Mostrando los 46Employee parked a skid steer loader, when employee was getting out of skid steer and stepping down, employee hung left foot in between the bucket frame and step for dismounting skid steer.
The employee stepped off of a ladder onto an uneven section of ground, causing EE to fall and land on elbow. EE suffered a hairline crack in the ulna, which required a brace to be worn. There were no restrictions as it did not affect EE's regular job.
While welding inside the Stage 5 Cyclone, the employee turned and fell off of the work deck to the one 4 1/2 feet below. The employee suffered a 2.3mm tear in the left shoulder ligament.
EE stepped on cross beam to gain advantage to see over conveyor belt; foot slipped and he jerked his arm exasperating a preexisting non-work related shoulder injury he was being seen for.
While EE was position the barge, he slipped and struck his wrist against the decking causing a suspected scaphoid fracture of his right wrist; soft cast and restricted duty prescribed.

Employee stated when he was dismountlng the front end loader, his hand slid on the hand rail and he struck his elbow on the platform. (He reported the injury to the safety manager on 10/21/2009.)
Employee was walking up stairs carrying a ladder. He tripped and braced his fall with his right hand. Caused pain in his right shoulder - possible muscle tear.
employee was stepping down from a palletizer and tripped on an emergency stop cord causing him to fall against a handrail. He fractured 4 ribs.
Employee was attempting to move a piece of plate steel that was setting on steel grating. He was using a pry bar to wedge in grating to move the plate. Instead of the plate moving, the grating shifted off the support beam, allowing the grating and employee to fall to the ground below. Sprain wrist and contusion.
Employee had been loading material in a hopper with a front end loader. He walked up the ramp to check the level of material in the hopper. As he was walking down the loading ramp, he slipp ed on loose material. He braced himself with his right arm, causing discomfort in his right shoulder.
Employee tripped over a pressure washer hose and twisted his knee. He thought it was okay but later said it was not getting better. Saw doctor in January 2008, scheduled and had surgery in February 2008.
Employee was putting oil in the reducer motor at the reclaim belt. He fell from a make-shift work platform, fell approximately 2 feet, landed on his right wrist.

The employee was pushing an electrical motor off of an elevated platform. The employee was squatted down pushing the motor through an accessway in the handrails below a safety chain. It appears that when the motor rolled off it caught on the employee's gloves or himself and pulled him off the platform. He fell approximately 15 ft. to the ground.
The employee was walking through the mill helping other employees set up to clean up spilled material. As the material went down the hose it caused the hose to jerk and move. As the injured employee stepped over the hose, the hose jerked striking the employee on his left leg. This caused the employee to fall back on his right leg. This caused him to hyper-flex his right ankle.
The ee was walking from the finish mill to the control room. The ee was passing by the clinker silos which have an elevated pad around the silos. The surface is approx 3 inches higher than the surrounding ground. The ee began modified duty 3-11-04.
AN AIRSLIDE CONNECTED TO THE DISCHARGE OF A SILO BECAME PLUGGED WITH MATERIAL. EE OPENED AIRSLIDE & REMOVED CHUNKS OF MATERIAL & THREW THEM DOWN ONTO THE ADJACENT WALKWAY. AFTER CLOSING AIRSLIDE THE EE STEPPED DOWN ONTO THE ADJACENT WALKWAY & STEPPED ON ONE OF THE MATERIAL CHUNKS. THE CHUNK ROLLED & HIS ANKLE TURNED.
THE EE WAS WALKING THROUGH THE PLANT. THE EE WALKED ACROSS ICE AND SLIPPED. THE EE REACHED OUT FOR AN ADJACENT TRAILER TO STOP HIMSELF FROM FALLING. THE EE CAUGHT HIMSELF BUT IN DOING SO STRAI NED HIS BACK.
EE WAS OPERATING A DUMP TRUCK. THE EE GOT OUT OFTHE OPERATOR'S CAB TO INSPECT THE TRUCK BED. HE CLIMBED THE BED LADDER & LOOKED OVER INTO THE TRUCK BED.WHEN CLIMBING BACK DOWN HE MISSED THE BO TTOM RUN, SLIPPED & FELL LANDING ON HIS SIDE. HEWENT FOR MED TREATMENT BUT NONE WAS INDICATED. 0N 3/4/04 EE RET'D TO DR WITH ELBOW PAIN AND BEGAN MODIFIED DUTY ON 3/5/04
THE EE WAS CLEANING UP MATERIAL ON A WORK PLATFORM. AFTER FINISHING THE EE WAS RETURNING TO THE GROUND LEVEL VIA STAIRS. THE EE SLIPPED ON ONE OF THE STEPS WHERE OIL HAD BEEN SPILLED. THE EE CAUGHT HIMSELF USING THE HANDRAILS BEFORE LOSINGHIS BALANCE. WHEN HE CAUGHT HE TWISTED AND STRAINED HIS LOWER BACK.
THE EE WAS CLIMBING DOWN A VERTICAL LADDER AND FELT A PULL IN HIS CALF MUSCLE.
THE EE WAS PERFORMING MAINTENANCE WORK ON A WATER LINE. THE EE CROSSED OVER A CONVEYOR TO ACCESS THE WATER LINE. AFTER COMPLETING THE REPAIR THE EE CROSSED BACK OVER THE CONVEYOR AND AS HE STE PPED OFF THE CONVEYOR FRAME HIS FOOT SLIPPED AND IN DOING SO HE TURNED HIS LEFT KNEE.
THE EE SAT DOWN IN HIS SEAT AT HIS DESK. WHEN HE LEANED BACK IN THE SEAT THE BACK OF CHAIR GAVE WAY AND HE FELL BACK ONTO THE FLOOR. THE ACCIDENT WAS REPORTED ON 9-2-03. THE EE WAS SENT TO THE DR AND PLACED ON MODIFIED DUTY ON 9-2-03.
EE WAS ORIGINALLY INJURED ON 12/30/02 & REPORTED ON 1/14/03. ON 8/7/03 EE STATED THAT HIS NECK WAS BOTHERING HIM DUE TO THE EARLIER INJURY. DURING THE ORIGINAL ACCIDENT EE WAS COLLECTING SAMPL ES & TRIPPED ON DEBRIS. AFTER STATING HIS NECK INJURY WAS RELATED TO THE INJURY OF 12/30/02, HE WAS SENT TO THE DOCTOR ON 8/7/03 & PLACED ON MODIFIED DUTY.
EMPLOYEE WAS PULLING ON A TROLLEY CHAIN WHEN HE LOST HIS FOOTING AND SLID INTO A HAND RAIL. THE EMPLOYEE CONTACTED THE HANDRAIL IN HIS RIB AREA.
EMPLOYEE WAS CARRYING SAMPLES OF MATERIAL DOWN STAIRS FROM THE RAW MATERIAL BUILDING WHEN HE LOST HIS FOOTING ON TOP OF STAIRWELL DUE TO ROCKS BEING ON TOP OF STAIR LANDING. INJURY RIGHT FOOT, PAIN IN HIS NECK AND ARM.
EE WAS ON A LADDER WELDING ON THE RAW MILL REJECT VIBRATORY FEEDER WHEN HE SLIPPED OFF THE LADDER DUE TO MUD ON HIS FEET INJURING HIS RIGHT SHOULDER TRYING TO CATCH HIMSELF.
EMPLOYEE WAS TRAVERSING FROM ONE ROOM TO ANOTHER WHICH INVOLVED A STEP DOWN TO THE NEXT ROOM. WHEN THE EMPLOYEE ENTERED THE ROOM SHE SLIPPED AND FELL, STRIKING HER FOREHEAD ON A TABLE AND LEFT KNEE ON THE TILE FLOOR. SHE TORE THE TENDON IN HER RIGHT KNEE.
EE WAS DISCONNECTING A HOT WATER HEATER COUPLING WHEN HE SLIPPED AND HIT HIS HEAD ON A GAS PUMP IN THE FLOOR. HE RECEIVED A SMALL CUT ON HIS FOREHEAD THAT REQUIRED FOUR STITCHES.
EE WAS WORKING FROM AN ELEVATED PLATFORM APPROX.2 FEET ABOVE THE FLOOR. WHEN HE FINISHED WORKINGHE JUMPED DOWN TO THE FLOOR & BROKE A BONE IN HIS RIGHT FOOT. HE STARTED MISSING TIME ON 2/5/02.
EE WAS WALKING DOWN A FLIGHT OF STAIRS WHEN HE SLIPPED AND STRAINED HIS LOWER BACK. EE DID NOT START MISSING TIME UNTIL 1-8-02.
EMPLOYEE WAS HELPING A REPAIRMAN REPLACE A SINGLE LINE CABLE, ON AN OVERHEAD CRANE BUCKET WHEN HE SLIPPED AND FELL ON THE PLATFORM WHERE HE WAS WORKING.
EMPLOYEE WAS CUTTING ANGLE IRON, STEPPED BACK AND FELL OVER ANOTHER PIECE OF ANGLE IRON. RIGHT ANKLE POPPED.

THE EE HAD CLIMBED A LADDER TO ACCESS THE STRUCTURAL STEEL AS HE STEPPED HE MISSED A PIECE OF GRATING & STEPPED ON THE STEEL. HIS L ELBOW STRUCK THE STEEL & DISLOCATED HIS L SHOULDER
COMING DOWN STEPS FROM THE #7 SLURRY TANK, EMPLOYEE WAS IN A HURRY, STUMBLED AND FELL ON RIGHT SHOULDER.
EE WAS ATTEMPTING TO STEP ACROSS A TRENCH. HE WAS HOLDING ONTO A SECURED PIECE OF REBAR AS HE STEPPED OVER. HIS FOOT SLIPPED AND HE ATTEMPTED TO CATCH HIMSELF ON THE REBAR. FELT STRAIN AND POP IN SHOULDER.
EE WAS WALKING UP THE STAIRS AT PREHEATER TOWER. HE SLIPPED AND FELL DOWN STAIRS.
EE WAS WALKING ON STEEL BEAM IN THE STORAGE YARD. BEAM WAS WET CAUSING HIM TO SLIP LODGING HIS LEG BETWEEN BEAM AND A MOTOR. HE HAS A CONTUSION.ON THE RIGHT CALF MUSCLE.
OFFICE EE WAS WALKING BESIDE OFFICE BUILDING ALONG A SLOPED WALKWAY WHEN HER RIGHT FOOT SLIPPED CAUSING HER TO FALL. EE FELL IN AN AWKWARD POSITION WITH HER LEFT KNEE BENT UNDER HER BODY CAUSI NG INJURY TO HER LEFT KNEE AND LEFT FOOT.
