EE WAS GETTING DOWN FROM THE SCALE AND SLIPPED ON LOOSE GRAVEL AND TWISTED HER LEFT KNEE.
EE ALLEGES HE WAS CRAWLING OUT FROM UNDER TUNNELBELT AND HE WENT TO STAND UP AND SLIPPED ON WET FLOOR AND WENT DOWN VERY HARD ON RIGHT KNEE.
EE ALLEGES HE STEPPED ON CATWALK PLATFORM AND A PIECE GAVE AWAY AND HE FELL TO THE GROUND LANDING ON HIS FEET. THEN FALLING BACKWARDS FROM PLATFORM APPROX 10" OFF THE GROUND.
EE ALLEGES WHILE WALKING TO HIS TRUCK HIS LEGS AND FEET SLIPPED FROM UNDER HIM, THE GROUND WAS COVERED WITH ICE AND HE FELL ON HIS RIGHT ARM AND SHOULDER.
