Miner was climbing on and off a crane during shift. He does not recall a specific injury event but experienced pain in his left knee at shift end. He was initially evaluated and diagnosed with a possible cartilage tear in the knee. He continued to work at normal duties until October 24, 2007 when he had arthroscopic surgery to repair the torn cartilage.

Mostrando los 12Miner was trimming a piece of rubber at a conveyor discharge using a box cutter knife when he lost his footing and slipped. When he lost his footing he sliced across his left hand with the knife and cut his hand. Miner was wearing leather work gloves but the knife cut through the glove.
Miner was positioning a tie-down chain on a floating conveyor. While using his foot to hold the chain in position on the deck as he wrapped the chain around a cleat, the chain slipped under his foot causing him to twist and strain his back.
Miner walking along beltline when foot got stuck in mud. While pulling foot loose he strained knee. Continued to work at full capacity until 9/18/06 when he was placed on restricted work activity by his doctor. Area had experienced several days of heavy rain prior to date of injury.
Miner was attempting to free a stuck conveyor using a 36 inch aluminum pipe wrench and a 5 foot cheater bar. While pulling down on the wrench/bar with his hands above his head, the wrench handle broke. Miner fell and landed on his tailbone. Wrench had previously been bent.
SLIPPED ON 2ND RUNG OF LADDER.
EE WAS OPENING A DOOR ON A GRAVEL CAR. WHEN THE DOOR OPENED, HE FELL AND STRAINED HIS BACK.
EE FELL ON A PALLET WITH HAND UNDER IT. HAND IS NOW BROKEN.
EE WAS WORKING ON OVERHEAD CONVEYOR ON A LADDER. HE HAD BEEN USING AN OXYGEN/ACETYLINE HOSE. FOOT GOT ENTANGLED IN HOSE WHEN CLIMBING DOWN FROM LADDER CAUSING HIM TO FALL BACKWARDS APPROXIMATE LY 5' INTO CORNER OF METAL FRAMEWORK.
EE WAS INSTRUCTING ANOTHER EE AT UPPER PIONEER CONVEYOR. THEY WERE CHANGING FROM SIDE TO SIDE BETWEEN HEAD ROOL OF KOHLBERG AND TAILROLL OF UPPER PIONEER. EE STEPPED ACROSS A 10 INCH TAILING P ILE CAUSING HIM TO LOOSE HIS BALANCE, ROLLING & TWISTING HIS LEFT KNEE.
EE WAS CLOSING TRAIN CARS. HE HAD A DOOR 3'4 SHUT WHEN SOME MATERIAL CAME DOWN ON DOOR PUSHING IT BACK OPEN KNOCKING EE DOWN HITTING DUMP BOARDS.
EMPLOYEE WAS STRINGING NEW CONVEYOR BELTING. AS HE WAS CLIMBING DOWN FROM KOLEBERG STACKER, HE STEPPED WRONG ON UNEVEN GROUND TWISTING HIS ANKLE AND KNEE OF HIS LEFT LEG.
