EE was walking to a conveyor belt to check the condition of the belt splice, when EE slipped on a patch of ice and fell. Initial diagnosis was shoulder pain, later diagnosed with rotator cuff tear.

Cleaning up materials on the decking around the new crusher platform. Employee had harness on, the lanyard caused employee to fall into the hopper opening, striking lower back against an I-beam.

Greasing the loader. Several grease fittings weren't taking grease, needed to be changed. Employee climbed ladder, attempted to open door from too far down. Employee lost grip as they opened the door with the right hand, causing them to lose balance and fall, landing on right hip.

Welding back side of hopper frame, EE was on one side of hopper, another EE on the other side of the hopper. There was frozen material build-up on catwalk of hopper and scrap steel in the area. EE slipped and fell to the ground.

Loading customer truck. The air line to the gate valve froze due to subzero temps. EE attempted to access platform, carrying a weed burner to thaw air lines. EE's hand slipped off the rung, EE fell backwards, left arm struck handrail. EE then fell down the set of steps.

Exiting excavator cab onto the tracks, which were icy. Employee did not use 3 point contact, went out face first, and slipped to the ground, but landed on their feet. Employee initially indicated they were not hurt.

Getting in/out of truck to load bins. Exiting truck, put foot down on what he thought was a step, but was fuel tank. As he attempted stepping to catwalk, his foot slipped, he fell down, hitting wrist on catwalk

Employee was descending stairs, carrying a small piece of steel in his right hand & using his left hand as guide on handrail. He misjudged a step and slid. His left hand slid on handrail, his left 4th & 5th fingers split between the handrail support, pulling his 5th finger backwards. He went to occ health & was given a splint. No fracture.

Employee was walking across the lot near the plant to measure a piece of steel to cut. Employee was walking on uneven ground, he felt his ankle give out and he fell to the ground. Employee received a sprained right ankle. Employee was off work.

Employee was walking down small hill on road by the Lab. His feet slipped and went out from under him causing him to fall to the ground and landing on his right side. The hill was snow covered. Employee had a contusion to his right elbow and right ribs. He was restricted but there was work for him within the restrictions.

Employee was at the secondary plant and was stock piling crushed stone from the plant to the yard. After loading his truck, employee was getting back in the truck when he slipped on dust and loose stone. Employee fell and twisted his left ankle. Employee received a left ankle sprain and is restricted.

Employee filled the truck with fuel and was walking to fill out the fuel ticket and tripped over the fuel hose causing him to fall and as he fell he wrenched his left elbow and shoulder. He received a strain to the elbow and shoulder. He is restricted.

EMPLOYEE WAS TAKING DOWN AN ELECTRIC MOTOR FROMA COVNEYOR INJUREDEMPLOYEE AND ANOTHER EMPLOYEE WAS CARRYIN GIT ON A BAR WALKING DOWN A PLATFORM. INJURED EMPLOYEE FELT A PULL IN HIS BACK. HE T HOUGHT IT OWULD BE OK BUT PAIN BECAME WORSE AND HE WENT TO THE CLINIC THE NEXT DAY.

EE WAS WALKING ACROSS GARAGE UNROLLING A GARDEN HOSE WHEN THE TOE OF HIS ARTIFICAIL LEG CAUGHT ON A HIGH SPOT ON THE FLOOR CAUSING EE TO FALL OVER A STEEL SAWHORSE AND LANDED ON HIS SHOULDER. HE THOUGHT HE WOULD BE OK. LAFER HAD DISCOMFORT WENT TO CLINIC 10-21-94. WAS REFUNDED TO SPECIALIST AN ARTHROGRAM REVELAED A ROTATEN CUFF REAR. EX SCHEDULED FOR SURGERY.

EMPLOYEE WAS UNDERNEATH BINS AND STEPPED OFF CATWALK AND STEPPED INTO A HOLE THAT WAS COVERED IWT MUD. EMPLOYEE TWISTED HIS ANKLE.

EE WAS CLEANING STONE BIN AND SLIPPED IN THE BIN AS EE WAS FALLING HE CAUGHT HIMSELF AN DLANDED ON HIS HAND.

EE WAS GETTING OUT OF THE 769 C TRUCK AND TRIPPED ON THE CATWALK HE FELL ABOUT 8 FEET CATCHING HIMSELF WITH HIS HANDS AS HE FELL HE HIT HIS LEG ON THE CATWALK.

EMPL WAS GETTING OUT OF HIS TRUCK AND STEPPED DOWN ON A PILE OF STONE AND TURNED HIS ANKLE
