Employee's steel toe boot was caught between boxcar door and forklift support while exiting railcar causing damage to boot. Exposed steel toe in boot then caused abrasions to fourth and fifth toes.

Maintenance employee was trying to un-clog a product line beneath Dust Collector/Rotary Valve. Employee accessed 6" cap beneath rotary valve and stuck his hand into line and into rotary valve. Rotary valve was not properly locked/tagged out. Injury resulted in partial amputation of three fingers. Employee was transported/treated at local ER, and released.
Maintenance employee was changing out drive belts on mill motor, while replacing belts employee's finger got pinched between belt and pulley causing minor laceration and partial fingernail dislodgement. Employee was transported to Occ Medical Clinic for treatment.
At approx. 11:00pm on 7/22/10 two employees were operating the facility's TM 4305 TrackMobile. When TrackMobile connected to railcar, employee lost his balance causing him to hit his head on the cab door. Employee sustained minor cut above left eye. First Aid was administered and employee was able to complete remainder of his shift.

The employee used a rounded tip box blade cutting tool to cut a bad section of a hose out. Dust blew from the hose and into the face of the employee causing the employee to cut his hand with the cutting tool.
Mill operator cleaning ledge above rotary feeder with air chisel, when chisel slipped into operating feeder veins. Air tool struck employees left hand, middle finger, pinning it between tool and feeder housing. Injuries are fractures and lacerations to middle finger on the left hand.

Loading barge night tide out EE was ascending from barge to dock. Lost footing & fell hitting knee on dock bumper. EE was not using ramp or ladder provided for ascending/descending climbed up dock bumper.

EE WAS STACKING MATERIAL ON PALLET AND FELT PAIN IN LEG AND BACK.
