Brooksville South Cement Plant Metal/Non-Metal

Miner was working in Kiln 1 ballroom doing routine housekeeping, EE went to lift by self a 3x3 door (weight 50 pounds) used to return cleanup material to the silo. About an hour later EE felt a sharp pain in EE's groin and a dull pain in EE's lower abdomen. EE continued to work and finally reported the incident at 4:50pm

The miner was assigned the task of changing the drive pulley on Coal Mill Line 1 main drive ID fan when around 11:45am EE's left index finger was caught between the sheave and drive belt. Medical attention was provided. No fractures and 22 stitches were given to EE's finger. Miner returned back to work on same day with no restrictions.

EE was closing the doors on the Finish Mill 2. While using a hammer to help close the door EE accidently hit EE's right index finger. Medical attention was sought, and EE was diagnosed with a fracture and placed on restrictions of no use of the right hand.

EE was attempting to clean spilled material with a dingo skid steer with one hand when EE's left hand was cut by a metal plate used to hold a motion sensor. Medical attention was sought and the EE received stitches and placed on restricted duty of no use of the left hand.

EE was attempting to lift a safe rack when EE felt pain in EE's left shoulder resulting in restricted duty.

A contractor was entrapped on the Clinker Silo #2 Elevator for more than 30 minutes due to an electrical problem with the elevator. The contract employee was safe and was not in any danger. The elevator contractor was called to repair the elevator.

Employee was checking the gearbox to pinion alignment on a ball mill. EE was turning the coupling when the stored energy in the drive system caused the coupling to counter-rotate resulting in EE's hand slipping. The right pinky finger was pinched between the 2 coupling hubs causing a laceration and fractured and displaced bone.

During the EE's work shift, EE experienced what was reported to Contractor by the treating physician as a heart attack which resulted in EE's death. It is Contractor's understanding that the incident is classified as Degree 8 - Natural Causes.

Employee's finger got smashed while trying to lift a tire. EE stated it was heavy and EE dropped it on their finger.

Employee was working on line one coal mill drive when they stepped onto uneven ground and twisted their left ankle. The employee stepped through scaffolding in the area and placed their foot on an area where the floor is not even, causing the left ankle to twist. This resulted in a left ankle sprain and work restrictions.

EE was dismounting a loader. On last step to the ground, the EE lost balance and fell. EE attempted to catch themselves with left arm. X-Rays indicate that the left arm is broken.

The injury occurred when a large pin dropped from approximately 4 feet and landed on the employees left foot. The employee was in the process of completing the change out of rolls on the finish mill. This resulted in a contusion of the left foot, the employee was placed on restrictions.

The injured EE was assisting a Lube Tech to load a drum of used oil. After positioning the drum on a dolly, the drum slid off the forks of the dolly, as the drum was rotating the Lube Tech lost grip and the drum rotated towards the Injured employee. The force of the barrel drum pushed on the left wrist of the injured EE. Two hairline fractures in the wrist.

While changing the belts and motor on fan 611-FN007, employee made contact with the belt while it was in motion, causing it to run EE's hand through the fan sheave and received 8 stitches in EE's right index finger.

Employee was working at low Quality clinker silo when dust got in their eye.

employee got dust in eyes when adjusting EE's safety shield and glasses. Dust continued to bother EE even after flushing, overnight.

Employee touched hot metal door, burned palm of right hand (blister).

Employee closed an elevator door on own hand. This caused a closed fracture to the tip of right ring finger

Employee stepped over a hose on level ground, and felt ankle twist as they did. The employee continued working for an hour, and foot began to swell. It was discovered that they had a broken bone in their foot.

While operating a loader, the miner hit bumps in the road, potholes, etc. which jarred and injured EE's back.

Employee was removing a broken bag of cement from a conveyor when they felt a pull/strain in back. This only became reportable on Sept 15 when Physical Therapy was ordered.

Employee was walking in concrete sidewalk area and stepped into a small depression and hyperextended EE's knee. It was not noticed immediately, and the employee continued to work. Eventually, EE's knee began to hurt. After 3 months without medical treatment, a doctor recommended surgery to repair it. The Surgery occurred July 1, 2021, and this report filed immediately after.

Employee was changing a blower motor filter cover when EE twisted wrist and back. This was diagnosed as a simple sprain, and treated with first aid care for nearly 5 weeks (ice, ibuprofen). On February 24th, EE was given Physical and Occupational Therapy orders, making it a medical treatment case.

Employee was shoveling dirt when wind blew some dirt from shovel into air, getting it in eyes. Employee rinsed with saline, and after examination, was prescribed an eyedrop.

Employee got clinker dust in eye and eye became irritated overnight.

Injury occurred on January 16, however, first medical treatment for the injury was August 5th, 2020, making it reportable then. Employee was removing gas tank from pick up truck, and felt back pop. Employee has been exercising and strengthening since that date but pain is still present. Doctor recommended steroid injection to relieve pain.

Miner slipped while walking and hit ground 12/20/2019 bruising elbow hip and knee, but has NOT received any medical treatment until June 25, 2020. Miner has now had outpatient surgery to scrape scar tissue off elbow.

This report is submitted under contest as operator is investigating the reported injury. On October 13, 2019, a miner reported stepping on a 1/2" diameter water lance and feeling pain in EE's foot. Miner was wearing proper footwear and not performing any unsafe activities. X-ray showed a fracture to a metatarsal bone. Miner returned to work in a soft splint under restrictions.

Employee went to move a circular fan and the guard fell, allowing employee's thumb to contact the blade. Laceration occurred.

Employee got dust in eye causing irritation.

Employee was cleaning out gate on Silo 3 and got cement dust in EE's left eye. Taken to doctor on 3/15/2018 where eye was irrigated, cleaned and prescribed eye drops and eye patch.

Employee was working inside Raw Mill 1 and fell after material hit EE's right leg. EE fractured right ankle.

Employee was cleaning up some material in front of the Kiln 2 Kiln Feed Overflow Dog Box. EE slipped on a slick section of ground and twisted right knee.

Employee dropped a piece of square tubing on EE's right foot resulting in a fracture to the small toe.

EE was cleaning ash chute in Raw Mill 2 with an air lance working through and access door. Material came through door, knocking EE down against the handrail. EE sustained a dislocated right shoulder.

Maintenance mechanic was performing repairs to a pneumatic cylinder and pinched left ring finger.

Employee got some dust in left eye and eye was irritated. Took to Urgent care clinic and eye drops and tetanus shot was prescribed to prevent infect. No days off or restrictions given. Employee back to full duty that day

Employee sustained laceration to the left ring finger from contact with a sharp object. Laceration required 5 stitches.

Employee was holding a crimped hose in employee's hand and accidentally let go of the hose and the hose opened up and hit employee in the face causing a small laceration on the inside of employee's top lip requiring a few stitches.

Employee was descending a fixed ladder in the Finish Mill and lost grip and fell to the decking below. Employee was taken to a doctor for evaluation where X-rays and MRI were performed. No fractures or significant injuries were sustained other than bruising. Employee was placed on light duty.

Employee was working on changing a bearing in the plant. Their foot slipped and they rolled their ankle resulting in a sprain. Employee was placed on light duty.

Employee and co-worker had cut a piece of scroll plate out, and while laying it down, the plate caught the top of employee's glove and hit their hand and wrist as they laid it down, bruising their left hand/wrist area.

Employee caught right hand in cable sheave while hoisting a valve

EE was troubleshooting an FK Pump when the actuating counterweigh took an unexpected upward thrust and then downward catching EE right hand middle finger between the counterweight and the discharge pipe flange

EE was assisting a crew of maintenance mechanics replacing a link in an elevator chain using a pin driver to drive out a broken pin. While striking the pin driver a sliver of metal became airborne and struck EE on his right cheek causing a cut.

EE was in the process of attaching two 1" pieces on conduit together using a coupling. EE was holding a pipe wrench with EE's left hand holding the piece of pipe from rotating and treading the piece on the right using a pipe wrench also. While holding the pipe wrench with EE's left hand EE heard a pop from EE's left shoulder.

EE states he was lifting a 5 gal bucket of oil above his head and pulled left lower back.

While tightening bolts on a gearbox housing ee was positioning the hydraulic wrench on bolt 8 out of 16. While placing in position the hydraulic unit moved slightly catching ee left thumb between the hydraulic wrench and the gearbox housing causing a cut that required 4 stitches.

The task was re-positioning of a metal hopper preparing it for welding. During this transition the metal hopper tilted backward toward the forklift mast. The EE placed his hand between the metal hopper and the forklift mast to stabilize the metal hopper. Thus causing it to pinch the tip of his middle finger of his left hand.

Employee was inspecting a work area. During the inspection the employee squatted down next to the supervisor who was inspecting the position of a damper. When the employee attempted to stand upright they had immediate pain in the right thigh. The employee then sat down on the deck and informed the supervisor of the issue. The employee was then taken for medical attention.

EE was removing a packing gland with a com-a-long when the com-a-long hook slipped off its attachment point and re-coiled and struck the employee on the right hand finger causing a fracture.

EE was transporting equipment from one level to another up a set of stairs when he felt pain in his lower back.

EE was exiting a fixed ladder which allowed access to a baghouse. As EE reached the last rung and stepped onto the grated flooring he turned to his left to exit the area and his left boot adhered to the grated flooring. He felt a strain in his left knee when he made the turn and took a step.

The injured employee was cleaning around the raw material conveyor belt using a shovel when he extended to far and struck his left wrist against the conveyor frame causing a fracture.

Employee was removing a broken piece of a hammer handle from the hammer section using a bolt and another hammer. He struck the bolt twice with the hammer in order to remove the broken handle section when the third strike he hit his left thumb causing impact damage to the thumb.

EE was removing 2"x2"x1/4" angle iron from the storage rack to place on the cutting table when a sharp protruding sliver of the metal cut his right hand causing a cut that required 3 stitches to close the wound.

EE was working removing shell liners from raw mill (ball mill). The process required striking the liner with a hammer to loosed to remove. A piece of shrapnel struck EE in the right shoulder embedding under the skin.

In the process of pulling a tarpaulin over a roof vent. EE was pulling on a rope attached to an eyelet in a tarpaulin when the eyelet ripped in the tarpaulin and caused the EE to fall on same level impacting his elbow which impacted his left ribs.

EE was preparing floor grating for replacement by trimming the flat metal that was installed at the butt ends to aviod a trip hazard, when the piece he was standing on gave way and allowed him to fall.

In the process of pulling electrical wire in a cable tray, EE was feeding the wire in the cable tray when unexpectedly it moved and caught his little finger of his right hand between the wire and the metal tray, lacerating the finger.

EE was working in tandem with a co-worker removing a bearing race. EE was cutting off the bearing race while a co-worker was tapping on the back side of the bearing race to remove it off the shaft when a chip of metal struck the EE on the lower left side of his neck going under the skin.

Employee was arc gouging, cutting with an electrode, when a spark entered his ear canal causing burning.

Removing Bag-house inspection door with chain-fall, handle pulled out of its anchor and flew into scaffold crossmenber then into worker's uppper lip. Created a laceration from base of nose to edge of lip and required 5 stitched to close the laceration.

Employee was walking through the cooler area - he bent down to walk under low connector plate. He came up too soon and his helmet struck the bottom of the plate causing a sore neck.

Whilst inspecting the 331 area, the employee, EE twisted his ankle on the debris and uneven ground.

Employee was assisting co-workers with removal of track beams. When beams were removed off of the support beam, the support beam fell and struck her left foot, resulting in a fracture.

While flagging equipment transporter (vehicle ID#8024 Ottawa), a loaded flat bed with hot gas return duct, employee's right ring finger was caught between scaffold pipe and duct brace, breaking and lacerating ring finger.

Employee was standing on top of an air slide, the air slide is 6? 6? high, the employee slipped and fell to the floor. Employee had a broken arm from falling.

Employee was using a spud wrench to do an alignment. The wrench slipped and came into contact with his hand between the thumb and index finger resulting in a laceration which required stitches to close.

The person involved says he was adjusting the water valve when the bull hose came off the gunning machine and caught him on the elbow. At the time, he only reported a cut to the forearm on Friday 9/28/07 but then was reported as a fractured arm on Monday 10/1/07.

Employee was wrecking forms. When he was taking a clip out with the hammer, the clip came flying out and hit him in the lip and tooth.

The employee decided to assist with a crane load which was beyond his normal scope of duties. He was walking backwards with a tagline when he stepped off of the structure contacting a lower level.

The grinder the employee was using kicked back and struck the employee in the face shield. The face shield cracked and the grinder came into contact with the employee's face.

Employee was handling pieces of rebar; another employee working above mishandled a piece of rebar which slipped causing a laceration on chest area.

A piece of rebar struck the employee (Reinforced Ironworker Helper) in the shin. The resulting abrasion became infected.

Employee was on a work platform beating tank pins down tight. After completing the task he pivoted and felt a pain in his right knee. When he stepped down to the ground, he felt his knee "pop".

Employee was holding a form in place when it slipped out of his grip and fell into injured's hand. Improper method for setting forms in place was employed.

EE was removing a wire choker from a section of equipment. The wire choker fell off the equipment and struck him on the head, resulting in two lacerations to the head. He was treated in the ER and Trauma Center for wound closure and observation.

While in the process of water blasting the kiln, the preheater feed chute material was being water blasted & expelled back toward the employee & entered the protective hood. He gasped & inhaled (hot)dust into his mouth causing mouth burns.

Employee was driving a T-post stake into the ground with a sledge hammer when he struck his right ring finger resulting in a laceration. He went to the ER after work requesting they remove his fingernail for comfort. He was prescribed pain medication for comfort and antibiotics for preventive reasons.

While inspecting a expansion joint on a kiln exhaust duct. Hot dust came through fabric and contacted right hand forearm causing 1st degree burns to right arm & top of right hand and neck.

While walking on a walkway beside a belt conveyor, he tripped on a piece of rope that is used to brack the conveyor. He never fell. The rope was removed. He broke a metal wire in his knee from an auto accident he was in back in 2001.

CONTACTED GUARDRAIL WITH UTILITY VEHICLE, SCRAPED DOWN THE LEFT SIDE OF VEHICLE STRIKING HIS HAND WHILE HOLDING ONTO SEAT BAR.

REPLACING LINERS IN MILL, A ROLLER SECTION IS USED TO TRANSFER THE LINERS FROM OUTSIDE TO INSIDE. A LINER ROLLED OFF THE END OF THE ROLLER SECTION STRIKING ANOTHER LINER. EE WAS BENDING DOWN TO PICK UP A BOLT, THE LINERS STRUCK SENDING A SLIVER OF METAL ABOVE HIS SAFETY GLASSES.

INSPECTING EQUIPMENT, STEPPED ON PIECE OF DEBRI TWISTING RIGHT ANKLE.

EMPLOYEE WAS WATERBLASTING ON THE FEED SHELF WHEN HOT MATERIAL GOT UNDER HIS GLOVE AND SLEEVE CAUSING 1ST AND 2ND DEGREE BURNS.

TWO MECHANICS WERE WORKING ON THE #2 AIR SLIDE. THEY HAD NOT DISENGAGED THE AIR VALVE OR SHUT THE MANUAL SLIDE GATE. WHEN THE LOADOUT OPERATOR RETURNED FROM A SAFETY CLASS & TOOK OVER LOADING TRUCKS, HE DID NOT KNOW THE MECHANICS WERE WORKING ON THE AIR SLIDE. HE SWITCHED THE KNOB TO LOAD OUT OF #2.

TWO MECHANICS WERE WORKING ON THE #2 AIR SLIDE. THEY HAD NOT DISENGAGED THE AIR VALVE OR SHUT THE MANUAL SLIDE GATE. WHEN THE LOADOUT OPERATOR RETURNED FROM A SAFETY CLASS & TOOK OVER LOADING TRUCKS, HE DID NOT KNOW THE MECHANICS WERE WORKING ONT HE AIR SLIDE. HE SWITCHED THE KNOB TO LOAD OUT OF #2.

EMPLOYEE WAS UNPLUGGING A CHUTE.

EE WAS GREASING BEARINGS. HE DID NOT CLOSE THE ACCESS HATCH WHEN HE GOT ON THE LANDING. HE STEPPED BACKWARD THROUGH THE HATCH AND FELL TO THE LANDING BELOW.

EE WAS REMOVING GRAPHITE BLOCKS FROM THE KILN. THE BLOCKS WEIGH LESS THAN 2 LBS.

CEMENT DUST GOT IN EYES. ANTIBIOTIC WAS PRESCRIBED.

CEMENT DUST IN EYE. CONDITIONS WERE WINDY AND DUST FLEW UP UNDER SAFETY GLASSES.

EE SLIPPED ON A FINE LAYER OF DUST AND LANDED ON HIS RIGHT KNEE.
