Mostrando 50 de 89An employee stepped then fell into an opening for a sump resulting in a laceration to a leg and scrapes to arms. Employee was seen by a site medical responder, and then brought to a hospital for medical treatment where sutures were used to close the laceration. Employee was released back to work without any lost days or restricted duty involved.
EE was getting back into her loader at the fuel building when her left heel snagged the ladder rung. This caused her to lose her balance and fall backwards onto the ground striking her wrist. She was evaluated by a Physician and diagnosed with a fracture to her left wrist. She was taken off of work.
EE was de-boarding his production truck, when he slipped and fell to the ground. The employee was evaluated and PT was ordered. A series of PT appts have been completed.

Lower Tower Hill: the site had large frost slabs with snow covering them. We could not see what was under the snow. I stepped on a rock. The rock slipped and rolled out from under my foot. I fell backwards on my left side. My lower left side came in contact with a sharp boulder and knocked the wind out of me. The walk-in clinic x-rayed and diagnosed a bruised lower left rib.
On 12/22/12 employee was walking to a vehicle when he slipped on the ice and reported of right knee pain. He was evaluated by a physician and knee surgery was performed on 2/14/13. He continued to work in his normal position from his date of injury up until surgery. Last Date Worked - 2/13/13. First Date Missed - 2/14/13.
WORKER WAS WALING ACROSS A SNOW COVERED ROAD ON HIS WAY TO LOOK AT A BUILT RITE CABLE HANDLER. WORKER SLIPPED ON ICE UNDER THE SNOW FELL BACKWARDS HITTING BACK OF HEAD, AND RIGHT ELBOW ON THE GROUND. THIS RESULTED IN A SLIGHT CONCUSSION AND R ELBOW PAIN.
The employee was getting out of the parked van at the end of the shift. As he was walking toward the office door, he slipped and fell on his left side, upper arm area. He was evaluated by a Physician after alleging continued problems. Physical therapy was ordered on March 20, 2012 and a series has been completed.

JOE WAS CLIMBING TO THE COUSNEST ON CABLE TRUCK.HE STEPPED BETWEEN THE CAB AND STEP GETTING HIS FOOT CAUGHT HIS FOOT TWISTED HE WAS TREATED FOR A FRACTURED TIBIA.
EMPLOYEE STEPPED ON A USED SHANK WEDGE WHILE PERFORMING WORK DUTIES. HE FELT A STRAIN TO HIS LEFT HEEL. HE WAS LATER DIAGNOSED AND TREATED WITH A CAST (IMMOBILIZATION) AND NO WEIGHT BEARING. H E WAS REMOVED FROM WORK ON 12/29/03.
WHILE UNWINDING 2"CONDUIT ELBOW WITH MOTOR LEADS INSIDE THE CONDUIT IT SLIPPED OUT OF HIS GRASP & LIKE A SPRING IT SPRUNG BACK & HIT HIM IN THE JAW. HE CAUGHT HIMSELF FROM FALLING OFF THE LADD ER. HE WAS TREATED & REMOVED FROM WORK FOR 2 DAYS DUE TO THE BACK SPASMS FROM BREAKING THE FALL.
CHECKING BEARING CLEARANCES ON A CONVEYOR BELT. HE WAS CLIMBING OFF THE CONVEYOR BELT WHEN HE PUT HIS RIGHT LEG ON THE FLOOR WITH ALL OF HIS WEIGHT & FELT HIS RIGHT KNEE TWIST. HE HAD SURGERY ON 6/5/03 FOR A MENISCAL TEAR. IT WAS DETERMINED ON 7/10/03 THAT THE UNDERLYING CAUSE WAS WORK RELATED VS. DISEASE.
WALKING IN FRONT OF VIBRATING SCREEN HE TRIPPED ON ROCKS, STUCK INT HE GRATING. TO CATCH HIMSELF FROM FALLING, HE GRABBED ONTO THE HANDRAIL AND STRAINED HIS BACK.
