A transformer caught fire at the surge pile area. 911 was called as it involved electricity. Fire was contained to the box and no miners were exposed to hazard of fire. There were no injuries. Fire contained and extinguished promptly after the arrival of the local fire department. We do not believe the fire lasted over 30 minutes.

Employee alleges employee was climbing down from a loader and employee's footing got caught on the catwalk and employee twisted right ankle.

Later after seeking medical attention they stated they did hurt their lower back at work. This is late reporting do to just receiving Dr. notes.

Employee was changing out a troughing roller when a speck of rust/metal got into employee's right eye. Employee thought employee got metal out of eye, but eye swelled up the next morning, and sought treatment to remove piece of rust/metal.

Employee alleges EE was climbing a ladder to change screens and later that night EE's left knee started swelling. EE was diagnosed with a sprain and was splint.

Employee alleges they were walking at plant site and stepped on a large rock and fell down landing on right ankle. Employee is now wearing a boot on ankle.

Employee alleges was using a prybar to remove a large rock from the conveyor, As EE was using the prybar, the rock got caught and cut EE's rt. middle finger. Employee did seek medical attention but was released. EE went back for medical treatment again on 10/15/20. A procedure was performed on EE's finger and EE is losing time as of 10/15/20.

Employee alleges was removing a bolt from the cutting edge and the sharp edge of the bolt cut the middle palm of left hand.

Employee alleges was holding back the middle deck rubber and turned on the hose and it slipped out of employee's hands and hit employee in the mouth breaking front tooth in half.

Employee alleges ee was hooking up chains to move a conveyor and ee stood up and heard knee pop. Late reporting employee just notified employer on 11/10/17. Employee went to Dr. on 10/23/17 and is now loosing time.

Employee alleges was hooking a chain to a loader bucket and as EE turned to walk away left foot slipped into some mud, wet clay and rocks hurting left knee. Employee just seek medical attention on 4-19-17 and has some restrictions. We can accommodate the restrictions. This employee isn't losing anytime.

Employee alleges employee was tightening screen bolts and employee turned to walk up some steps and employee slipped on the steps and fell down hurting employee's lower back. Employee starting loosing time on 02/15/17.

Our tech arrived to location where tires are stored. Grabbed the tire needed an as he was moving the tire to the bed of the service truck (using the boom) a hydraulic fitting cracked causing a leak of hydraulic fluid causing the tire to drop form the boom hands. Out technician recognized the danger and moved but, the tire bounced hitting our tech in the leg.

EE alleges he was removing bolts from the wash screen with a driver guard and he turned to switch positions and he twisted his right knee.

We were just notified on 11-08-11 that he was going to be receiving his first epidural steroid injection. Depending on how he feels he could receive more. The employee is not loosing any time.
