Employee was walking on ground surface by conveyor when EE slipped and fell twisting left ankle.
Was operating Excavator. Got out and walked over to talk to dozer operator. As EE was walking over, EE stepped in a hole and dislocated their ankle.
EE rolled ankle when stepping off the last rung of an A frame ladder. After recommended first aid care was not progressing the EE was taken to our clinic and transferred to a specialist and was diagnosed with an unspecified fracture of right talus on 7/6/2023.
EE states walking down the stairs, slipped and fell, landed on right hand causing swelling to R shoulder and difficulty using arm. EE went to clinic the following day and was given self treatment instructions. On 4/24/23 EE went to the follow up clinic appt and was prescribed an MRI and physical therapy; diagnosed with right shoulder joint sprain.
Employee missed a step descending down stairs and reported pain in left knee. Employee was taken to our clinic and treated with first aid and prescribed an MRI. After MRI follow up was referred to an orthopedic DR. After initial visit to orthopedic DR employee was prescribed physical therapy and diagnosed with a sprain and tear of medial meniscus on 2/8/23.
Employee was working on line one coal mill drive when they stepped onto uneven ground and twisted their left ankle. The employee stepped through scaffolding in the area and placed their foot on an area where the floor is not even, causing the left ankle to twist. This resulted in a left ankle sprain and work restrictions.
EE was dismounting a loader. On last step to the ground, the EE lost balance and fell. EE attempted to catch themselves with left arm. X-Rays indicate that the left arm is broken.
Maintenance Mechanic was walking to the work station and rolled ankle resulting in a sprain.
On 9/24/21 employee fell while working on dredge and hurt elbow and shoulder. The employee did not ask to see doctor at that time. For 3 weeks EE said EE's injury was getting better. On 10/20/2021 EE was still feeling pain in R Shoulder and asked to see a doctor where EE was placed on lifting restrictions which turned this into a Reportable Incident.
Employee was walking in concrete sidewalk area and stepped into a small depression and hyperextended EE's knee. It was not noticed immediately, and the employee continued to work. Eventually, EE's knee began to hurt. After 3 months without medical treatment, a doctor recommended surgery to repair it. The Surgery occurred July 1, 2021, and this report filed immediately after.
The employee was attempting to throw a switch on the rail yard when EE lost EE's balance and sprain EE's left ankle.
Employee was ready to start using the forklift when they noticed the seat belt wedged under the seat of forklift. As they were dismounting, in a rainy morning and thinking about safety and supporting the start up of the plant, they used left foot and try to reach the floor without using the step and not using three points of contact.
An employee was returning from staking out the limits in area when they tripped and fell over a rock. Employee fell face forward and injured chin and lip requiring stitches.
Miner slipped while walking and hit ground 12/20/2019 bruising elbow hip and knee, but has NOT received any medical treatment until June 25, 2020. Miner has now had outpatient surgery to scrape scar tissue off elbow.

Employee was descending a fixed ladder in the Finish Mill and lost grip and fell to the decking below. Employee was taken to a doctor for evaluation where X-rays and MRI were performed. No fractures or significant injuries were sustained other than bruising. Employee was placed on light duty.
EE was dismounting the 4600 Drag Line. When EE stepped on the second step from the top EE's foot slipped off. This caused EE to loose EE's handhold and fall to the ground. This resulted in a fracture to the LEFT hip. EE fell approximately 4'.
Employee was coming from the cement silos heading to the car shaker walking across the railroad tracks when EE twisted the left knee. EE was taken to the company clinic where was seen by a doctor and diagnosed with Sprain of unspecified site of left knee and prescribed Physical therapy. Weather conditions and visibility were good at the time of the event.
The employee was walking to the working station and tripped and fell over a box, landing on left leg on the side of the knee.
Uneven Surface. Twisted Right Ankle.
The incident happened while trying to weld a flatbar, 6" by 36", to the inside of the hopper. While placing the flatbar, right before lowering his welding hood to start welding, some material gave away under his feet. He slipped backwards and struck himself with the flatbar right above his right eye causing a cut on his right eyebrow.
EE (Contractor: Mobile Contract Services LLC representing Navman Wireless) was climbing down from cab of a Cat Grader while inspecting for install of telematics GPS equipment, and on the ground, he stepped back and tripped on grader blade that was parked at an angle, falling backwards. While attempting to catch his fall, he broke his left wrist.
Employee was shoveling spillage of rock under the conveyor belt. As he was repositioning, his foot slipped on a rock and lost his balance. While trying to regain his balance, his knee was hyperextended while trying to prevent a fall to the ground. The injury was reported on 9/23/14 as a first aid injury, first medical treatment was 10/16/14, and restricted on 10/31/14.
Electrician was walking down the North 42 Inch conveyor catwalk, slipped and fell backwards. He landed on his left elbow and felt pain in his left shoulder. He was taken to the doctor the following day (11/08/2013) and placed on light duty (no overhead work) and given pain medication.
EE was exiting a fixed ladder which allowed access to a baghouse. As EE reached the last rung and stepped onto the grated flooring he turned to his left to exit the area and his left boot adhered to the grated flooring. He felt a strain in his left knee when he made the turn and took a step.

Employee was talking to another employee when other employee's cell phone rang. When employee stepped backwards to give other employee privacy, he tripped & fell over a landscape rock striking right shoulder on the ground. Employee was on a previously scheduled vacation from 12/11/10 to 1/9/11 and returned to work on restricted duty on 1/10/11.
Employee was walking down a concrete slope, he slipped and fell backwards hitting the back of his head. His head sustained a 1.5 inch laceration which required 6 staples.
EE was preparing floor grating for replacement by trimming the flat metal that was installed at the butt ends to aviod a trip hazard, when the piece he was standing on gave way and allowed him to fall.
Employee was climbing into the cab of a Catepillar Skid Steer Loader. The employee slipped and fell while he was attempting to access the cab of the machine. The fall resulted in a back and cervical strain. Employee was prescribed meds and placed on light duty
Employee was climbing down the ladder of a CAT 777D Haul unit and lost his footing. He fell backwards and hit his head on the ground resulting in a 1 inch laceration to the back of his head. The injury required 3 stitches.

EE sustained an ankle sprain unloading rail cars on 1/21/09. Over the past 6 months he experienced minor discomfort with the ankle. On 7/23/09 he was reviewed by a doctor and no abnormalities were found. He was seen by an orthopedic specialist on 8/7/09. On 8/10/09, The orthopedic doctor gave him lifting and walking restrictions and prescribed physical therapy.
