EE was working changing a line on compressor, standing on skid at compressor. EE re-positioned right foot and left foot slipped on the steel and fell backward striking head on the concrete resulting in laceration requiring 8 staples.
Employee was climbing down from 980 loader and reached the 2nd step when his foot slipped off of the step causing him to fall back onto the concrete slab. Employee was complaining of lower back pain.
THE EMPLOYEE, TRANSIT DISPATCHER FROM SCALE, WAS WALKING TO HER CAR AND TRIPPED ON A WATER HOSE, AND FELL TO GROUND. ACCIDENT RESULTED IN A FRACTURED RIGHT ELBOW AND LEFT KNEE CONTUSION. RETURNED TO WORK THE NEXT SHIFT.
