Injured was climbing down the ladder of a 980M, left foot was on the ground, the right foot got caught on the rung, causing the injured to fall backwards into a parked pickup truck. Suffered a shoulder sprain.

Employee was climbing into pick up truck, foot slipped on running boards and employee fell to the ground causing a sprain to the left ankle. Employee was placed into a brace.

Employee was cleaning out the inlet duct in the raw mill. Miner stepped on the incline side of the chute and lost footing on the loose material falling on their left elbow.

Miner was walking 208 Belt line, felt a twinge in right knee.

Employee was using a pry bar to remove a screw conveyor, when the screw became loose it made the pry bar slip and caused the employee to fall back into screw housing.

Affected employee was assisting in the removal of scaffolding from the Pre-heater tower via pallet jack. Whilst loading the scaffolding on to the elevator, another employee slipped on snow/ice. This sudden movement resulted in the shift of material thus trapping the EE's left hand between the material and elevator wall, smashing the fifth digit (pinky finger) on the left hand.

The initial injury happened in 2019, employee was going into a chute at a belt transfer and as EE was climbing in EE's foot slipped off of the step and EE caught self with EE's arms. EE's shoulder hurt but EE did not want to seek medical attention. EE ended up having surgery on 5-18-20 and returned to work 5-19-20

Employee was dismounting a skid steer and as EE placed foot on the bucket and went to stand up EE's ankle twisted and EE went to the ground. EE was dismounting correctly and the bucket was not slick. This was not an MSHA reportable injury when it occurred. Employee had surgery 11/20/19 because ankle felt unstable.

Employee was carrying an approx. 50-lb ladder guard on shoulders across an unpaved lot when employee slipped & hurt back. Employee has been placed on restrictions.

The employee stated they stepped forward off a platform with their right foot onto a 4-in drop onto a lower platform, felt their weight shift, and reported pain in their back & hips on January 11. On January 14, OccMed placed the employee on limitations and prescribed 800mg of ibuprofen.

Slip on 6' ladder 2nd step up using small bar to align bolts. EE did not maintain 3-point contact on ladder.

Employee was changing a sight glass on a drive. When on the east side of the platform on the VSK bearing pedestal, EE hooked their left foot on an RTD Cable going to the outboard bearing, catching the wire with a foot and falling forward, landing on the left knee. Employee was prescribed 3 weeks of physical therapy by Ortho Surgeon on 10/2/2018.

An employee was walking a 5-ft ledge of loose gravel to check the delays on a shot line when the employee lost footing. The employee fell against right forearm, which jarred shoulder and back. The employee requested medical care and cannot complete regular duties. The employee will be reviewing MRI results with a doctor shortly.

Two employees moved the wagon through the narrow pathway with a 500 lb tail pulley. The bottom of the wagon struck a bracket protruding from the floor. While pushing the wagon to free it, the tail-pulley rolled off wagon, Employee 1 fell backwards into the roll press door, and pinned Employee 1's left ankle between the pulley and the removed roll press door.

The employee was grinding on cracks underneath the water truck. The employee was using a 6-ft ladder placed near the back tire to reach the undercarriage of the truck. The employee stepped off the ladder onto the berm area and rolled the left ankle. The employee sought medical attention and was given a rigid boot and restricted duty for one week.

Employee was walking and slipped in mud. In efforts to stop EE's fall, EE grabbed a nearby handrail, and EE's shoulder twisted during the fall. Injury did not become reportable until 10/19/2016 when surgery was performed to repair partial rotator cuff tear

Employee was walking and was tripped by a guard that was being moved by a coworker. Employee landed with elbow underneath body, resulting in injury to shoulder. Injury did not become reportable until 08/04/2016.

Employee was scrapping belt conveyor frame, was leaning on frame when cutting with Ox/Acetyl torch. Frame shifted, and employee was off balance, but never fell. Back strain from becoming off balance. Did not become reportable until 6/22/2016 when restrictions were written.

Employee claims to have stepped off ladder of wheel loader and twisted his knee. Injury did not become reportable until 01/15/2015, when surgery was performed to repair damaged meniscus.

Employee was exiting skid steer and twisted ankle as the foot made contact with slippery material that had accumulated on barrel grabber attachment. Employee suffered a sprained right ankle.

Employee was walking around the machine,slipped and fell.

Employee was descending a fixed ladder, and misjudged the number of rungs to the ground. He stepped down, thinking it would be to the ground, when there was another rung to the ground. His right knee twisted as his left foot went farther than expected. This did not become reportable until 09/27/2012.

Employee was walking to hydropulper control panel when he caught his foot on an anti-fatigue mat and twisted his knee. (Did not become reportable until 10/27/11 when he had surgery.)

While making repairs to dozer, employee slipped on the ice and fell to the ground. (Did not become reportable until he started to miss work due to surgery on 10/06/11.)

While collecting cement samples, employee slipped on ice falling to the ground. It was still snowing at the time of the accident.(Did not become reportable until he started to miss time due to surgery on 3/3/11.)

While using an air lance to remove kiln feed build up in preheater tower, employee tripped over support beam. (Did not become reportable until he started to miss work due to surgery on 2/24/11.)

As employee was responding to fire alarm, he slipped on concrete floor, striking his chin on desk. (Stitches required.)

As employee was removing guard from apron pan conveyor, the guard caught, causing him to lose his balance and take a couple of steps backwards. As he did so, he stepped in a pool of hot kiln feed.

Employee was climbing a stationary vendor ladder attached to equipment. He was grasping for the next rung and his hand slipped and he fell backwards 4'. His back hit against an adjacent railing.

EMPLOYEE FELL OFF SCAFFOLDING.

Employee was climbing onto loader when his foot slipped, causing him to strain arm and shoulder. (Did not become reportable until he started missing work due to surgery on 2-29-08.)

Dismantling elevated boom section about 5 feet in the air, while stepping off of boom section he got foot caught in lanyard. Uneven contact with ground resulted in twisting of right ankle.

EE was setting a 6 foot concrete column form and when the form started to tip over, he attempted to keep it from falling and he stepped off the concrete footing and fell approximately 3 feet to the ground below and struck his left knee and right ankle on rocks and concrete rubble, which resulted in a torn meniscus in his left knee and a sprain to his right ankle.

Employee twisted knee as he was stepping down from raw mill. (Did not become reportable until he started missing work due to surgery on August 2, 2007.)

Employee slipped on ice while carrying sample buckets. Fell backwards straining both shoulders. Did not become reportable until he started missing work on 5/24/07 due to surgery.

Injured slipped on ice on top of cement silos, twisting his knee.

Employee was climbing out from under a haulage truck after repairing leak in hydraulic tank when he twisted his knee. (Did not become reportable until he started missing work due to surgery on December 19, 2005.)

Employee was assisting in removal of kiln chain from clinker cooler when he fell, striking his elbow. Did not seek medical treatment until August 23, 2005 (radial head fracture).

Employee was getting on a Cat D6 tractor dozer to fuel when his foot slipped on the step and he fell forward striking his mouth on the handhold then fell backwards and struck his head on the side of the dozer, then struck the ground.

The employee was stepping down off of a dozer when his right foot hung up on the step between the tracks and caused his left foot to strike the ground with a jolt which caused pain in his lower back. An M.R.I. indicated a herniated disc at L5-S1.

Employee was measuring bin levels on river silos when he slipped on the ice (freezing rain). Did not become reportable until 4/18/05 when he started to miss work due to surgery.

INJ EE WAS WALKING FROM THE YARD OFFICE TO THE MILL BUILDING, WHEN HE TWISTED HIS KNEE DUE TO UNEVEN GROUND. DID NOT BECOME REPORTABLE UNTIL 1/31/05 WHEN HE STARTED TO MISS WORK DUE TO SURGERY.

Employee stumbled, fell and cut finger on leg of work stool.

SALARIED EMPLOYEE WAS SUPERVISING EMPLOYEES WHO WERE CHANGING LINERS AND FEED PIPE IN RAW MILL WHEN HE TWISTED HIS KNEE ON THE UNEVEN SURFACE. (DID NOT BECOME REPORTABLE UNTIL HE HAD SURGERY ON 1-18-05 AND STARTED TO MISS WORK.

EE WAS WALKING BETWEEN KILN AND MILL BUILDING WHEN HE SLIPPED ON ICE, TWISTING HIS KNEE. DID NOT BECOME REPORTABLE UNTIL 4-7-04, WHEN HE STARTED TO MISS WORK DUE TO SURGERY.

EE WAS SITTING IN SEAT OF FRONT END LOADER, HAD REMOVED HIS COAT AND WAS TWISTING TO HANG COAT OVER BACK OF SEAT WHEN HE FELT BURNING PAIN IN HIS LOWER BACK. HAD SLIPPED ON ICE WHILE WALKING T O LOADER, BUT DID NOT FALL. (DID NOT SEEK MEDICAL TREATMENT UNTIL 12/19/03).

INJURED WAS CLIMBING DOWN OFF OF A TANKER TRUCK AND STEPPED ONTO A PIT GRATING WHICH HAD BEEN PUSHED APART.

EE WAS WALKING ACROSS A GRAVLED PARKING LOT WHEN SHE STEPPED IN A HOLE AND TWISTED HER KNEE.

INJURED WAS DESCENDING FROM QUARRY HAUL TRUCK WHEN HE SLIPPED STRAINING HIS BACK. DID NOT SEEKMEDICAL ATTENTION UNTIL 11-19-01, AT WHICH TIME HE WAS DISABLED BYU COMPANY DOCTOR.

INJURED WAS INVOLVED IN A BI-ANNUAL MSHA INSPECTION IN THE BASEMENT OF THE PLANT PUMPHOUSE WHEN HE SLIPPED ON MUD TWISTING HIS KNEE. AS HE ATTEMPTED TO CATCH HIMSELF, HE ALSO STRAINED HIS SHOU LDER. DID NOT BECOME REPORTABLE UNTIL HE STARTEDMISSING WORK DUE TO SURGERY ON 12/21/01. HAS RETURNED TO WORK, BUT IS SCHEDULED FOR KNEE SURGERYON 1/18/02.
