Subject was exiting a work pick up truck when EE twisted EE's left knee. Subject continued to work and went to doctor on 7/14/2026 and was given clearance to work but with restrictions.
While putting a conveyor inspection plate back up, it slipped and dropped to the ground and swept the feet out from under a miner causing the miner to fall onto their shoulder. This occurred at ground level.
It was sleeting when subject was descending a metal grate staircase when they slipped. Subject grabbed the railing and heard a pop in right shoulder and felt pain. Subject also felt pain in right ankle. Subject has returned to work but is awaiting medical follow up. Subject is employed by Yaggy Trucking and not a miner. Yaggy trucking does not have a contractor ID number.
On 03/05/2026 Subject was walking when EE strained EE's calf. Subject first went to the doctor on 03/09/2026 and was put on light duty restrictions. The doctor has indicated that the subject is likely to be on light duty restrictions until May. No further medical information is available.
We were removing pipe from the clients well. Crew was using a dong tong (large pipe wrench) utilizing a sling on a ram to help break the coupling loose. The sling slipped off the wrench causing the wrench to move back toward employee. Employee twisted EE's knee when this happened. Employee has been seen and is currently on light duty working in our shop.
Miner went to close hatch on trailer bulk material tank at hatch closing station, stepped in the hole of the tank causing EE to fall. EE's injuries were a sprain of left shoulder, low back sprain and traumatic ecchymosis of right thigh.
Contractor climbed up ladder to aid in removing cover to head section. When transferring from ladder to conveyor to tie off, took a wrong step and fell roughly 12 feet backwards landing on the chute and then to the concrete on their back. Contractor broke their tailbone.
Associate was walking towards the stacking conveyor kind of fast next to the clarifier when EE slipped and fell down head first and hit right side of head on the concrete. The concrete was wet and may also have had some algae and floc on it. Possible concussion without loss of consciousness, and sprain of right side ligaments of cervical spine.
Customer Truck Driver was closing the trailer lids at the safety rack when EE lost EE's footing falling off the trailer to the ground below. The employee was transferring from the safety rack platform to the top of the trailer before the incident occurred.
Miner was walking in the wash and tripped on a pump cover and fell to the ground. Reported the accident on 02/11/2019. Miner had surgery on 03/18/2019 which resulted in being off work.
While the miner was stepping off the dredge onto the gang plank, the plank shifted causing the miner to fall into the water. Associate sought medical attention on 2/11/2019 and was placed on restrictions.
Miner was walking along the railroad track when EE stepped into a depression. Stated EE felt intense pain in the left knee and heard a loud pop. Sought medical treatment on 08/16/2018 and was placed on restrictions.
While descending the 972K loader, the miner missed the last step. EE's hand slipped off rail causing a fall on left shoulder. Refused to be taken to hospital and went home. EE said would see the doctor from home. On April 13, 2018 miner had rotator cuff surgery.
The miner was fueling the haul truck from the fuel tank. The miner turned around to talk with another associate and tripped over the large orange wheel chock falling on left knee cap. The miner had surgery on December 7, 2017 and is expected to be off work two to four weeks.
When stepping down from a conveyor, the miner miss stepped and fell striking against another conveyor.
Miner was standing on a ladder cleaning the grizzly in dry 2. The ladder twisted causing miner to loose balance and fall approximately 5 feet to the ground.
Miner was checking the proper sequence of rail cars. The miner did not realize that ee had traveled past the end of fall protection lifeline. This stopped the miner and caused body to turn and fall onto the side of the rail car. Miner did not miss work but was sore so went to doctor on day off 2/15/17. X-rays showed cracked ribs. No other medical treatment was given.
The miner's foot slipped off or missed a rail tie causing the miner to fall into a rail car resulting in a cut forearm.
Employee slipped when climbing up on to a flatbed pick-up truck
Miner was getting out of a truck when he fell hitting his back on the running board.
Miner was loading railcars and at the time of the incident crossing from one railcar to the next. The lanyard from the fall protection system is rolled out ahead of the miner. In this case the rollers attached to the lanyard which rolls on the overhead cable caught on a splice. When this happened the associate fell backward on the catwalk of the car behind.
Installing a fender on haul truck box assembly. Associate was standing on an elevated platform to position fender. Fender was swung into position prior to him mounting the platform stairway. As he was aligning bolt holes to fasten the fender it became wedged in the assembly. When fender unstuck it hit associate causing him to overcenter his weight and platform to tip over.
The 345D excavator was low on hydraulic oil and needed to be topped off. While attempting to remove the Hyd. oil cap (which was on very tight) with a ratchet wrench the ratchet wrench malunctioned. The momentum of the slipped wrench caused the associate to fall backwards to the ground on a rocky surface. This resulted in the asociate breaking the bone in his left arm
Working in a scissor lift, got off of it ot carry angle iron. As he was walking with the angle, his safety harness lanyard got caught on the heal of his boot, which caused him to fall foward over some pipe and land on the ground.
Associate was getting a production sample at the secondary wash and tripped on the grating and fell into the derrick and jammed his pinkie finger.
CUSTOMER DRIVER CAME IN THE SCALE OFFICE FOR HIS PAPERWORK. DRIVER TOLD SCALE OPERATOR HE FELL ON THE SCENE." DRIVER WAS INSTRUCTED TO COMPLETE AN ACCIDENT REPORT. DRIVER SAID HE SAID HE CA LLED HIS BOSS, AND "THEY WOULD TAKE CARE OF IT ON THEIR END." DRIVER RECEIVED PAPERWORK AND LEFT.
EE WAS EXPERIENCING BACK PAIN AND SOUGHT MEDICAL ATTENTION ON 1/25/99. AFTER SUBSEQUENT INVESTIGATION AND REFLECTION, EE & HIS DR ON 2/26/99 ATTRIBUTED THE PAIN TO AN INCIDENT INVOLVING A STRA IN AT WORK FROM SLIP ON ICE (NO FALL) ON 1/12/99. EE WAS CHIPPING ICE AND SLIPPED AND CONTINUED TO SHOVEL SNOW THE REMAINDER OF THE DAY.
EE WAS STEPPING FROM ONE RAIL CAR TO ANOTHER ON THE WET SAND LOADING TACKS. HIS FALL PROTECTION LANYARD CAME TO AJOINT IN THE OVERHEAD CABLE WHERE THE CABLE IS ATTACHED TO THE SUPPORT POLES. W HEN THE LANYARD STOPPED SLIDING ON THE CABLE, IT BECAME TIGHT AND PULLED HIM BACK ONTO THE RAILCAR HE HAD STEPPED OFF OF.
EE STEPPED OFF THE GRADER AND TWISTED HIS RIGHT KNEE.
THE ASSOCIATE DOES NOT KNOW HOW INJURY OCCURED. HE CLIMBED STAIRS AND FELT A PULL IN THE BACK AREA.
CATWALK NOT FASTENED TO STRUCTURE. AS ASSOCIATE STEPPED ON CATWALK IT SLID OUT FROM UNDERNEATH CAUSING HIM TO FALL APPROX 10 FT. AS HE FELL, TAPE MEASURE RAN THROUGH HIS HAND CUTTING HIS FINGE R. AREA IN UNDER NEW CONSTRUCTION.
EE WAS WALKING OVER TO CLOSE A VLAVE, WHEN HIS FOOT SLIPPED AND HE TWISTED HIS LEFT KNEE. HE HAS A SPRAINED KNEE AND IS AWAITING FOR MORE TEST S TO BE DONE.
WALKING DOWN AT THE OLD WASH TO OPEN SOME VALVESAND SLIPPED ON ICE AND TWISTED KNEE.
EE WAS WALKING FROM PARKING AREA TOWARD CRANE AND SCREENING CONVEYOR. HE SLIPPED ON THE ICE BETWEEN THE CRANE AND CONVEYOR AND FELL.
EE GOT OF HIS TRUCK IN THE PARKING LOT, IMMEDIATELY FELL ON THE ICE. DOESN'T KNOW IF HE HIT HIS SHOULDER ON THE ICE OR THE VEHICLE PARKED NEXT TO HIM. HIS TRUCK DOOR WAS NOT CLOSED YET.
EMPLOYEE SLIPPED ON A 2' ICE PATCH WHICH WAS FORMED FROM THE RAIN ON 1-18-96. EMPLOYEE CLIMBED DOWN A 10' STATIONARY METAL LADDER, TURNED AROUND AND SLIPPED ON THE ICE WHICH WAS APPROX. 2' FRO M THE BOTTOM OF THE LADDER INSIDE OUR RESCREEN BUILDING.
THE OPERATOR WAS CLOSING THE COVERS OF A PNEUMATIC TANK TRUCK A FALL PROTECTION DEVICE WAS BEING USED THE DEVICE WHICH SLIDES ON A CABLE WAS NOT POSITIONED OVER THE DRIVER BECAUSE OF THIS BIND ING OCCURED THE MECHANISM WHICH WAS MOMENTARILY RESTRICTED GAVE WAY AS THE OPERATOR BENT FORWARD THE OPERATOR THEN SLIPPED & SLAMMED THE HATCH COVER ON HIS RIGHT HAND
EE WAS TYING 3,000# TOTE SACKS ON PALLETS. HE WALKED BETWEEN TWO PALLETS AND TRIPPED ON ONE, FELL FORWRAD, AND TRIED TO CATCH HIMSELF. UPON CATCHING HIMSLEF, BENT BACK LEFT WRIST.
WHILE EMPLOYEE WAS WASHING THE WINDSHIELD OF A 966F CAT FRONT-END LOADER, HE CROSSED OVER THE TOP OF THE CENTER LOADER BUCKET RAM TO GET FROM ONE SIDE OF THE LOADER TO THE OTHER. HE SLIPPED & FELL APPROX 2' ONTO HIS SIDE & BACK. HE FELL ON RT LOADER BUCKET ARM & FENDER.
WHILE EE WAS STEPPING DOWN APPROX 28" FROM CATWALK OF LOADOUT TO TOP OF A GBW 6000 RAILCAR, HE LANDED WRONG ON HIS FOOT, TURNING HIS ANKLE.
INJURED WAS BRUSHING THE DERRICK SCREENS. AS HE FINISHED THE TASK, HE STARTED DOWN THE LADDER. THE LADDER TIPPED. HE REACHED FOR A PIPE TO HANG ONTO. HE MISSED THE PIPE AND LANDED ONT HE D ERRICK FRAME, BRUISING HIS LEFT SIDE RIB CAGE.
HE WENT INTO A STORAGE TRIALER VAN TO CHECK WHAT BAGS WERE ON HAND. TWO STYLES OF BAGS IN STORAGE. HE WAS CLIMBING OVER STACKS OF BAGS AND SLIPPED ON THE POLY SHRINK-WRAP COVERING. HE ATTEM PTED TO CATCH HIMSLEF ON THE WALL WITH HIS LEFT ARM. THE IMPACT WITH THE WALL CAUSED THE INJURY TO HIS LEFT ARM.
HE WAS SITTING ON A BEAM TO BE REMOVED WHILE ATTEMPTING TO RIG THE BERM IT ROTATED & THREW HIM OFF WHILE FALLING HE CONTACTED A STEEEL TANK & A STEEL BEAM ON THE GROUND HE RECEIVED A BRUISE TO THE HEAD WHICH RESULTED IN A SKULL FRACTURE CONCUSSION & CUT FOREHEAD
HE WAS GOING DOWN THE BASEMENT STAIRS AND TURNED DOWN AT THE BOTTOM OF STAIRS WHEN THE BACK OF HIS RIGHT KNEE WENT OUT.
EE PARKED THE DUMP TRUCK AND THAN JUMPED OUT HE INJURED HIS BACK IN DOING SO.
EE WAS CLIMBING DOWN THE LADDER ON A BN RAIL CAR AND WHEN TRANSFERRING TO THE OUTSIDE LADDER ON THE CAR, HE SLIPPED AND FELL TO THE GROUND. HE FELL ABOUT TWO STEP LENGTHS ON HIS BUTT. AT THE T IME OF THIS REPORT, HIS LOWER BACK AND KIDNEY AREA HURTS.
SLIPPED ON WET SLIPPERY SURFACE OF THE TRACK AND LIGAMENT TEAR TO R KNEE
INJURED WAS WALKING ACROSS THE EMPLOYEE PARKING AREA TO START A NEW DAY. HE SLIPPED ON THE ICY AREA AND STRAINED HIS BACK.
HE AND CO-WORKER WERE STANDING NEAR A BEAMED 8' LEDGE DISCUSSING WORK PROCEDURES FOR THEIR SHIFT FOR APPROX. 5 MINS. AS THEY TURNED AWAY FROM THE LEDGE, THE BANK GAVE WAY DROPPING HIM 8 FEET, STRAINING AND SPRAINING HIS RIGHT FOOT.
AS EMPL WAS LOADING BULK SEMITRAILER WITH SAND AT OUR FAIRWATER PLANT HE WAS GOING TO REMOVE ICE FROM TOP OF TRAILER HE LOST FOOTING & FELL APPROX 12' TO GROUND. ON WAY DOWN HE HEAD & LEFT KNE E ON CONCRETE WALL CAUSING LACERATION TO FOREHEAD & BRUISE TO LEFT LEG.
