Employee was exiting a D11R. EE stated that EE's right foot was on the last step on the push arm as EE was lowering EE's left foot down to the ground. EE lost EE's balance on some frozen material and twisted EE's knee as EE tried to regain EE's balance.

The employee slipped and fell while walking across the parking lot at the end of shift. This resulted in an injury to right arm. On 03/17/2018 the employee went to the doctor and found that the employee in-fact fractured arm. Employee reported this to supervisor on 03/18/2018. The employee has not missed any time due to this accident as of this date.

Employee was walking back to the face area in the intake entry when they slipped on ice and fell. Diagnosed with fracture to the left fibula.

Employee was walking across a "bench" when he stepped on a dirt/mud clump causing him to slip and fall twisting his left ankle. it had been raining all day which also contributed to the fall. the ankle bone was found to be chipped through an x ray

Employee slipped on miner cable and sustained a strain to the right knee.

Exiting IT14 loader, when he stepped down onto ground with his left leg under him, his knee buckled causing him to fall to the ground. Employee continued to work in his normal occupation until 7/27/15 and then started missing time.

Employee was descending down the ladder when he missed the last step causing him to twist his left ankle. This accident resulted in a fracture.

Employee was getting out of coal truck to change a flat tire and as he was exiting the truck he slipped & fell on his left side catching himself with his left arm & dislocating his left elbow & fracturing his left wrist.

Management has been made aware through medical documentation that the employee is missing time due to the shoulder accident Jan 21, 2014. On that date the employee alleges that he slipped on ice, fell, and struck his left shoulder against the push arm of a dozer that was being repaired.

EE was carrying the top part of an overcast when he slipped on loose rock and fell causing a fracture to the left hand. He was placed on restricted duty as of 1/19/14 and has a follow-up with Ortho. 1/30/14. EE is employed by MC Ent. Brroks Run os providing this report in accordance to MSHA PPL P-11-V-05

EE was getting ready to weld a stop of for an overhead crane. EE fell 20 feet to the steel floor below landing on his left wrist and knee. EE was transported to CMAC General and was treated for a broken wrist and knee. EE was released on 12/19/13 and is now home.

Employee was pulling cable when he slipped in a mud hole and began to fall. He caught himself with his left hand and felt a burning sensation in his left shoulder. He has been placed on restricted duty as of 11/7/13 pending the results of an MRI.

EE was greasing head pins on 12-12 Joy miner when he dismounted he stepped on cable twisting ankle causing him to fall against rib striking his lower back.

Employee fell from a ladder while hanging ventilation curtain

EE evening shift EE reported to work at 15:35p.m. at the Gate 2 Guard shack. While attempting to enter guard shack a strong gust of wind blew the door into EE knocking him to the ground. EEG received a laceration to his left elbow requiring stitches.

Employee was operating a roofbolter and slipped on a lump of coal tired to catch himself grabbing the roof bolter tray causing him to strain his shoulder. Employee sought medical attention and was released with no days lost or restricted. On 11/27/12 employee sought additional medical treatment.

Employee's knee gave out on him, he slipped and fell 2 steps to a lower landing on the steps at the load out. No defects were noted on the steps and employee stated there was no defects to the step.

Employee stepped on a pallet covered in plastic in shop, employee slipped off plastic and his leg went under a drill carousel. The drill component then struck his right lower leg causing a laceration and minor fracture.

Employee was painting a centerline. As he backed up, he slipped on a rock and fell on his right hand.

EE was doing inventory when a truck pulled up and the EE turned around then twisting her left knee.

Employee has chronic pain in both knees and has arthritis is his knees. His condition is attributed to crawling in low coal and walking over uneven terrain underground. He recently stepped into a depression in the mine floor causing additional pain in his knee. He is currently assigned duties to limit crawling and walking over mine bottom pending further medical evaluation.

Employee slipped on ice and snow and twisted right knee.

Employee was walking down a snow/ice covered road and fell backwards onto the hard surface on his back. His back was injured. However, he finished his work/shift and did not realize he was injured until Saturday 01-09-2011.

Was lapping belt when EE stepped into a hole twisting his right knee.

Employee complained of pain in back when he fell from a ladder approximately 46" while performing maintenance on a rock truck. Employee has a history of back and knee damages from 37 years total mining experience.

Employee stepped into a culvert at the surface area of the mine and twisted his left knee. Employee suffered a torn ligament and started missing time from the accident on 05/29/07.

Employee was walking from power when he slipped on wet mine floor and twisted his right knee.

The employee was exiting the supply trailor. As she started down the steps she slipped and fell, striking her left shoulder.

While performing normal duties of operating a continuous miner the employee complained of pain in the back when he lost his footing.

Employee was unloading supplies from the mantrip and stumbled against side of mantrip. He has continued to work un-restricted. On April 5th management received a medical report from the doctor stating he has a non-displaced fracture of the 3rd finger on the left hand. This is now reportable.

Employee was making a belt splice. As he stepped backward without looking, he stumbled upon the 3" water line and twisted his left knee.

Employee was on a step ladder hanging a cable when the ladder became unstable and fell over. He fell on top of the ladder.

Employee was walking up a hill when he slipped and fell, spraining his left hand. Ground was snow covered and slick. Employee was attempting to light a pile of brush. He notified me 12-30-05.

Employee twisted left knee when he stepped from the boom platform to the main bolter platform on the walk thru bolter.

Employee sought medical treatment on 06/16/05 on the following: Employee was walking from the face to the mantrip in the TRACK ENTRY when he slipped on wet mine floor and twisted his left knee. Employee is currently working in a light duty capacity.

On March 3, 2006 employee started missing time from an accident occurring on 5/19/2005. On that day he slipped and fell against the tail of the continuous miner while working on the trailing cable. He underwent surgery on March 3rd to repair a damaged rotator cuff. Management was informed that ee was missing time on March 24, 2006.

STEPPED OFF OF A FLAT CAR, LEFT FOOT LANDED ON TRACK RAIL CAUSING HIM TO TWIST HIS LEFT ANKLE.

Employee was getting out of mantrip and stepped on slick bottom and sprained his knee. Employee has continued to work performing his normal duties. On October 03, 2005, the employee had exploratory surgery on his knee without any new event, and is now missing work.
