EE drove a stake body truck down to level 12. EE exited the vehicle and then reached back in to turn off a strobe light. While reaching, EE stepped in a 6" hole and tweaked EE's back. Appears to of strained a muscle. Upon clinic visit, EE was deemed unable to currently work. Missed workdays and likely light duty ahead.
EE was on the ground helping load a face for a shot. The EE stepped on a rock and twisted EE's left knee and fell to the ground. Work Care was called and recommended a clinic visit. The clinic did an x-ray and said nothing was broken and referred EE to an orthopedic doctor. EE visited the orthopedic doctor on 7/29 who placed the worker on restricted duty and physical therapy.

EE was descending down the ladder on the scaler. As they did, a foot slipped off of the second (2nd) step. The EE did not fall. They held on to the railing with their left hand and momentum spun their body. This caused their left shoulder to dislocate.
Employee was descending a fixed ladder when fell from the ladder striking the handrail below with their lower back causing injury.

The injured employee stepped up onto the platform to grab the hook, when EE stepped through a hole in the platform. The edge of the expanded metal deck caused a laceration to EE's R/ shin.
While descending a portable ladder, employee missed the last rung and fell backwards. The employee received a fracture to the left foot.

EE was assisting on the repair of a broken water pipe under the secondary tower. After they were finished EE was walking under the tower and got foot caught in hoses causing EE to trip and fall resulting in a fracture in ankle of right leg. EE was taken to the ER.
Employee climbing up machine put fuel nozzle in and began fueling the 988f. EE climbed back down to check machine over it auto shut off engaged when full. EE climbed back up to top fuel off as EE was topping off, EE slipped off step landed directly on feet and felt right knee pop.
Employee was cleaning a spray nozzle on S-17 while standing on the catwalk alongside screen frame. Employee's foot slipped on the wet surface of the catwalk and wedged between the catwalk and screen deck frame.

Employee was turning flat rock stuck in cone crusher and lost his balance when the rock moved and banged his knee on the crusher feed inlet. He later felt pain in his knee and it started to swell. He went to the ER and was X-rayed and diagnosed with a sprain. He was given an immobilizer and crutches to use as instructed and to follow up in 3 to 7 days with a physician.
While employee was climbing the steps on a Caterpillar 777 Haul Truck the employee strained his lower back.
Employee twisted knee exiting conveyor head box. Employee was treated at Riddle Hospital where he was diagnosed with a right knee sprain and discharged with instructions of "No weight bearing on right leg until released".
EE tripped on piece of angle iron sticking out from a pallet of parts by the old scale house after he returned a shove. He fell face down catching himself on his hands and rolled on the ground injuring his right arm. Wrist hurt a little and he shook it off. After about an hour he had pain and could not pick anything up. Took him for X-rays, small compound fracture around elbow.
Employee was exiting the welding truck. As he stepped on the running board the bracket broke. The sudden drop caused him to slip and jerk his lower back. He was treated for a lower back strain and returned to restricted duty.
EE was walking out of the electric house for the plant slipped after taking step to the outside.EE was diag. with a lower back sprain and told to stay out of work for a few days by the treating phys.
Employee was exiting Bobcat skid steer loader. IW foot slipped off of snow covered tire while exiting. Employee twisted his right knee as he fell to the ground. IW was released to return to light duty work.
IE was standing on a conveyor catwalk when the conveyor frame broke. Sudden drop of the conveyor caused the employee to fall. As employee was falling he grabbed the handrail and strained his shoulder. Injury has been classified as first aid to this point and will be changed to lost time effective May 26, 2011 We discovered that the employee will require rotator cuff surgery.
Employee was assisting on blast crew as a driver & laborer. He was attempting to exit the rear of a straight van truck. He was using the rear door and door frame as hand holds & was reaching with foot to use ICC bumper as a step when his foot slipped. He fell to the ground injuring his right knee dan right shoulder.
Employee was coming down the ladder on a 988 Cat loader. He had fueled the machine up and had the nozzle in his hand. He had three points of contact & he thought he was on the bottom step and realized he wsn't. He missed the bottom step and hurt his right knee. He said it didn't really hurt at the time but the next day it was swollen up.

Employee was climbing down from conveyor head pulley chute and slipped off of a shaft for the flop gate. His leg scraped against the shaft as he fell and scraped his leg causing lacerations.
