Employee was clearing a blockage from the suction hose on the vacuum truck, when their left arm had been drawn into the hose.

Maint Tech was climbing a step ladder, lost their balance and tried to catch their self. Employee's wrist contacted a piece of metal which caused a laceration.

Supervisor noticed smoke in the alternative fuel storage pile, instructed a loader operator to move some material to get to the smoking pile of material. The material was subjected to the atmospheric conditions, it was oxygenated, igniting the TDF. Local fire department was contacted and arrived on scene and extinguished the fire.

During the heavy rain down fall near the end of the shift on 2nd shift, a rock from the rock pile came sliding down the pile and struck the right-side window of loader, breaking out the side window and striking operator on left knee area causing a laceration to knee.

Employee parked a skid steer loader, when employee was getting out of skid steer and stepping down, employee hung left foot in between the bucket frame and step for dismounting skid steer.

An employee was WOMA Gunning the tower on the 2.1 level when a chunk of material fell on the wand and caused the employee's right arm to be jerked. Employee reported to supervisor, but did not want any medical treatment at the time of reporting the incident. On 3/13/2025, employee still felt soreness in their right shoulder and requested to be evaluated. Had surgery on 6/9/25

On a hot, humid day, an employee passed out due to heat exhaustion. Fell forward and cut a finger on the platform's deck requiring stitches.

While removing a drive sprocket with a porta-power and gear puller, a piece of the sprocket came free and struck the EE on the edge of EE's lower neck which caused a small laceration that required a stitch.

The employee stepped off of a ladder onto an uneven section of ground, causing EE to fall and land on elbow. EE suffered a hairline crack in the ulna, which required a brace to be worn. There were no restrictions as it did not affect EE's regular job.

In the process of unloading a portable building, a hinged wall fell and struck the employee's left thigh, causing a contusion. Initially seen by the local hospital and released with no restrictions. Due to weekend soreness, the employee's doctor placed them on one week of restricted duty.

EE was lancing a chute when a piece of material fell and struck the end of the lance. The lance pivoted up and struck the EE on the left cheekbone, causing a cut that needed stitches to close.

The EE pinched hand on chute while removing it. The EE suffered broken hand.

Employee was passing bricks in the kiln and a brick fell and hit EE's hand.

While welding inside the Stage 5 Cyclone, the employee turned and fell off of the work deck to the one 4 1/2 feet below. The employee suffered a 2.3mm tear in the left shoulder ligament.

On February 7th, 2022, a non-injury unplanned ignition occurred inside the coke mill dust collector at the Chattanooga Plant. The incident was immediately reported to MSHA.

While observing the air dryer control panel, the Maintenance Planner turned and struck EE's shin on a cinder block that EE's assistant had been using to sit on. Due to the Planner's age and existing health conditions, a two-inch skin tear occurred which required seven stitches to stop the bleeding.

Employee came to work after mowing EE's lawn for 2 hours in 93F/65%H weather, then went straight to work in the Clinker Feeder Hall changing out a belt roller for two hours without taking a break. Felt ill, and partner took EE out. Supervisor took to hospital for hydration due to heat illness.

Employee was attempting to straighten a lifting eye for one of the Grizzly Back Stops with a 5' pry bar. EE strained and felt a pop in EE's groin - diagnosed as an inguinal hernia.

After shoveling, felt discomfort in abdomen. Went to the doctor the following Monday (04/30/2018) and was diagnosed with a hernia.

At the start of a Monday shift, the Quarry Haul Truck driver bumped the berm while backing into the load out bench, causing a jostling to the driver's body. When dumping the load, the driver felt a twinge in lower back when the driver twisted. Doctor stated strain, and advised one week of no truck driving.

EE stepped on cross beam to gain advantage to see over conveyor belt; foot slipped and he jerked his arm exasperating a preexisting non-work related shoulder injury he was being seen for.

While drilling through a piece of airslide fabric, the EE placed his finger on the backside of the fabric and contacted the drill bit.

Employee was performing routine cleaning in the Preheat Tower with a WOMA gun. When the employee initiated the WOMA gun, hot feed blew back onto the employee causing burns to the back of his unprotected head, neck, and left ear. EE refused to wear PPE.

While EE was position the barge, he slipped and struck his wrist against the decking causing a suspected scaphoid fracture of his right wrist; soft cast and restricted duty prescribed.

Employee alleges that when he raised the face shield on his hard hat, dust got behind his face shield and safety glasses and got into his left eye.

Dust blew into employee's eyes. Caused conjunctivitis requiring prescription medication.

Employee was burning off a bolt. A piece of hot metal caused 2nd degree burn to palm of his hand.

Employee was stepping across barge. It was raining and was slippery. He slipped and hit his knee on the barge. He has a contusion to his knees

Supervisor was attempting to unplug blockage on 3rd stage of pre-heater tower using an air lance. Hot material back flushed causing 2nd degree burns on his forearms and back of head.

Employee was positioning a mill liner plate. It slipped pinching his right middle finger, causing a laceration requiring stitches

Employee stated that he felt faint. He stated the last thing he remembered was squatting down.

Employee stated that he was shoveling material from top of gyp silo and he felt pain in his abdomen area. He felt he had re-injured a previous hernia.

Metal splinter in employee's right forearm - 2 sutures

Employee was turning valve, lost balance, slipped and landed on his buttocks on the floor

Employee was using a punch bar to clear a blockage at the discharge chute at our RMS reversible belt. He was standing on a platform. When the material broke loose, his momentum caused him to lose his balance and fall from platform.

Employee stated that he felt pain in his left forearm after turning a valve. He was seen by a physcian and released back to full duty. He was sent to a specialist on 11/09/2010. Although the doctor returned him back to full duty, he did get an injection and was prescribed physical therapy. Therefore it is being classified as a reportable injury as of this date.

Employee stated he was pulling a piece of damaged conduit that was bonded tightly to some melted wires. He stated that while he was pulling on the conduit trying to remove the damaged wires, the conduit abruptly broke loose, causing his left bicep to pop causing pain.

Employee was using a hook to pull a branch out of the water at a barge loading operation. Employee stated that he strained his back.

Employee stated when he was dismountlng the front end loader, his hand slid on the hand rail and he struck his elbow on the platform. (He reported the injury to the safety manager on 10/21/2009.)

Employee was attempting to unplug a kiln seal hopper. He was using a chipping gun and must have loosened dust from an eye beam. When he took off his goggles, a small amount of dust got in his left eye.

Employee was walking up stairs carrying a ladder. He tripped and braced his fall with his right hand. Caused pain in his right shoulder - possible muscle tear.

Cable broke on Ameco loader causing the operator station to jerk and the employee fell into the handrail causing discomfort in his stomach and back area.

employee was stepping down from a palletizer and tripped on an emergency stop cord causing him to fall against a handrail. He fractured 4 ribs.

Employee was attempting to move a piece of plate steel that was setting on steel grating. He was using a pry bar to wedge in grating to move the plate. Instead of the plate moving, the grating shifted off the support beam, allowing the grating and employee to fall to the ground below. Sprain wrist and contusion.

Employee had been loading material in a hopper with a front end loader. He walked up the ramp to check the level of material in the hopper. As he was walking down the loading ramp, he slipp ed on loose material. He braced himself with his right arm, causing discomfort in his right shoulder.

Removing panels and one slipped and cut his left middle finger.

Employee complained of left shoulder soreness from a work activity. He was holding a return roller to install it. He worked without incident until 11/4/08. He stated that he wanted to see a doctor due to his complaint back in April.

While doing demo work, the employee was cutting an 8" I-beam. The rigging slipped, the employee's glove was caught by the I-beam as it was falling, pinning his hand. Laceration on right ring finger required stitches.

Employee alleges he struck his little finger on his right hand while lifting a shovel. He only wanted to report it and did not want any treatment at the time. He later complained that it was swollen and wanted to seek medical treatment. He had PIPJ Arthrotomy to repair ligament on 11/20/08 and returned to work on his normal scheduled day on 11/22/08 on restricted duty.

Cut on finger required 2-3 stiches. Was back to work same day.
