Miner opening rail car hopper doors when load in rail car became unbalanced causing car to suddenly lean, pinching miner between car and catwalk handrail. Rail car is a rotary dump car and material was not flowing out from one side of car when bottom dump hopper doors were opened.

Becon light on loader fell off mount, causing operator to stop suddenly while backing off crusher feed ramp, resulting in muscle strain to neck. Seat belt worn at time of accident.

Operator was loading trucks during shift and began to feel lighthead. He was taken off loader and treated at emergency room for dehydration. Worker drank liquids throughout shift however air conditioning in loader was not working. Temperatures were near high for the year at approximately 70 degrees F.

Worker opening and closing rail car hopper gates and felt pain in his back. Moved to a different job next day. Sought medical attention on 7/24/06 and placed on limited duty.
Operator and mechanic were lifting pieces of sheet pile using an excavator. Mechanic was attaching load to a large wire rope sling. Other end of sling fell off teeth of excavator striking him on the head. Mechanic was wearing a face shield but no hard hat and sling was attached to teeth of the excavator instead of attached by a hook or proper lifting connection.

EE and two co-workers were attempting to remove a large asphalt chunk off of a belt when the load shifted causing the asphalt chunk to come down and smash his right index finger. He sought medical attention and received 3 stitches.

EE WAS OPENING A DOOR ON A GRAVEL CAR. WHEN THE DOOR OPENED, HE FELL AND STRAINED HIS BACK.
Ee was making adjustments to the gear box. Finger was trapped between counterweights. Skin was ripped off index finger and nail fell off. Pain and throbbing occured.

TRIPPED AND FELL IN TUNNEL & GOT HAND STUCK IN TROPHING ROLL. EE PULLED EMERGENCY STOP CORD LOCATED ON BOTH SIDES OF BELT TO SHUT OFF CONVEYOR.
EE WAS JARRED IN THE SEAT OF THE LOADER HE WAS OPERATING. LOADER RAN OVER AN UNEVEN SURFACE CAUSING THE SEAT TO BOUNCE
EE WAS PICKING UP GARBAGE AROUND WORK BUTTON SHACK WHEN SHE WAS STUCK WITH A SHARP NEEDLE THAT HAD BEEN USED FOR CHECKING BLOOD FOR DIABETES
THE LABORER WAS SHOVELING A CONVEYOR THAT HAD NOT BEEN PROPERLY LOCKED OUT. THE BELT WAS ENERGISED AND HIS HAND WAS CAUGHT IN THE TAIL PULLEY.

EE WAS CLEARING 720 CHUTE OF RAP WHEN LARGE PIECE OF RAP DISLODGED AND LET GOT OF THE BAR HE WAS PRYING WITH AND HAND SLAMMED INTO UPPER I-BEAM CAUSING INJURY TO HIS LEFT INDEX FINGER KNUCKLE.
EE WAS CUTTING OUT A PIECE OF RUBBER ON CHUTE FROM 7 X 20 TO CONE. HE WAS KNEELING DOWN CUTTING THE RUBBER WHEN HE SLIPPED AND CUT HIS LEFT KNEE.
EE WAS INSTRUCTING ANOTHER EE AT UPPER PIONEER CONVEYOR. THEY WERE CHANGING FROM SIDE TO SIDE BETWEEN HEAD ROOL OF KOHLBERG AND TAILROLL OF UPPER PIONEER. EE STEPPED ACROSS A 10 INCH TAILING P ILE CAUSING HIM TO LOOSE HIS BALANCE, ROLLING & TWISTING HIS LEFT KNEE.
EE WAS CLIMBING THROUGH SCREEN DECK ON CRUSHER AND BUMPED HIS INSTEP (ARCH) OF RIGHT FOOT ON CLAMP RAIL WHEN STEPPING ON END OF SCREEN.

THE EMPLOYEE WAS CUTTING SCREEN TENSION BOLTS ON THE CRUSHER USING A TORCH AND A HAMMER. HE RAN HIS RIGHT HAND IN FRONT OF THE TORCH EXPOSING HIS WRIST TO THE FLAME.
EE WAS CLOSING TRAIN CARS. HE HAD A DOOR 3'4 SHUT WHEN SOME MATERIAL CAME DOWN ON DOOR PUSHING IT BACK OPEN KNOCKING EE DOWN HITTING DUMP BOARDS.
EMPLOYEE WAS CLOSING TRAIN DOOR USING A PRY BAR WHICH SLIPPED HITTING HIM ON THE CHIN CREATING A CUT.
EMPLOYEE WAS STRINGING NEW CONVEYOR BELTING. AS HE WAS CLIMBING DOWN FROM KOLEBERG STACKER, HE STEPPED WRONG ON UNEVEN GROUND TWISTING HIS ANKLE AND KNEE OF HIS LEFT LEG.
