Employee's steel toe boot was caught between boxcar door and forklift support while exiting railcar causing damage to boot. Exposed steel toe in boot then caused abrasions to fourth and fifth toes.

Climbing up ladder to clean mirror, foot slipped on rung and scraped leg on the edge of equipment.
Employee stepped on a partially covered inspection door and fell back hurting his left knee. Employee incident occurred on 05/31/2012. First aid measures were administered. After further medical evaluation, treating physician put him on restricted duties on 06/28/2012.
Injury arose while employee fell into a jig while holding 25lbs. of steel in his arms. The injury paperwork was not received until 4/17/12.
During the pre-shift inspection of his haul truck the employee slipped from the same level and fell on his back/tailbone.

He raised the ramp a little to release it from the lid. The truck driver thought he was loaded and drove off leaving the employee hanging from the handrail of the ramp. He fell approx 12' 9" to the truck scale.
Employee was cleaning the rail scale at the Dunphy Grinding Plant, employee walked across the manhole cover located in the middle of the scale. The employee was unaware of his surrounding and stepped into an uncovered manway.
Loading barge night tide out EE was ascending from barge to dock. Lost footing & fell hitting knee on dock bumper. EE was not using ramp or ladder provided for ascending/descending climbed up dock bumper.
Contract employee was stepping down off of pickup truck to ground. Improper foot placement resulted in contract employee twisting right knee, injurying it.
Ee had just refueled water pump motor, engaged the unit clutch and was walking around front of pump to close water valve when a piece from the front of the motor flew apart striking him in the left hip knocking him to the ground. Was diagnosed with a fractured hip and an abdomen puncture wound.
RAINY WET CONDITIONS CREATING MUD IN AND AROUND WORK AREA. EMPLOYEE'S BOOTS CARRIED MUD ONTO THE WORK PLATFORM. HE SLIPPED ON THE MUD ONE FOOT SLIPPED OFF THE PLATFORM TO THE GROUND 1 FOOT BEL OW.
EE ALLEGED WHILE WORKING ON THE MILLE SEPARATOR, HE LOST HIS BALANCE WHEN GOING UP THE STAIRS. EE REACHED FOR THE HAND RAIL, MISSED & FELL OVER THE RAIL.
EMPLOYEE ALLEGED THAT WHILE SQUATTING DOWN TO TURN A VALVE, HE HEARD AND FELT A POPPING SOUND WITH PAIN IN RIGHT KNEE. PAIN WAS MINOR ON 8-31 OBUT GOT WORSE WITH TIME AND ACTIVITY.
