Worker was taking turns with other workers swinging 20lb. sledge hammer to tighten nut on HP-4 crusher cone. Each worker was swinging about 10 times and switching off. This was EE's second time swinging and he made about 8 out of the 10 swings they were making. He stopped and said he felt his back strain. There were 5 miners taking turns.

Miner was unhooking lifting hook on boom truck from storage position. Miner placed right hand near shrive and when block move it caught right little finger between shrive and wire rope.

Worker was changing a tail pulley on a conveyor. He was using a 2-lb. hammer to align the equipment. The worker raised the hammer slightly and felt something in his lower back.

Miner was fabricating a support for a water tank overflow pipe. The miner removed his hard hat temporarily to put on his goggles and was shaking the goggles to remove rain water, when he accidentaly dislodged the support angle iron which struck him in the forehead.

Worker called in to say he would not be at work. Worker allowed his back was sore and he needed to see a doctor. Worker gave no information as to how his injury had occurred. It was not initially reported as workplace related. Worker has had a prior injury of the same general type. Worker was scheduled to see his doctor on 02/13/08.

Equipment operator drove loader over pot hole and jarred neck.

While lifting a steel grate to position it into its concrete framework the miner dropped the steel grate without first removing his fingers smashing the middle and ring finger of his right hand.

Miner failed to follow heat related illness policy and special instructions from superviser the same morning and allowed himself to become dehydrated resulting in heat exaustion.

Worker was picking up a 42" troughing roller when he felt a pain in his left middle back.

WHILE UNLOADING A CONVEYOR OFF A FLATBED, DURING LIFTING THE CONVEYOR, A 4 FOOT SECTION PIVOTED DOWN HITTING EMPLOYEE IN THE BACK.

SPRAINED LEFT KNEE WHILE HELPING ANOTHER EE POURA SLAB OF CONCRETE & WHILE LIFTING ONE END OF THE BOARD HIS KNEE POPPED & HE FELL DOWN

INJ EE & ANOTHER EE WERE TAKING OFF CUTTING EDGES FROM LOADER & WERE USING A 1"GUN TO TAKE THE PLOW BOLTS OFF. ONE OF THE PLOW BOLTS SPUN IN INJURED EE'S GLOVED HAND & CUT HIS HAND

EE CLIMBED UP SIDE OF CONVEYOR TO CLEAN OUT 2ND RECEIVING BOX ON TELESCOPING STACKER WHEN STACKER TELESCOPED IN AND CAUGHT RIGHT FOOT BETWEEN CONVEYOR FRAME. 5TH METATARSAL FRACTURE AND TOE LA CERATION TO RIGHT FOOT. TREATMENT WAS A SPLINT AND SUTURES.

EMPLOYEE WAS CLEANING OUT GRIZZLY, FINGER BECAME CAUGHT BETWEEN PRY BAR AND HAND RAILING.

EE WAS HOOKING A CHIAN UP TO A LIFTING EYE ON A CONCRETE BLOCK. THE CHAIN WAS HOOKED TO THE BUCKET OF 9 CAT 980 LOADER. THE LOADER OPERATOR PUT TENSION ON THE CHAIN BEFORE HE GOT HIS HANDS CLA EAR. IT PINCHED HIS FINGERS. EE AND OPERATOR LOWERED THE BUCKET, IT BROKE A FINGER AND TOOK SKIN OFF AND A NAIL.
