. The feeder would not turn. Management was informed at 7:30 am - checked. There was high heat in the vessel. Took all safety precautions - isolated, inerted with Co2 & nitrogen, used heat gun. Temperature went down. MSHA called at 7:45 am.

EE was welding a patch on a fan housing. He was hot, sweating. He put a new electrode into the stinger. He was positioning himself and the electrode touched the fan blade. He received a electric shock. The ground clamp was too far from the work.

Employee removing elevator door. A piece of channel iron fell striking his finger. Sustained a fracture to finger.

Cumulative trauma - carpal tunnel injury to both wrists after years of wrenching, etc.

Employee has diagnosed tennis elbow. After performing a lot of welding it flared up. He went to the doctor on 2/8/07. Dr. restricted his duty for 1 week.

Cutting up old conveyor belting-knife slipped and punctured employee in leg-stitches

Employee was using a torque gun to tighten/loosen bolts. He thought he switched to reverse when he was going forward, pinched his finger between coal mill and torque gun.

Cutting bolts on the liners in the finish mills. Replacing cutting tip on torch. Went to purge the line of oxygen. Heat and oxygen ignited glove. Glove had a hole in the palm.

Hammering with a sledge hammer inside finish mill. A piece of metal broke and punctured right calf.

Changing bags in dust collector. Heavy with dust which caused him to wrench harder. Felt pain in lower back.

Employee was jackhammering on discharge screen in finish mill. Piece of ball punctured left arm - 2 stitches.

EE cutting a bad piece of hose off the vac truck hose assembly. Cut his right thigh. Required 3 stitches to close.

EE was walking towards a metal shop carrying a piece of cut and rolled aluminum. On the way he tripped over a 2x4 piece of wood that was sticking out from under a stack of galvanized sheets. As EE was falling, he put out his R hand onto galvanized sheets to break his fall. One of the sheets cut through his glove and cut R thumb.

Lifting a cover & poking at tower felt pulling & pain hernia surgery required.

Employee is a lab tech - had to take a sample at the precipitator. Drove Kabota to building, parked, went up 4 or 5 steps to get sample at screw. Coming back down steps, feet went out from under him. Landed on concrete slab at base of steps, put his left arm out to catch himself. Sprained left wrist.

SHOVELING COAL AT M40 TRIPPER ON 12/18/2005. FELT PAIN IN LOWER BACK. CONTINUED WORKING. ON 12/20/2005 BENT TO TIE SHOES - COULDN'T STRAIGHTEN UP.

Employee operating a lathe had lifted guard to tighten chuck. Left key in chuck and turned on machine key caught ip of employee s finger and cut it off.

SLEDGEHAMMERING - FELT SHARP PAIN IN BACK.

Employee bent down and hard hat fell off. As he was standing up to keep hard hat back on he bumped into edge of vac truck sustaining puncture wound to head.

Repairing a conveyor belt at the tail pulley. Pulling hard on belt and felt soreness in left arm & shoulder. Surgery required in Feb. 2006. Using a comealong to help with pulling.

While lifting a heavy object & sliding it sideways employee felt a severe pain in his back. He was lifting with help but lifted & twisted.

Closing pocket knife, he cut his hand requiring 3 stitches.

QUARRY YARD WAS ICY WITH SNOW COVERING. EE GOT OUT OF LOADER AT FUELING STAION. AS HE WAS WALKING AROUND THE LOADER, HE SLIPPED ON HIDDEN ICE & FELL. HE SUSTAINED A FRACTURE TO HIS RIGHT TIBIA.

Coal was frozen in coal silo. Employees had to use pokebar to get material to flow. Felt sudden pain in right wrist.

EE in a manbasket working on blast pipe trolley system, using a hydraulic jack to hold shaft up. Partner went to adjust jack while ee was aliging shaft, bumped knob and jack released, employee's right thumb was smashed.

THIS IS AN ONGOING STRAIN PROBLEM RESULTING IN CARPAL TUNNEL SURGERY ON 11/12/04. EE WILL BE OFF WORK FOR 2 1/2 WEEKS.

EMPLOYEE OPENED HIS POCKET KNIFE AND TRIED TO OPEN A PACKAGE. KNIFE SLIPPED AND PUNCTURED HIS HAND. HE REQUIRED 2 STITCHES.

Employee was lifting a heavy lid/door on a dust collector. he was lifting with help. he felt sudden pain, dropped the door and doubled over.

LIFTING CEMENT PUMP BEARING ON #3 MILL (WEIGHT ABOUT 200-250 LB), HE FELT A PULL IN RIGHT BICEP. ON THE 29TH, WALKING UP STAIRS, HE REACHED OUT AND FELT A SHARP PAIN IN HIS RIGHT SHOULDER. RES TRICTED DUTY AND PHYSICAL THERAPY ON 2/29/04.

2 EE'S RIDING IN GATOR DROVE UP TO 3 EE PEDESTRIANS, THE PASSENGER IN THE GATOR WENT TO GET OUT AND SLIPPED. HIS FOOT HOT THE ACCELERATOR AND THE GATOR DROVE INTO THE PEDESTRIANS. ONE EE SUSTAINED A FRACTURE OF HIS LEFT ANKLE.

EMPLOYEE LIFTED AN AIRSLIDE (PARTIALLY EMPTY) WITH ANOTHER EMPLOYEE. HE FELT IMMEDIATE PAIN IN THE GROIN AREA. STRAIN OF GROIN - NO HERNIA - RESTRICTED DUTY.

EE TWISTED HIS LEFT KNEE WHEN HE STEPPED ON A ROCK.

HIGH WINDS, EMPLOYEE OPENED A DOOR, WIND CAUGHT IT AND SLAMMED IT SHUT CATCHING EMPLOYEES FINGER.

EE WAS POKING OUT A BUILDUP OF MATERIAL IN THE TOWER. A CHUNK OF BUILDUP HIT THE POKE BAR CAUSING IT TO JERK SIDEWAYS AND HIT EE IN THE LEFT CHEEK.

EMPLOYEE POKING A MATERIAL WITH BAR. MATERIAL FELL ONTO OPPOSITE END OF BAR CAUSING A SEESAW EFFECT. EMPLOYEES END CAME UP AND HIT HIM IN THE NOSE & FACE.

WHILE PULLING AN OIL PAN FROM UNDER THE KILN TRUNION, THE PAN STUCK. EE PULLED HARDER, THE PAN LET LOOSE & PINCHED EE'S FINGERS BETWEEN THE PAN & THE CONCRETE FLOOR.

WALKING DOWN ROCKY SLOPE TO A WATER VALVE. SLIPPED AND FELL INJURING LOWER BACK.

EMPLOYEE SLIPPED AND FELL ON OIL SPILL. INJURED RIGHT UPPER ARM AND SHOULDER.

SUPERVISOR ASSISTING WORKER REPLACING BOLTS IN THE DRIVE SHAFT OF A SCEW CONVEYER. WRENCH SLIPPED OFF THE NUT, HIT HOLD ON CONVEYER SCREW. FRACTURED RIGHT INDEX FINGER.

CONNECTING CHICAGO FITTINGS - IT WAS IN AN AWKWARD POSITION, FELT MUSCLE PULL ON RIGHT ARM. CONTINUED WORKING FOR 4 DAYS - FELT NEED TO SEEK TREATMENT. AIR WAS NOT RUNNING AT TIME OF INJURY.

EE TACK WELDING WEARING WELDERS JACKET BUT NOT GLOVES. SLAG FELL INTO SLEEVE. EE HAD SHIRT WITH POLYESTER LINING, CAUGHT ON FIRE. 1ST & 2ND DEG.BURNS TO FOREARM.

REMOVING STAND UNDER AIRSLIDE FROZEN TO THE GROUND. TRYING TO BEAT IT FREE - HIT 4TH FINGER ON LEFT HAND REQUIRED STITCHES.

KILN WENT DOWN AROUND 1:30 AM. OPERATOR DID NOT TAKE COAL MILL DOWN. IN MORNING INSTRUMENT MAN WENT TO CALIFBRATE BELT. HE RAN REMAINING COAL INTO MILL. MILL WAS RUNNING AND RAN IT INTO KILN. COAL HIT HOT CLINKER CAUSING EXPLOSION INSIDE KILN.

ELECTRICIAN WAS REMOVING TAPE FROM MOTOR LEADS WITH UTILITY KNIFE. UTILITY KNIFE SLIPPED SLICING ELECTRICIAN'S HAND. 4 STITCHES.

WALKING DOWNSTAIRS - CARRYING SMALL STEEL PLATE, MISSED LAST STEP. FELL ONTO GRATING. SPLIT LEFT KNEE OPEN REQUIRING 14 STITCHES.

TRUCKS WERE DUMPING ON LEVEL GROUND TO BE USED IN BUILDING A LANDFILL CELL. THE DRIVER BACKED UP. THE LEFT TIRE WENT ON THE PRECEDING PILE PARTWAY AND THE TRUCK WAS UNLEVEL WHEN HE STARTED TO DUMP. THE CAB TURNED OVER.

POUNDING ON COAL FEEDER WITH SLEDGE HAMMER. STRAINED LOWER BACK.

EMPLOYEE FELL THROUGH GRATING, THAT HAD BEEN REMOVED TO FACILITATE LIFTING OF BURNER PIPE. EMPLOYEE TWISTED KNEE AND CHIPPED KNUCKLE.

EMPLOYEE WAS ON LADDER WELDING. THE LEAD WAS STRAPPED OVER HIS SHOULDER. HE WAS SWEATING HEAVILY. THE CONNECTION HAD A 1/4" GAP. THE GROUND WAS NOT FIRMLY CONNECTED. WE DO NOT KNOW WHAT MADE C ONTACT BUT EMPLOYEE WAS ENERGIZED.

EE WAS PULLING A PIN OUT OF THE BLAST PIPE FLANGE AND TWISTED HIS BACK.
