EE was in the process of closing an overhead ventilation door when the counter weight cable failed causing the door to fall and strike the left foot. EE was evaluated by a Physician and taken off of work.
An employee using an ironworker sustained a finger injury. Taken to ER for treatment. Has been taken off work.
Employee had been moving 12" x 12" blocking with a partner. After finishing job employee was riding in a truck and began to feel pain in right inner thigh. Was seen by NP and returned to work. On 8-15-16 employee returned from one week of vacation and complained of increased pain. Seen by MD on that date and put on restricted duty.
EE was trying to open a food package with a knife. The knife went through the package and cut the EE's left index finger. The EE was taken to the hospital where EE received 4 stitches and returned back to work full duty.
Employee complained of discomfort in right hand and was seen by a doctor and evaluated for repetitive trauma. It was made known on 6-2-16 that work activities were a contributing factor to this illness.
EE was re positioning a stop rail, as EE picked it up EE felt a pop in left wrist. Evaluated by MD on 1-16-2016 and put on restricted duty.
Employee was walking down stairs and slipped. Had complaint of sore shoulder. The employee was evaluated by a physician on 2-4-16. Physical Therapy was ordered.
A mobile jib crane was being used to lift a piece of grating. When the grating did not move the crane was readjusted while still attached and under pressure. The grating cross bar failed and caused the chain assembly and crane arm to strike the EE in the face. EE was treated by MD and taken off work.
EE was welding a plate from the underside of a bucket and when he began to pry the plate off, it broke free, hit the ground and flipped onto the ee's R foot. He was evaluated and given restrictions to return to work. He returned to full duty on 12-31-15.
Alleged. Employee states feeling pain in knee while standing at toolbox, pain caused him to stumble. Employee states no specific event and is not sure if this is work related or not. X-rays were performed with no apparent finding. Employee was provided restrictions and restrictions accommodated. EE to full duty on 12-31-15.
EE states he had back pain after working with a partner to lift a small barrel of lube from the floor onto a portable cart. He refused treatment initially but upon wakening the next day, he requested an evaluation. He was evaluated by a medical provider and taken off of work.
While torqueing hold down bolts for rear leaf springs on fuel truck, mechanic felt strain in left shoulder. EE was evaluated by MD and given restrictions on 1-22-16.
Two employees were removing an inner race from a primary mill reducer shaft. During removal of the inner race, a piece of the race chipped off and became lodged in the left inner forearm of an employee. EE was seen by an EMT and site nurse and transferred to the ED for further treatment.
Mechanic installed a distribution hose on portable steam cleaner. When he started the steam cleaner hose came off, noise, caused mechanic to jump back twisting knee. Employee was evaluated and provided restrictions the department accommodated as normal work duties. 11/5/15 determined duties performed are not part of normal job duties and thus to be counted as Restricted Duty Days.
Alleged, conversation between supervisor and employee prior to shift at 1:20 pm, EE told supervisor of knee issues non work related and need to have fixed. Later in shift employee stated his knee was sore. Claim was denied by workers compensation, determined to be non work related.
EE felt twinge in left shoulder while loosening fill plug on the rear circle drive of GR20. EE used 3/8 ratchet with no excessive force when shoulder made a pop. EE did not want to be seen by EMT. Eval 08/26/15 lifting restrictions of no > than 10 lbs. Employee had been placed back to normal role prior to 08/26/15 evaluation.
Mechanic was moving wood blocking from one pallet to another when he experienced pain to his lower back. EE was evaluated by a physician and given restrictions.
An employee stepped then fell into an opening for a sump resulting in a laceration to a leg and scrapes to arms. Employee was seen by a site medical responder, and then brought to a hospital for medical treatment where sutures were used to close the laceration. Employee was released back to work without any lost days or restricted duty involved.
Alleged - Employee walking in basement near LN22 pebble mill lube system stepped on rocks, then reported twisted right knee and twisted lower back. Employee was assessed by site responder. Employee completed remainder of shift. 04/10 provided restrictions. Claim was denied by workers' compensation.
EE was using a Bobcat walk behind loader for clean up when his left hand was pinched between the machine and the cement wall. He was evaluated by a Physician after the event and diagnosed with a fracture to his left middle finger. He took himself off for his next scheduled shift due to the need for pain medication. He has returned to work.
EE was working on a spray bar and while attempting to remove the spray bar he injured his left pinky finger. He was evaluated by a Physician, diagnosed with a fracture and sutures closed the wound. He returned to work.
EE was preparing to run a test in the furnace. He had loaded the retort and was using a 3/4 inch combination wrench to tighten the bolts on the retort lid. While attempting to tighten one of the bolts, the wrench slipped and the wrench hit him in the mouth resulting in a chipped tooth. He saw his Dentist on 2-11-15 for the repair.
EE was working on 5 conveyor discharge chute replacement. During demolition of the chute, a piece of the chute dropped down and pinched the left hand resulting in a suturable laceration to his left index finger. He was evaluated by a Physician & nine sutures closed the wound and the ee returned to work.
EE was carrying a 2' long piece of 2 x 4 macro bar. He bumped his head which caused him to lose his grip on the bar causing it to fall. It struck the employee on the top of the foot. He was evaluated by a Physician and given a release to return to work with restrictions.
Alleged, employee states he was hosing, turned to his left, felt pain. was seen by on-site EMT and returned to work. Employee took self off work the following shift, went to personal chiropractor that he has been seeing for non occupational matters.
EE was getting back into her loader at the fuel building when her left heel snagged the ladder rung. This caused her to lose her balance and fall backwards onto the ground striking her wrist. She was evaluated by a Physician and diagnosed with a fracture to her left wrist. She was taken off of work.
While hoisting a pulley up a stairway, the employee's right foot became pinned between the tail pulley and the stair tread when the load shifted. He was evaluated by a health care provider and released back to work with restrictions.
EE reporting pain, numbness & tingling to both arms and hands. He was evaluated and diagnosed with Carpal Tunnel Syndrome. It was made known that it was determined to be occupationally related on 3-9-15. Surgery is scheduled for 3-18-15.
EE was using the walk behind loader to clean in the basement of the old filter bay. He drove into the pile & when lifting the bucket, the back of the machine lifted up and pinched the left hand. He was evaluated by a Physician; diagnosed with a laceration and fracture to left pinky finger. He was given restrictions & returned to work.
EE was de-boarding his production truck, when he slipped and fell to the ground. The employee was evaluated and PT was ordered. A series of PT appts have been completed.
EE was removing hoist cylinders. When the ee set a cylinder down, it rolled unexpectedly and struck the ee on the inside of his left knee. He was evaluated by a Physician on 10-21-14 and was given restrictions to return to work.
EE alleges back pain from leaning forward in an awkward position to weld. On 9/19 it was made known that he alleged a missed shift on 9/15 as related to his work injury. He was not disabled off of work by a health care provider. EE had self-treated with a chiropractor and on 9/24 the workers comp carrier determined to accept chiropractic tx with his comp claim.
Employee alleges right eye pain. No specific event stated; employee reports he'd been welding & grinding at work frequently. He was evaluated by a MD and a foreign object was removed. He was given Rx eye drops and narcotic pain medication. He missed his 8/15 shift due to taking pain medications. He was scheduled off 8/16-17 & returned to work full duty on 8/18.
Our worker was cleaning a restroom and as she was connecting a sprayer (that had a chemical called Century Q in it) to a garden hose the connection wasn't secure spraying back on her face even with safety glasses on the water and chemical ran down her forehead and into her left eye.
While changing troughing rollers on a conveyor, employee reported a sudden onset of lower back pain that radiated down his right leg. He was evaluated by a physician and taken off of work.
The material the employee was moiling broke free, causing the moiler to twist suddenly and strike his right middle finger. Employee was evaluated by a physician. No fractures were found. Employee was released back to work with a restriction of no moiling.
Employee was backing a dozer up and reported that he hit a chunk of rock, causing him to be jostled in the dozer and experience back pain. He was evaluated by a physician and taken off of work.
EE states he was walking & stepped on pipe on floor in the walkway. The pipe rolled & he began falling. He caught his fall by grabbing on to a machine guard. Later in the day he felt back pain. He went to the onsite nurse. Ice/ pain pills given. The decision was made by SKF to allow employee to rest & returned to work 7/10/14. A doctor did not put EE out of work.
Employee attempted to latch a dry basket in the up position in the women's locker room at the end of shift. She thought it was latched, but when she let go the basket fell and struck her in the head. She was evaluated by a physician and sutures were administered to close the wound. She was scheduled off on 7/4. She returned to work full duty for her next scheduled shift on 7/5.

Employee alleges back pain from bending over to install a seat belt. He was evaluated by a physician and taken off of work.
Employee was using a barrel cart to remove an oil drum from a pallet. While lifting the drum, a board on the pallet broke, causing the drum to fall over and the employee to twist his right knee. He was evaluated by a health care provider and released back to work with restrictions.
Employee alleges back pain from driving production truck over bumpy roads and a railroad crossing. He was evaluated by a physician and released back to work with restrictions.
While striking a roller bearing with a ball-peen hammer, a small piece of roller bearing broke off and struck the employee in the right groin area. He was evalauted by a physician and taken off of work.
The employee was using a grinder to perform work when the grinder "jumped" and caught his left index finger, pulling it between the guard and cutoff wheel. He was evaluated by a physician and four sutures were used to close the wound. He was released back to work with restrictions to keep the wound clean and dry.
A platform fell off of the wood dunnage striking his right foot above the steel toes. Contusion to right foot.
A production truck driver reported of being jostled while his truck was being loaded, causing back pain. He was evaluated by a physician and given Rx narcotics & muscle relaxant. He was released with restrictions, but unable to report for his 4/2 shift due taking the medications. He returned light duty for his 4/3 shift, was scheduled off 4/4-4/7 & released full duty as of 4/8.
Employee reported lower back pain while pulling a blow pipe from a sump. She was evaluated by a physician and taken off of work.
While loosening a c-lamp, employee felt a "pop" in right wrist. He was evaluated by a healthcare provider and given a splint. He was released to full duty work.
An incher chain fell, catching employee's left hand between the chain and mill sprocket. Employee was evalauted by a physician and diagnosed with two fractured fingers. He was released back to work with restrictions.
Employee entered an elevator near Empire Pellet Plant Repair Bay. Compartment proceeded to rise when elevator stopped between levels. Employee used portable radio to communicate situation. MSHA Hot line called 15 minutes into event. Employee "freed" after 50 minutes. No injury. Service company found open fuse in system, but no known reason for condition.
Employee was climbing down ladder and foot slipped off rung - banged right shin (3 stitches).
Driver was dumping a load of Coal and while the box was raising it became unstable due to frozen coal stuck in the trailer and an uneven surface which caused the truck and trailer to tip over.
Employee alleges right knee pain in the parking lot prior to the start of a 12 hr night shift. Employee was standing outside of his personal vehicle, leaned in to vehicle to grab his lunch and felt a "pop" in his right knee. He was evalauted by a physican, released with restrictions and completed his remaining shift. On 1/23 he was evaluated by a specalists and taken off of work.
On 1/11/14 the employee was using a pry bar to dislodge jammed rocks in a drop chute, when the bar shifted and the employee's left ring finger was struck against the side of a metal hatch. He was evaluated by a physician on 1/11/14 and diagnosed with a fractured finger tip. He was issued a finger splint and released back to work without restrictions.
Employee alleges that she struck her right knee on a cyclone structure at work around 8:45am on 12/19/13. The incident was first reported to supervision around 4:00pm on 12/19/13. She was evaluated by a healthcare provider on 12/20/13 and released back to work with restrictions.
Employee reported lower back pain while lifting a piece of grating. He was evaluated by a physician and taken off of work.
EE was removing trash from an office waste basket that seemed to be struck. She yanked the plastic bag, freeing it. Then the bag and the contents quickly came up. Unable to control this action her left wrist struck the corner of the metal desk chipping the wrist bone.
The employee is enrolled in the hearing conservation program and has an STS shift (age corrected) to his right (31.7) and left (36.7) ear after annual hearing test. His results were reviewed by an Audiologist and it was made known on 11-27-13 that this was reportable.
On 10/15/13 the employee tripped on the grating and fell onto her knees. She was evaluated by a physician on 12/19/13 and prescribed physical therapy. She completed a series of physical therapy sessions on 12/30/13.
Employee rolled right ankle while walking down stairs. He was evaluated by a physician and issued an air cast to wear on the affected ankle.
Employee was barring lodged materials when the bar struck his hand and injured his right ring finger. He was evaluated by a physician and diagnosed with a fractured finger. Sutures were administered and employee was released back to work with restrictions. Employee could not return to light duty work as scheduled due to taking narcotic pain medications.
Employee was cutting a burn ring when a piece of hot slag entered into his boot, causing small burns to his left foot. Employee treated with his personal doctor on 9/26. He was taken off of work and missed his scheduled shifts on 9/25-9/27. Employer was made aware on 9/30 that the missed time was related to his to his reported 9/5 work injury.
Employee alleges that she slipped in the women's locker room and injured her left shoulder. She was evaluated by an ER physician and taken off of work for a left shoulder fracture.
Employee made contact with supervisor to inform of being in an inoperative elevator located in the Empire Crusher 1 Building. Supervisor responded to the site to try operating control buttons. The elevator remained inoperative, so a service tech was called. About 4:00 am MSHA Hotline was notified of incident. Service tech reset the elevator and employee was "freed" at 4:18.
On 6/30/13 employee reported a sore back from getting in and out of a service truck. Employee was evaluated by a physician on 7/1/13 and given restrictions. He was released to full duty work as of 7/8/13. Please note: This report is being submitted at this time to terminate a citation issued during a Part 50 audit.
Employee reported right knee swelling and pain after repeated entry and exit from a tire handler. He was evaluated by a health care provider on 7/9 and released with work restrictions that prevent him from working within his normal job scope.
Working on an engine compartment, employee felt debris enter right eye. Was evaluated by an ER physician on 6/8 and given a referral to eye doctor on 6/10. Employee didn't report for his 6/9 shift due to eye complications. He was evaluated by the eye doctor on 6/10; a foreign object was removed from eye, Rx drops were issued, & the doctor took him off of work for the date of 6/9.
While lifting a grid cover from a water truck, employee felt a "pop" in his left bicep. He was evaluated by a physician and released back to work with restrictions. Surgery has been scheduled for 6/17/13.
Employee alleges right knee pain after deboarding a tire buggy. Employee reports a twinge of pain in his knee after his right foot caught the grating of the stairs. He continued working the remainder of his shift. On 5/24/13 he was evaluated by a physician and given restrictions.
The ee underwent a comprehensive audiological eval after an abnormal hearing test. It was made known on July 2, 2013 that the employee's work history and test results are consistent with noise-induced hearing loss; the workplace being a significant contributing factor. The ave. age corrected change in the left was was 31.7db from the baseline at 2, 3 , 4 K.
Employee reported back pain after slipping on packed snow in parking lot. Employee was evaluated by a physician and physical therapy was prescribed. Employee attended second session of PT on 6/12/13.
Employee was putting a tool away in his locker when his left pinky finger allegedly caught the shelf. He was evaluated by a physician on 3/14/13 and taken off of work for a fractured 5th metacarpal.
While using an air chipper to chip rust off of a beam, employee felt a foreign object enter his left eye. He was evaluated by a physician. The foreign object was removed using a cotton swab and the employee was given a prescription for antibiotic eye drops.
On 2/23 the employee reported back pain after allegedly slipping in the parking lot while waking to his personal vehicle. He was evaluated by a physician on 2/23 and was taken off of work.
On 2/18/2013 the employee bent over to set down a jig. When he stood up back up, he allegedly reported of back pain. He was evaluated by a physician and physical therapy was prescribed. The employee attend his second session of physical therapy on 3/7/2013.
Employee was lifting a 39 lb brick to a partner above him when he felt immediate pain in his right shoulder. He was evaluated by a physician and released back to work with restrictions. Due to his seniority, employee went to layoff on 2/17/2013.
Employee slipped on ice outside and felt pain in his left knee. He continued to work in his normal position without restrictions. On 4/9/13 he was evaluated by a physician and was given a prescription for a knee brace and physical therapy. The knee brace was received on 4/10/13 and the first physical therapy session will begin on 4/16/13.
Lower Tower Hill: the site had large frost slabs with snow covering them. We could not see what was under the snow. I stepped on a rock. The rock slipped and rolled out from under my foot. I fell backwards on my left side. My lower left side came in contact with a sharp boulder and knocked the wind out of me. The walk-in clinic x-rayed and diagnosed a bruised lower left rib.
While striking a lip shroud with a sledge hammer, a piece of metal broke off and struck employee's left calf. Employee was evaluated by a physician; sutures were administered and employee was released back to work with restrictions. Employee was scheduled off from 1/31-2/3 and was released back to full duty work as of 2/4.
On 1/23/13 a chunk of material rolled off of a mining face and made contact with the shovel, causing the shovel operator to be jostled. He was evaluated by a physician on 1/23 and released back to full duty work. On 10/10/13 he was prescribed physical therapy. He completed a "series" on 10/21/13.
Employee reported back pain after moving a 73 lb launder cover. She was evaluated by a physician on 12/26/12 and released back to work for 12/30/12 with restrictions. Employee was scheduled off on 12/29/12-1/1/13. Due to pain and pain medications, she could not work on 1/2/13. She was evaluated by a physician again on 1/3/12 and was taken off of work at this time.
On 12/22/12 employee was walking to a vehicle when he slipped on the ice and reported of right knee pain. He was evaluated by a physician and knee surgery was performed on 2/14/13. He continued to work in his normal position from his date of injury up until surgery. Last Date Worked - 2/13/13. First Date Missed - 2/14/13.
On 12-17-2012, employee reported a "pop" in his right middle finger while lifting a trolley onto a beam. He was first evaluated by a physician on 1-10-2013 and was prescribed a splint for his right middle finger.
The employee was operating a fork lift when he hit a launder and the forklift stopped suddenly. As a result, the employee hit his mouth on the steering wheel and sustained a laceration to his lower gum. He was evaluated by a physician on 12/10/12 and the laceration was closed with sutures. He was released back to work full duty at this evaluation.
While using a pneumatic grinder with wire wheel, the grinder kicked back and struck the employee in the right knee, causing a laceration. The employee was evaluated by a physician on 12/7. Dermabond medical glue was used as an alternative to sutures to assist with wound closure. The employee was released back to work full duty for his next scheduled shift on 12/10/12.
The injured party was a driver. The driver was uncoupling his lead and pup trailers, and through this process strained his shoulder by over-exerting himself. The driver is undergoing physical therapy to regain full range of motion.
Employee's pickup truck made contact with a 240 ton production truck. As a result, employee was trapped in vehicle. He was removed and is currently hospitalized.
EE jumped up to tighten the guide roller safety clamp and as he jumped into position, his right thumb was caught in the new belt and roller guide.
On 10-22-12 the employee was struck in the right knee by a porta power head. He was evaluated by a physician and released back to work with restrictions.
Employee alleges left foot pain after traveling up and down stairs at work. He was evaluated by a physician on 10-2-12 and released back to work with restrictions. On 10-25-12 it was made known that this alleged claim would be accepted as work related.
While putting trash into dumpster at pit, picked up a bag and was cut on ring finger. Further investigation found it to be a food can lid still attached and sticking up.
The employee alledges back pain after opening a door at the start of his shift. He was evaluated by a Physician and taken off of work.
While closing an elevator door, the employee's left ring and middle fingers were caught in the door--resulting in a severe laceration leading to surgical repair. He was evaluated by a Physician and taken off of work.
On 9-7-12, the employee felt a pop in his left shoulder while lifting an impact wrench (8 lbs) to his co-worker. He was evaluated by a Physician, given an immobilizing sling and returned to work with restrictions.
While rolling a clamp over to a pick-up site, the clamp rolled on uneven ground and fell onto the EE's right foot. The EE was evaluated by a physician on 8/30/12 and was able to continue working full duty. In January 2014 the EE complained of non-resolving right foot pain. He was evaluated by a physican and surgery was performed on 3/13/14.
Employee was in a man-lift welding on a pipe hanger bracket. As he stepped down from the tool shelf of the man lift, the man lift shifted from his movement and his left thumb was pinched between the man lift railing and the bracket. The employee was evaluated by a physician and dermabond was used to close the wound.
The employee was using a 1/2 inch tap driven by a socket and ratchet combination to thread retaining blocks. While tapping, the tap broke and the broken end cut his left thumb while wearing gloves, causing a laceration. He was evaluated by a Physician and the wound was closed with sutures.
Employee was struck in the head with falling debris and buildup while working in the pocket. He was evaluated on 5-15-12 and released back to work. He took himself off of work on 5-16-12 and 5-17-12: returning to duty on 5-18-12.
Employee complained of bilateral numbness and tingling in his hands. He was evaluated by a Physician and on 7-23-12 it was made known that his work activities contributed to the diagnosis of CTS.
Employee was handling a 13.8 V cable (that was partially coiled), while picking up the coil- he walked over a berm and twisted the cable to align the pins in order to plug in the cable. He felt pain in his lower back while doing this task. He was evaluated by a Physician on 5-12-12 and taken off of work. He returned to duty on 5-17-12.
On 5/8/2012, employee was allegedly lifting batteries into the back of a service truck when he reported of bilateral middle finger pains. The employee was evaluated by a physican and on 3/4/2013 was issued a left fourth digit finger splint.
Welding/Arc Flash.
The employee was laying flat on his back checking and changing flange bearings that were 1-1 1/8 inch in diameter and weighed approx 1 lb when he allegedly reported back pain. The employee was evaluated on May 1, 2012 and taken off of work.
Employee complained of wrist discomfort related to his work activities. He was evaluated by a Physician and on June 4, 2012 it was made known that he was diagnosed with Carpal Tunnel Syndrome. It has been determined that his work activities appear to be a significant contributing factor to his condition.
WORKER WAS WALING ACROSS A SNOW COVERED ROAD ON HIS WAY TO LOOK AT A BUILT RITE CABLE HANDLER. WORKER SLIPPED ON ICE UNDER THE SNOW FELL BACKWARDS HITTING BACK OF HEAD, AND RIGHT ELBOW ON THE GROUND. THIS RESULTED IN A SLIGHT CONCUSSION AND R ELBOW PAIN.
EE experienced pain in his lower back while working inside the line 7 feed chute. He was evaluated on 2-17-12 and taken off of work. He missed one scheduled shift. This may potentially be related to a previous incident in January 2012.
The employee was getting out of the parked van at the end of the shift. As he was walking toward the office door, he slipped and fell on his left side, upper arm area. He was evaluated by a Physician after alleging continued problems. Physical therapy was ordered on March 20, 2012 and a series has been completed.
The employee was walking around a pickup truck to open the overhead door, slipped on the ice and fell on his right arm. He was evaluated by a Physician on 1-8-12 and returned to work with restrictions working restricted duty.
Employee was traveling in base area of unit 2 pan conveyor in pellet plant. Employee was maneuvering away from a sudden overflow of water when he struck his leg against a diagonal cross brace. Employee sustained a laceration to his lower right shin. Laceration required stitches which were administered at Bell Hospital in Ishpeming, MI.
Employee slipped and fell on his left hip in the parking lot coming into work. He was evaluated by a Physician and given restrictions.
Employee allegedly states she received pain in her left shoulder after shoveling up spilled coal that was a result of a conveyor jackpot. She was evaluated by a Physician and Physical therapy was ordered.
The employee was lifting 5 gallon pails of chemicals approx. 44 inches and felt a strain in his right shoulder. He was evaluated by a Physician and had surgery on 2-24-12.
Employee threw a chain over a beam with his right arm and the needle nose pliers that were clipped to his jacket came up and him hit in the front left tooth and chipped it. He was evaluated by a Dentist and it was temporarily repaired on 11-9-11.
An employee was using an impact wrench attempting to remove rusted bolts from Line 17 Finisher Feed Autovalve. The employee looked up and some debris fell into his eye. He was evaluated by a Physician on 11-3-11, prescription eye drops were ordered and he returned to work.
EE was working on a cobber feed pump. In the process of changing the liners, a pallet had to be moved closer to the forklift. The suction liner slid onto the left hand, trapping it between the liner and the pallet. The ee pulled his hand free and a short time later, he felt back pain. He has been evaluated by a Physician and a steroid injection was ordered & admin on 12-16-11.
Employee was loading parts, felt pain in r shoulder. He did not seek medical attention at that time. Visited doctor on 1/31/12 and was prescribed physical therapy making it reportable as of that date.
Using a grinder to remove pipe & hangers & supports on a lube oil skid. Cut thumb while switching hands on the grinder.
An employee was riding alone in an elevator when failure occurred. An intercom was used to request assistance. MSHA promptly notified. Service technician was able to float the car and open the door to allow the employee to walk out of the elevator. The individual stated no injuries or illnesses resulted from incident. Time of "entrapment" was 6:50 till 8:25.
Employee was putting an old hood from the lab into a dumpster when he felt something in his eye. His eye was flushed and he felt irritation in his eye. He was evaluated by a physician-no foreign body noted; however, eye medication was prescribed.

The driver of the mine water truck had stopped in the middle of the road to advise the GSSC patrol vehicle that there was brush on the front of the vehicle. The GSSC patrol officer was looking at another area when he returned his eyes forward he had struck the rear of the water truck.
EE was winding up the electrical cord when another EE called him from the next room, EE dropped the cord and proceeded to see what she wanted. Employee was assisting with lifting a separator filter onto a deck, when his finger was caught between the deck and filter. This resulted in a fracture to his left ring finger.
