Employee was doing maintenance on the crusher. EE was tasked to grind the left hand side of guard rail on the tall pully of conveyer belt. While getting down from the conveyer EE mis stepped and slipped, causing EE's right hand to come into contact with a piece of metal cutting EE's right hand

Employee was checking a battery to a welder while standing on the back step of a Mechanic Truck. When EE leaned the step moved because the pin was not in place to secure it causing EE to fall landing on right side. Seen in ER 4/23/24 no restrictions. Seen again 4/26/24 placed on restrictions.

Employee was attempting to operate a 980 loader. As employee was walking around the loader they tripped on a rock causing them to fall on left knee.

Employee was leaving work for the day and was looking up at the hill while walking to car. Employee tripped on a concrete barrier, falling to hands and knees.

Employee stepped off the bumper of pick up truck to unlevel ground. When EE's right foot came in contact with the ground, EE twisted the ankle and fell on knee.

Employee was going to go clean up around wash plant, when EE stepped in an area of mud and EE's foot sank about 6-8 inches in the mud, twisting the ankle.

Worker rolled left ankle when stepping down onto a grouser bar and sprained EE's left ankle.

This accident is still under investigation and my be revised at a later date. Employee had parked rock truck to help crusher crew. When he stepped on the second to last step he slipped and fell to the ground into his back. He did not wish for medical attention at that time. On 1/20/12 he decided to see a doctor.

This is an "Alleged Injury", this report may be amended apon further investigation. Employee was checking/cleaning spray heads when he slipped in some mud and fell onto right hip. Did not report this incident to management until 1/25/10.

While stepping off the skid loader stepped on bucket and slipped falling onto back

This is an "Alleged Injury", this report may be amended upon further investigation. Injured was stepping off of loader when foot got caught in last step, foot bent back.

EMPLOYEE WAS EXITING A CASE SKID LOADER AND STEPPED ON THE EDGE OF THE BUCKET, TWISTING HIS LEFT ANKLE.

EMPLOYEES WERE ATTEMPTING TO SWING THE LID CLOSED ON A ROCKEATER USING A ROCKBAR. THEY HAD BEEN INSTRUCTED TO CLOSE IT BY MECHANICAL MEANS ONLY. WHILE PUSHING ON THE BAR, IT BROKE AND BOTH EMP LOYEES FELL APPROXIMATELY 10' TO THE GROUND. ONE LANDED ON HIS FEET AND HAD NO COMPLAINTS OF ANY INJURY. THE OTHER SCRATCHED THE LEFT SIDE OF HIS FACE AND UPPER BACK STRAIN.

EE WAS STANDING ON A ROCK TRYING TO DISLODGE SOME ROCKS FROM THE CONE, THE ROCK HE WAS STANDING ON ROLLED CAUSING HIM TO FALL TO HIS RIGHT KNEE.

EE WAS CHANGING SCREENS ON TOP OF SCREEN DECK, CAUGHT GLOVE ON SCREEN WHICH PULLED HIM OFF THE TOP OF THE SCREEN DECK.

EE WAS CARRYING A SCREEN UP A LADDER, SLIPPED AND FELL AGAINST THE CORNER OF THE CAT WALK RAIL.

EMPLOYEE WAS CLIMBING DOWN FROM TEH EL JAY CRUSHER AND CRUSHER AND TWISTED HIS LEFT KNEE.

EMPLOYEE WAS CLIMBING UP STEPS TO A LOADER WHEN HIS FOOT SLIPPED ON STEP CAUSING HIM TO STRIKE HIS GROIN AREA.

SLIPPED ON STEOP WHILE GETTIDNG OUT OF WATER TRUCK.

WAS SETTING UP PUMP AT ACRUSHER SITE, TWISTED R KNEE WHILE GOING DOWN A HILL WHEN CARRYING THE PUMP.

SLIPPED ON CRUSHER SCREEN WITH LEFT FOOT, LEFT FOOT FELL THROUGH SCREEN AND EMPLOYEE FELL ON WRIST, BREAKING ARM.

EMPLOYEE WAS CHANGING AIR FILTERS IN 988 LOADER.HE MISSED THE THIRD STEP WHILE CLIMBING DOWN AND LANDED ON A ROCK TWISTING HIS ANKLE.

WHILE CHANGING SCREENS LIFTED SCREEN ONTO AC,CAUGHT FOOR ON CONVEYOR GUARD AND FELL,HEAD FIRST 5' TO GROUND.

WHILE CLIMBING OFF LOADER,STEP BROKE AND EMPL FELL TO GROUND 4'.
