EE stated that he was walking back toward the miner after conducting an air reading when he slipped on a rock and fell into the gear case of the miner. He was treated at MAR Hospital for a laceration to the forehead requiring 7 stitches. EE returned to work on the next shift at full duty.

Employee stated that he was walking down the #2 entry,when he fell onto his back. He was transported to the hospital with complaints of numbness in his left arm. He was examined with no fractures or abnormality. He returned to work on light duty 11-10-10 and was on call for Dr. apt. to give full release. He missed work on 11-09-10 while at hospital.
EE was working constructing cribs on top of a canopy. When he jumped down to get off the canopy he thought he had turned his ankle however when he was xrayed at the ER it showed to be fractured.
EE was conducting a pre-shift inspection when he slipped as he was walking back to the rail runner. He fell striking his ribs on the rail runner resulting in one fractured rib. EE was examined at the ER and returned to work on that day.
While setting the tail piece, EE slipped and twisted his right knee. He continued to work until May 4 when he had surgery.
Employee stated that he had filled the dozer with fuel and was standing on the track of the dozer when his foot slipped and he fell off of the dozer landing on his back on the ground. He went to the emergency room and x-rays did not show any broken bones and the diagnosis was bruising only.
EE stated that he was working on a belt set up, when he stepped on a piece of belt that was laying on the bottom as it was pulled by the scoop. EE saw a Dr. on that day and continued to work until 11/16/07, when this became a lost time injury.
Employee was standing on a step ladder, attempting to hang a cable from the roof when he fell off the ladder striking the ladder as he fell. He was transported to MARH where it was determined that he had fractured 3 ribs. He returned to work on the next regular scheduled shift on light duty without loss of time.
Employee was welding cyclone feed pipe when he fell three feet striking his upper left arm on an overflow box. He suffered a fractured upper arm.
As employee was walking along the track, his boot got caught on a cross tie causing him to fall, breaking his left ankle.
EE WAS STANDING ON A BELT RAIL DRAINING A WATERLINE WHEN HE SLIPPED AND FELL STRIKING HIS HEAD ON A CHUTE. HE SUFFERED A 1 1/2" CUT ON HIS HEAD. HE WAS GIVEN C-SCAN AND XRAYS AT HOSPITAL WHICH WERE NEGATIVE. RECEIVED STITCHES FOPR CUT. HAS POSSIBLE CONCUSSION.
EE WAS KNEELING DOWN PUTTING TOGETHER WATER LINE. WHEN HE STOOD UP HIS RIGHT KNEE POPPED AND BEGAN TO SWELL UP.
MINER CABLE WAS LAYING ON HILL. EE STEPPED ON CABLE CAUSING HIM TO SLIP AND FALL.
GOING TO GET LIL MAC OFF CHARGE, HIT ROOF BOLT PLATE AND FELL HITTING RIBS ON SIDE OF LIL MAC.
