Employee was walking to work site to start connecting a water line to a portable pump. The employee stepped into a uneven surface, causing EE to fall, twisting EE's left knee.

Employee claims left leg slipped behind them while exiting the restroom. Employee claims left knee made contact with the floor. Employee stated that there were no physical hazards present. Employee did not seek medical attention until 12/24/18, and provided a work restriction form on 12/26/2018.

Miner was attempting to pull a piece of conveyor skirting from a chute when EE lost balance and fell backwards onto the catwalk. Miner sustained a lacerations to EE's left hand requiring 4 sutures to close.

Injured employee had just completed rigging a load on the 300 ton Link belt crane. EE made eye contact with the operator and asked if EE could come and talk to EE. While up on the deck talking to the Crane operator EE fainted and fell to ground and broke left clavicle. Employee was taken to Hospital where EE was found dehydrated and taking 3 meds for blood pressure.

EE states was climbing stairs when left knee buckled and twisted.

03/17/14 Miner reported falling while descending screen house stairs, pain to right knee. 04/03/14 Miner requested to see doctor, released full duty. 05/02/14 Miner had follow up doctor apt. 06/12/14 Miner saw orthopedic doctor. Company was not notified of specialist 06/12/13 until 08/04/14. Injury meets reportable status on 06/12/14.

Miner was dismounting Bobcat 130 skid-steer loader when he lost his footing. Miner attempted to catch himself when he heard his shoulder pop, and he went to the ground. Miner was treated and released for a dislocated shoulder at the Heart of Texas Memorial Hospital ER.

Miner was removing wet sand from shakers on A Deck. When employee turned to move to the next access panel, his left toe caught on the I-Beam causing his knee to sprain/strain.

Employee was starting to walk down a set of steps from the palletizer equipment, lost his balance and bent his left wrist on the handrail.

EE, mechanic was descending down an a-frame ladder backwards after completing a torch job. When he stepped off the ladder, his foot slipped on the concrete located in the launderer. Employee descending down ladder improperly and working in a slippery area.

Changing out oxygen bottle. Had put up empty bottle and was moving full bottle, he picked up the bottle to go over some welding leeds and his foot got caught in welding leed. He balanced and fell holding bottle which caused his right hand between the ground and bottle.

Employee was carrying chains from the feed hopper back to the shop. His knee simultaneously "popped and buckled". The employee did not report this alleged injury until 03/05/10. The employee was placed on restricted duty on 03/17/10.

Employee was lifting empty sand bags onto platform by the bagger; as he lifted the bundle he felt his back lock up, his foot slipped and he fell to the ground. He stated that he hit is leg on either the guard or hand rail brusing his leg. He has also complained of back pain and sought medical attention. He did not loose any work time.

Truck driver entered mine property to pick-up load of material. While traveling from truck to scale house stepped through piece of floor grating and lacerated inside of left leg. Driver received ~7 stiches to close laceration. Driver returned to truck that night. Spent night in his truck, and left property @ ~8:00am on 04/26/07.

Employee was stepping down of the bottom step of the 970F endloader, which is approximately 18 inches from the ground. Employee was trying to reach for a concrete wall with his foot which was about 3 feet back from the endloader. Employee failed to reach the concrete wall which caused him to fall backwards and the middle of his back striking the concrete wall.

EE was using 2' water hose to clean up sand build-up. The hose was positioned differently than usual. He did not check to see if hose had enough slack to pull forward. As he went to pull, it got hung up and he stopped abruptly and fell to the concrete and hit his right elbow. He was placed on restricted duty. Co doesn't have restricted work program so this is a lost time accident.

EE was descending from the lower (midpoint) of the track area of the D8L Dozer, when he lost his footing and slipped off the equipment injuring his right lower leg. He did not need medical treatment until 5/23/06. He was released for restricted work activity, but there is none available. His first lost workday is 5/24/06.

Employee was stepping down from ladder and missed last step which caused him to slip to ground level. Resulted in sprain to his left ankle.

Operator was on walkway at creek pump to clean screen. the walkway frame broke & the operator grabbed the railing to keep from falling. At that point he felt a pull in his rt. shoulder. He did not see dr until 3-3-05 & has not lost time. Amendment: After 6 months the dr determined that surgery was reqd to repair a torn ligament. This was not reportable till he reqd surgery 10/13/05

EMPLOYEE WAS CLIMBING DOWN LADDER ON 980C LOADER WHEN HIS LEFT FOOT TOUCHED THE GROUND, HE APPARENTLY STEPPED ON A ROCK TWISTING HIS ANKLE.

Miner was helping to place in trash pump in quarry. While lifting pump off of pallet with another employee, his left boot became stuck in deep mud causing him to loose his balance and twist his torso awkwardly. He strained his lower back.

EE HAD OUTSIDE MILL BDLG., TO MOVE 50W STACKER. MAINTANCE WAS FABRICATING NEW FLOORING FOR NEW CONVEYOR ACCESS. THEY BRIEFLY REMOVED FLOORING TO CUT & EDGE IN ORDER TO FIT. AREA WAS NOT PROPERLY TAPED OFF--EE WALKED BACK INTO MILL & FELL 4' INTO HOLE. SUFFERED GASH TO LEFT SHIN AREA.

EMPLOYEE WAS WASHING DOWN UNDER A STACKER CONVEYOR WITH A WASH DOWN HOSE. HOSE PRESSURE SURGED CAUSING HIM TO LOSE HIS BALANCE AND FALL INTO THE CONVEYOR FRAME. HE SUFFERED CONTUSIONS TO HIS R IGHT SHOULDER AND RIGHT KNEE.

THE EE TRIPPED AND FELL HITTING HIS HEAD ON A WHEELBARROW.

EE CLAIMS TO HAVE MISSED A STEP ON A 3 STEP PLATFORM, FELL AND TWISTED HIS LEFT ANKLE.

EE STPPED OFF THE LADDER & SPRAINED RIGHT ANKLE

EE PLACED A LADDER TO STOCK STORAGE SHELVES. A PUMP SHAFT WAS AT THE FOOT OF THE LADDER. EE CLIMBED UP THE LADDER TO STOCK SHELVES. WHEN HE STEPPED OFF THE LADDER, HE STEPPED ONTO THE PUMP SH AFT LOST HIS BALANCE, CAUGHT HIMSELF WITH LEFT HAND.

EE WAS TAKING A 5 GALLON BUCKET OF OIL TO THE SCRUBBERS. BENT DOWN, HIS FOOT SLIPPED. HE HIT ACROSS HIS RIBS ON A RAIL.

EMPLOYEE OPENED A FLOOR GRATING ACCESS PANEL TO CHECK A VALVE. LEFT PANEL OFF AND WENT TO CHECK ANOTHER PIECE OF EQUIPMENT. WALKED BACK THROUGH AREA AND FELL INTO THE OPENING. BRUISED RIBS AND ELBOW.

AS EE STEPPED ONTO CAB ENTRY STEP ON WATER TRUCK, HIS FOOT SLIPPED OFF OF THE STEP CAUSING HIM TO HIT HIS UPPER LIP AGAINST THE INSIDE DOOR HANDLE. EE NOTED HE HAD OIL ON THE BOTTOM OF WORK BO OT CAUSING HIM TO SLIP. THE RESULTING LACERATION TO LIP REQUIRED SUTURES.

THE EMPLOYEE WAS STACKING SAND BAGS AND MISSED ARRANGED ORDER, SO THE PALLET NEEDED TO BE RESTACKED. HE PULLED ANOTHER PALLET OUT OF THE MAGAZINE RACK AND LEANED IT UP AGAINST IT AND WENT TO THE OTHER SIDE OF THE PALLETIZER. WHILE GETTING THE PALLET HE SLIPPED AND FELL AND THE PALLET HIT HIM IN THE FACE AND THE SHOULDER ON THE LEFT SIDE.

EE WAS HELPING REMOVE SPEED REDUCER COUPLING ON LEWISTOWN MILL AND SLIPPED WHILE STEPPING OVER SPEED REDUCER. TO BREAK HIS FALL, HE REACHED OUT WITH RIGHT ARM HITTING THE FLOOR RESULTING IN A BROKEN ARM.

EMPLOYEE WAS MOVING A SMALL STEEL STRUCURE WITH THE CRANE. HE SET STRUCTURE DOWN. CLIMBED UP ON CROSSBEAM (ABOUT 4'HIGH) TO UNHOOK CRANE. SLIPPED AND LANDED ON HIS WRIST AND SPRAINED IT.

BRUSHING SCREENS AND NOTED THAT GATE ON SCREEN MAY BE HUNG. STEPPED UP ON EDGE OF SCREEN ELEVATING FEET ABOVE STAIRS, APPROX. 1'. REACHED TO FEEL IF GATE HUNG AND SLIPPED OFF SHAKER. APPARENTL Y TWISTING BACK.

EE WAS DESCENDING STAIRS IN VOC NORTH SCREENHOUSE. HE FELL LANDED ON HIS BACK. APPARENT MUSCLE STRAIN IN LOWER BACK.

EE WAS HELPING TO CVHANGE OUT A FAILED DOUBLE TRUNNION BEARING ON THE LEACH TUBE. AFTER REMOVING THE TRUNNION, EE APPARENTLY STAYED BEHIND TO CHECK THE TIGHTNESS OF THE PINION BEARING BOLTS. A LL OTHER EES HAD LEFT THE AREA. WHILE APPARENTLY CHECKING THE PINION BOLTS, EE FGELL APPROX. 6', BREAKING HIS RIGHT WRIST.

EE SLIPPED AND FELL WHILE STANDING ON A PIPE. THE PIPE WAS COVERED WITH SAND MAKING IT A SLIPPERY SURFACE. HE FELL APPROX 2' AND FELL AGAINST A STEEL CROSS MEMBER, BRUISING HIS RIGHT SIDE RIB AREA. HE WAS PULLING ON A CABLE HOIST WHEN THIS OCCURRED.

WORKING IN SCREENHOUSE, FELL FORWARD, BENT LEFT RING FINGER BACK, DISLOCATED IT AT MIDDLE KNUCKLE.

EE WAS EXITING CAT 633 SCRAPER WHEN HE ACCIDENTLY HIT THE STEERING WHEEL CAUSING SCRAPER TO LUNGE TO THE RIGHT AND SUBSEQUENTLY CAUSING EE TO L OSE HIS BALANCE. EE GRABBED RAILING ON THE CAT 6 33 AND INJURED HIS SHOULDER.

EMPLOYEE WAS WALKING BETWEEN K RAIL AND PIT HOPPER WHEN HE SLIPPED AND HIT HIS RIGHT ARM ON A STEEL BEAM. EMPLOYEE CUT HIS RIGHT ARM WHEN HE FELL. THE CUT REQUIRED STITCHES.

EE WAS EXITING FROM A CATERPILLAR R-50 FORKLIFT, WHEN HE SLIPPED AND FELL ON THE FORKLIFT STEP, STRIKING HIS UPPER RT THIGH.

STEPPED OFF RAIL TRACKS, TWISTED KNEE.

EMPLOYEE IS PARTIALLY HANDICAPPED AND HAS DIFFICULTY NEGOTIATING STAIRS. EMPLOYEE'S FOOT SLIPPED OFF STAIR TREAD AND HE FELL BACKWARDS. EMPLOYEE CUT ELBOW WHEN HE FELL AGAINST LOWER MID RAIL. EMPLOYEE RECEIVED CUT ON LEFT ELBOW AND FOUR SUTURES WERE REQUIRED TO CLOSE CUT.

SHUTTLING FROM SHORE TO A BARGE IN SMALL BOAT. EE STOOD UP AND LOST HIS BALANCE. HE FELL AND STRUCK HIS LEFT KNEE.

THE INJURED EE CLAIMS HE SLIPPED IN SPILLED OIL ON THE FLOOR OF THE LUBRICATING OIL STORAGE BLDG. (LOCATED BETWEEN THE PROCESS BLDG AND THE ROD MILL BLDG.) HE WAS PLACING A FULL 55 GAL DRU M OF OIL IN A HORIZONTAL CRADLE USING AN OVERHEAD CHAIN HOIST.

EE & CO-WORKER WERE CARRYING A PIECE OF STEEL, WEIGHTING APPROX 250# TO THE LOADER BUCKET. CO-WORKER STUMBLED, CAUSING THE STEEL TO SHIFT TOWARD EE. EE FELL BACKWARDS & TWISTD HIS RIGHT KNEE. HE WORKED THE REMAINDER OF THE DAY & 8/4 THRU 8/7. KNEE WAS QUITE PAINFUL & RED. HE WENT TO THE DR, WAS ADVISED TO REMAIN OFF THE LEG FOR APPROX 10 DAYS.

FOOT SLIPPED FROM LAST RUNG OF LADDER OF RAILCAR. TWISTED ANKLE WHEN LANDED ON THE GROUND.

WHILE CLIMBING DOWN FROM A LADDER FROM THE MAIN ACID STORAGE TANK, LEFT FOOT SLIPPED ON BOTTOM LADDER RUNG, PULLING MUSCLES IN LEFT LEG. WEATHER CONDITIONS AT THE TIME OF THE ACCIDENT WAS HEAV Y RAIN. ACCORDING TO HOSPITAL DIAGNOSIS MUSCLES IN LEFT LEG WERE STRAINED.

EE WAS HELPING SPLICE BELT, WHILE BELT WAS BEING PULLED IT LODGED, WHILE TRYING TO DISLODGE IT THE BELT GAVE CAUSING HIM TO LOSE HIS BALANCE. HE FELL THRU A HOLE IN THE HANDRAIL. HE FELL APPRO X 50 FT LANDING IN A SMALL POOL OF WATER, 2 TO 3FT DEEP. HE GOT TO EDGE OF WATER HE WAS CONTAINED THERE UNTIL EMERGENCY ASSISTANCE ARRIVED. HE WAS AIR LIFTED FROM PIT TO HOSPITAL, HE WAS HELD

EMPLOYEE WAS WALKING FROM WELDING TRUCK TO MAINTENANCE SHOP AND TWISTED HIS RIGHT KNEE. HE WENT TO THE DR ON 4/29 AND WAS TOLD TO STAY OFF WORK UNTIL 5-6-96.
