Employee was walking off the face when EE stepped in a hole between Shield #2 and #3 causing EE to fall spraining EE's ankle.
Employee was walking to the face on the longwall, as he came around the end of the stage loader he stepped to the side to avoid a hose and slipped and fell catching himself with his left arm causing a dislocation of his left shoulder requiring surgery.

Employee was washing at the bottom of the slope - standing on loose coal. The loose coal gave way and employee lost his footing falling to the footwall. The water hose he was using apparently whipped around striking him in the face/eye area. He was taken off work with the first date of loss being 5/15/2015.
Employee was at the roofbolter when he lost his balance, slipped down under the operator side of the drill mast and accidently grabbed the controls causing the foot on roofbolter to come down catching the tip end of his boot causing fracture and sutures to toes on right foot.
Employee started work on 5/10/15 at 11:00 PM. Employee was walking down slope when he slipped and fell injuring his left foot. Employee stated that he looked to see what caused the accident and he had stepped in a hole and it was not visible due to water. Doctor took him off work since he had a fracture.
Employee was stepping off a ladder from the loader and felt sharp pain in the back of his knee, top of calf muscle. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT SEEK MEDICAL TREATMENT UNTIL 7/27/15. EMPLOYEE DID NOT START LOSING TIME UNTIL 8/1/15 DUE TO SURGERY.
Employee was walking when he stepped in a hole and his foot slipped back causing pain in his left calf muscle.
Employee was on the roofbolter, after he handed a new bolt and plate to an employee to install, he went to step down off of roofbolter twisting his left ankle causing sprain to ankle.

Employee was walking and directing the Lo-Trac operator that was loading a car when he stumbled and fell over a rock scraping his right leg. EMPLOYEE HAS BEEN WORKING SINCE DATE OF THE ACCIDENT AND EMPLOYEE DID NOT WISH TO SEEK MEDICAL TREATMENT UNTIL 12/18/14, THAT'S WHEN THE DOCTOR TOOK HIM OFF WORK DUE TO INFECTION TO HIS LEG.
REPORTABLE ONLY DUE TO SUTURES - Employee was helping unload cribs and stacking them on pontoon of the shield when he lost his balance and fell forward catching his left finger between cribs causing laceration to finger requiring sutures.
Employee was filling rockdust pods, he stopped and bleed off air line to disconnect and rockdust came out and he turned to run from the rockdust and he fell hitting his right thumb causing contusion. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS FOUND TO HAVE TORN LIGAMENT IN THUMB AND WAS TAKEN OFF WORK. EMPLOYEE DID NOT START LOOSING TIME UNTIL 1/29/15.
Employee was stepping off the manbus when his foot caught under track crib causing him to twist his knee. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 3/12/15 DUE TO SURGERY ON HIS KNEE.
Employee was carrying a boat winch from outby to the face when he stepped on the pontoon and his foot slipped off the pontoon into a hole causing him to fall onto his left leg causing a strain and possible tear to calf muscle on the left leg.
