Two equipment operators were on the ground discussing work to be performed at the staker plant. A service man arrived to service and lube the plant. Friendly horseplay occurred as they grabbed each other this caused them to trip backward over a large clump of dirt. The service man landed on top of the equipment operator fracturing the left ring finger

Mostrando los 22Employee was walking across refuge truck parking lot. Ground was uneven, snow and ice covered. Employee slipped and fell. He landed on a shovel handle that he was carrying.
Employee was loading pizza pans on the roof bolter when he slipped and as he was falling, his hands slipped down the side of the pizza pan. The edges of the pizza pan were rolled over but still had sharp edges that cut his fingers.
EE was dismounting dozer after installing a CB when his foot slipped off push arm step causing him to fall and strike the side of his service truck parked next to the dozer causing his right shoulder to dislocate.

Contractor was attempting to clean windshield on his coal truck. Contractor had his left foot on drivers side tire and right foot on driver side steps. Contractor slipped and fell backward onto another truck parked beside him. Total fall was approximately 4 feet. Slip, fall, strain.
Employee was stepping off the back end of the U307 roof bolter on the 002 section to go assist other workers with building a stopping, when he slipped and fell off the roof bolter and his SCSR located on his belt struck the step on the back of the machine causing pain in his lower back. X-rays were negative at Montgomery General Hospital and his is on modified duty until 11-11-11.
Employee was climbing down ladder of end loader when his boot became caught in the last step causing him to twist his right knee. Employee first visited doctor on 10/15/11 with follow up appointment of later dates. Due to continued discomfort has been taken off work effective 12/12/11 until he receives an MRI on knee by doctor at hospital in Beckley.
EE was inside deck of shuttle car he had just moved to use scoop to clean feeder dump. When exiting shuttle car his right leg gave out causing him to fall and his right hip popped out of socket due to previous injuries from vehicle accident in which reconstructive surgery had been done.
Employee was walking up roadway when he slipped and fell striking his right elbow on mine floor. Employee was examined at hospital and referred to specialist.

Employee drove himself to Montgomery General Hospital where x-rays showed a fracture and he was referred to specialist. Employee missed no work, but was restricted duty.
Employee was roof bolting and had steel hung in hole and while pulling on steel to free it, he took a quick step back and hyper extended his left knee. Employee was examined & returned to specialist but could not be treated until after a heart cath had been performed due to other health issues and therefore missed days have begun to accrue on 9/26/11.
Employee was stepping from platform of roof bolting machine to the walkway area of machine when his foot slipped causing him to lose his balance and fall causing pain in his lower back. Transported to Montgomery General Hospital where x-rays were negative. MRI is scheduled.
Employee was walking in roadway at #9 break on the #1 bolt when he stepped into a rut in the mine floor with his right foot causing him to lose his balance and fall twisting his right ankle. Employee transported himself to hospital where x-rays revealed a lateral malleolar fracture to his right ankle. Employee is currently on light duty, no days lost.
Employee was firebossing when he slipped and fell on a water line
While adjusting the 1/1/2" to 0" belt, an employee walked up an inclined 3" water line "steel". The water line broke causing the employee to fall. The employee was taken for x-rays and observation. He was released to return to his next scheduled shift which is 1/31/2011.
EE was stepping along belt line and twisted his left ankle.
Employee was walking across the 001 section in the #5 entry when he stepped in a hole and hurt his right ankle. Employee works through Superior Elite Coal Services.
While welding on an airlock door the employee was loading materials into the scoop bucket and fell striking tailbone on the scoop bucket. The employee did not inform the safety department that he went to doctor and was removed from work or had restricted duty.
EE was walking, slipped and fell onto a water line.
While walking along the beltline. Entry Injury stepped between two rocks and stumbled, in doing so he fell against the rib and accidently pulled rock from the rib when fell and struck him on the left ankle. He didn't report accident until 5/13/09 to his forearm.
