Employee was climbing up the steps on the 980H loader. EE reached for the door handle, the door handle was slick from the rain causing hand to slip off the handle as the door opened. This caused ee to fall off the loader hurting back, shoulder, neck and arm.

Mostrando los 23Employee was positioning an A-Leg at the staging on the job site. After he staged the A-Leg he turned and tripped over dirt/rocks, hitting his left knee on a piece channel in the staging area.
Employee was putting bottom roller in on #1 belt when roller came out of hanger causing employee to fall backwards hitting mantrip. Employee felt pain in his lower back on the left side and down his left leg.
The EE had completed trimming excess skirt rubber from the #6 tailpiece located at survey station #10483. When the EE stepped down off the belt conveyor his right knee twisted and possibly came out of joint. The EE seen a phys. on 12-2-2011 and again on 12-05-2011. At this time the EE was taken off work for P.T.
EE was helping move continuous miner cable out of the roadway and stepped in a dip and sprained his left ankle. The ee continued to work, but his foot continued to hurt. On 11/7/11 additional x-rays of the ee foot were taken. At this time is was determined that the ee has a hairline fracture to bone in his left foot. The ee has not missed any work due to this injury.
The driver was attempting to straighten a bent hand rail to dismount truck, when the hand rail broke, causing him to fall head first to the pit floor. The driver sustained simple, compound, and hairline fractures, and cuts and abrasions.
Employee was removing drill bit when he lost his balance and his left cheek hit the sharp edge of the drill steel in use leaving a small cut on his left cheek. Required three (3) stitches.
Employee was exiting a 993K loader, when he reached the bottom step he slipped jumping to the ground, resulting in strain to the neck and lower back. (Employee continued to work until 1-14-2011).
Employee stepped down out of supply building and tripped, fell landing on portable mine phone causing cut on left hand requiring (5) stitches to palm of hand.
Employee Slipped on mine floor and tried to catch himself with right hand. Employee suffered simple fracture to wrist, as a result of the fall.

EE was loading supplies from scoop bucket when his feet slipped and he fell. He has experienced pain in his lower back.
EMPLOYEE WAS HEATING A BRACKET THAT HOLDS THE DUST BOX ON A 320 CAT EXCAVATOR WITH A DRILL ATTACHMENT. AFTER HEATING THE BRACKET, EE WAS ATTEMPTING TO STRAIGHTEN THE BRACKET USING A LONG PIECE OF PIPE TO APPLY PRESSURE TO THE BRACKET. EE WAS STANDING IN A MUDDY AREA THAT HAD LOOSE MATERIAL. THE PIPE SLIPPED FROM THE BRACKET & EE SLIPPED, FALLING ON HIS LT. LOWER LEG BREAKING IT.
EE STATED HE WAS WALKING IN NUMBER 5 ENTRY. WHILE GOING THROUGH A HANGING BLOCK CURTAIN, HIS RIGHT FOOT BECAME ENTANGLED IN THE CURTAIN. HE FELL, BREAKING HIS LEFT ANKLE.
EE WAS USING OXY/ACTY TORCH TO REMOVE BARS FROM GRIZZLY WHEN HE SLIPPED & TWISTED HIS BACK
EE STATED HE WAS LIFTING A COVER FROM ROOF BOLTER & SLIPPED RESULTING IN PAIN IN HIS RIGHT HIP & LEG. ER VISIT AND RETURNED TO WORK, EE WORKED UNTIL 6-4-2003 WHEN A MEDICAL DOCTOR REMOVED HIM FROM WORK.
