EE was adjusting the press when EE stepped back and slipped, falling to the ground. EE injured EE's ankle, knee and back and was put on light duty. Diagnosis pending.

Miner was walking through pit and tripped. Landed on knee. Went to doctor as precaution.
Freezing conditions over the weekend of December 17 & 18 created frozen and slippery ground. Shortly after the beginning of the shift, the miner was walking across the gravel mine, and slipped and fell on the frozen ground.
Miner was climbing into the cab of a front end loader and missed a step. EE stated this caused injury to the back. After the shift EE was seen by the Chiropractic Doctor, who excused EE from work, and released EE to full duty without limitations on 9/19/2016.
Miner had replaced a broken motor mount bolt on jaw motor. As he made his way from beneath the structure he fell against a steel beam. His left rib cage was sore so we had him looked and it was determined he had fractured a rib.
Miner was working from 10' ladder when ladder moved and miner fell.
Miner was walking on wet concrete surface when he slipped and landed flat on his back. Resulted in Buttock Contusion/ sprain. X-ray taken and miner was put on light duty until X-ray could be reviewed. X-ray was negative and worker was returned to job of injury.

Worker was using water hose to clean around hopper. Worker climbed up on side of hopper and subsequently fell backwards.
Miner was using a metal bar to dislodge a rock from under a hopper where it discharges onto a belt. The bar slipped and he fell backwards against the wall and fell to the ground. As he impacted the wall his hard hat was dislodged and caused a laceration to his right eyebrow. In trying to gain his balance he sprained both wrists and his left shoulder.

As he desended a slightly elevated walkway, his right leg slipped out from under him. He was unable to support himself with his left hand that was on the handrail. He fell down with his left leg twisted under him. The walkway and handrail were slippery. He was immediately taken to a medical clinic where they diagnosed his left ankle as broken.
While performing routine cleanup he noticed that the twin scrubber to the wash plant was plugged with mud. While attempting to climb up onto the edge of the scrubber his foot caught on a lip or bar and caused him to lose his balance falling into the scrubber. While trying to brace his fall with his left hand it struck the scrubber screw.
