EE was getting a guard off some bolts, and the pry bar slipped and smashed EE's right ring finger. Tip of finger had a small fracture and the finger nail had to have a hole poked into it to let blood drain out from under finger nail.

EE was walking to a conveyor belt to check the condition of the belt splice, when EE slipped on a patch of ice and fell. Initial diagnosis was shoulder pain, later diagnosed with rotator cuff tear.

Repetitive motion - steering, shifting gears, switching from forward to reverse - all with left hand, has caused left hand pain

Maintenance work-pin removal. While driving pin from a hydraulic ram, a metal chip came off when the hammer struck the punch, penetrating right thigh, requiring sutures.

Using cutting torch to remove snubber pulley from conveyor frame. When removing, snubber fell and pinched left thumb. He was taken to hospital and surgery done.

Man was standing on step ladder welding. He stepped off side of ladder and fell hurting his foot.

When greasing loader, EE caught his left foot in the bucket brace.

EMPLOYEE WAS CLEANING MUD FROM LOADER BUCKET, WHEN A CHUNK OF MUD ROLLED OUT OF BUCKET AND HIT HIS LEFT KNEE. POSSIBLE STRAIN OR FRACTURE.

EE WAS DOING ROUTINE MAINTENANCE IN THE MILL. WHILE CARRYING A HEATER UP THE STAIRS HE FELT AN INCREASE PAIN IN HIS RIGHT SHOULDER, WHICH HE HAD SORENESS ALREADY FROM LIFTING STEEL TO REPAIR A CONVEYOR 2-3 WEEKS EARLIER.

WELDING AND TIP OF WELDING ROD BURNED THRU GLOVE AND INTO FINGER.

OPERATING AIR HAMMER TO BREAK LOOSE LIME DEPOSIT ON MILL CHUTE - WRIST HAS HURT SINCE.
