Mostrando los 12Employee was greasing the loader first thing in morning when EE slipped and fell.
Employee was exiting loader, when ee reached the 3rd rung hands slipped on handrails causing ee to fall to ground. At the time it was raining hard and handrails were wet. Took employee to ER to get checked out and they found nothing other then some bruising. Dr's excuse said to return in 2 days but employee was not scheduled to work except for half a day the following day.
Employee was carrying a chain over shoulder at secondary plant, ground was muddy, foot slipped causing the chain to swing around and hit employee in the mouth chipping a tooth.

Lanyard caught him but he tit his rt hand on scaffold causing injury. He did not report to management. Co-workers asked if he needed to go to Dr, he stated he was okay. Reported 11/12/12. He has surgery for joint locked in flexion.
Employee was walking to #1 tailpulley when he stepped across about a 2 foot puddle of water, he felt a pop in his leg. Employee pulled a muscle in upper right leg, causing him to use crutches with limited walking.
Employee had mud on his boots when he stepped on a piece of conveyor hood that was on ground and slipped landing on back and hip.
Employee was on platform below jaw crusher when his foot slipped causing him to fall backwards on a rock causing bruising to lower back.
EE WENT TO CLIMB UP SIDE OF DUMP TRUCK WHEN SLIPPED AND HIT RIGHT KNEE.
HIT HIS HEAD ON LADDER OF TRUCK AND FELL OFF. HESLIPPED WHEN CLIMBING LADDER OF EQUIPMEN & FELL AGAINST TRUCK, HITTING HIS HEAD.
