Faulty regulator on vaporizer caused gas to leak into the vaporizer unit. When the vaporizer ignited, it caused the gas to ignite and blow the cover off the top of the unit.

Associate was cutting a rubber hose and knife slipped and cut palm on left hand. 1 cm laceration to left palm. Received stitches and tetanus shot.

Unlatching door on D6 Dozer to get side compartment off; Needed to refill hydraulics after a service; machine was on 12" blocks, not ground level - Reaching pulled neck muscles.

Associate was diagnosed with Silicosis. The ILO Rating the diagnosis was based on is 1/1. The company is waiting for the official diagnosis paperwork from the doctor.

Miner was walking along the railroad track when EE stepped into a depression. Stated EE felt intense pain in the left knee and heard a loud pop. Sought medical treatment on 08/16/2018 and was placed on restrictions.

Miner slammed the right index finger in a truck door resulting in a swollen finger. Miner sought medical treatment on 7/31/2018 for traumatic hematoma. Miner had a procedure done on 8/3/2018 for a cyst removal.

While descending the 972K loader, the miner missed the last step. EE's hand slipped off rail causing a fall on left shoulder. Refused to be taken to hospital and went home. EE said would see the doctor from home. On April 13, 2018 miner had rotator cuff surgery.

On 12/1/2017, miner told coach that EE went to the doctor for tennis elbow. Miner commented it was from splitting sand in the dry lab. Miner was given options to do therapy or go on Worker's Compensation and EE elected therapy and kept working. On 1/15/2018, miner was given a hand brace with restrictions.

Associate was washing his dump truck with a water hose and brush. Bent or twisted wrong and felt a pain in his lower back.

Associate was bolting two 8" plastic pipes together and they were pinched together but slid apart and struck the associate in the lower leg knee area.

Associate was positioning hoses on top of a railcar to load the car with sand. After the associate got the hoses into the railcar he stood up and noticed a burning and pulling feeling in his groin area.

Associate was in the main office break room putting his lunch bucket in the refrigerator. As he bent over, he felt a pain in his lower back.

Employee slipped when climbing up on to a flatbed pick-up truck

Associate was lifting a barrel of grease from company service vehicle when he felt pain in his upper back.

Associate was sitting in a chair in the jet shack. When beginning to stand up, the associate indicated he was unable to stand up as his lower back was hurting. Associate did not hear or feel any "pop" in his back, but felt pain. Associate was sitting for a couple hours prior to standing up or getting out of the chair.

An associate was working at the Mobile Equipment Shop, changing out the waste oil tote (platic tote in a cage) when he cut his left hand resulting in medical attention (6 sutures). The associate used his left fist to pound an aluminum tag from the tote cage and was not wearing gloves.

EE was cleaning the north cooler with a ompressed air tool. EE inserted the tool into a cooler tube and when the tube reached the bottom of insertion, the EE's finger got pinched between a 90 degree union and the seal block. The EE was not wearing gloves at the time of the incident. The injury to his right hand pointer finger required four stitches.

The 345D excavator was low on hydraulic oil and needed to be topped off. While attempting to remove the Hyd. oil cap (which was on very tight) with a ratchet wrench the ratchet wrench malunctioned. The momentum of the slipped wrench caused the associate to fall backwards to the ground on a rocky surface. This resulted in the asociate breaking the bone in his left arm

WHILE TURNING THE HANDLE TO CRANK UP HIS ROLL TARP THE HANDLE CAME OFF UNEXPECTEDLY AND EE LOST HIS FOOTING AND FELL TO THE GROUND FROM THE DRIVERS SIDE OF HIS DUMP TRUCK.

ASSOCIATES WERE PRYING ON A BEARING IN AN ATTEMPT TO MOVE IT FAR ENOUGH ALONG SHAFT GOT GET A PULLER ON IT. WHILE PRYING THE BEARING MOVED FUTHER THAN EXPECTED & THE ASSOCIATES RIGHT LITTLE FI NGER WAS PINCHED BETWEEN THE BAR (LEVER) & ANOTHER PIECE OF STEEL

CUSTOMER DRIVER CAME IN THE SCALE OFFICE FOR HIS PAPERWORK. DRIVER TOLD SCALE OPERATOR HE FELL ON THE SCENE." DRIVER WAS INSTRUCTED TO COMPLETE AN ACCIDENT REPORT. DRIVER SAID HE SAID HE CA LLED HIS BOSS, AND "THEY WOULD TAKE CARE OF IT ON THEIR END." DRIVER RECEIVED PAPERWORK AND LEFT.
