Employee was assigned to close a chute flop door. As the employee was positioned on a ladder to begin ratcheting with the chain ratchet, he slipped off the ladder and fell to the ground. Approx 4 foot fall, but in the process of falling he struck his left knee on metal flooring. The impact caused a fracture to his tibula bone near the knee.

Mostrando los 12Employee was installing a curtain on an 8 foot ladder, when as he was dismounting the ladder, it twisted and employee rode the ladder to the ground. He tried to catch himself with his right hand and suffered an injury to his wrist.
Employee was cleaning 8 Mile No.4 belt, when he noticed a piece of rock in the off side of the belt structure. He alleged he used a nearby crossover to access the rock and removed it from the structure. As he was returning and dismounting the cross over his foot slipped on the step and he fell backwards and injured his lower back.
Employee was bolting and attempted to install a roof mat between the roof and a t-3 strap. He lost his balance and fell off of the man-up provided on the bolt machine He dislocated his shoulder and fractured his humorous (upper arm). His shoulder was placed back into socket and his arm was placed in a sling. He was placed on light duty and allowed to return to work the next day.
Miner was operating the bolt machine and picked up a bundle of plates to step onto the man-up. He nearly missed the step but caught himself to regain balance. He hyper-extended his wrist. Later in the day he went to the ER, x-ray was negative. He continued to have soreness and was referred to an orthopedic on 1/6/15 where an x-ray revealed a fracture. He was placed on light duty.
EE was shoveling belt and rockdusting the feeder cable on the ground was tripped over and the EE twisted his right ankle.
EE was hanging mining curtain stepped off ladder and twisted his ankle.
Walking to help pressure wash, and twisted right knee.
EE was climbing a ladder to get to the top of a overcast, when the ladder began to tip over. He jumped from the ladder landing on his left ankle. He went to the doctor and x-rays revealed no fractures. EE started missing work on September 2 when he was diagnosed with ligament damage.
EE was walking past the section loading point. Just as he walked past the tailpiece he slipped on uneven bottom twisting his left knee. He was diagnosed with a torn meniscus and started missing work on 8/30/2010.
Climbing up tarp rock, EE slipped and fell and stubbed his right hand.
Employee hurt knee about 3 weeks ago; continued working. He originally hurt knee by stepping on a water line & falling.
