Employee was loading supplies into EE's scoop. The employee's foot got caught and EE fell. As EE was falling, EE reached EE's arm out and fell onto a pallet. A nail from the pallet poked EE in the right forearm requiring one stich to close the wound.

The IE slipped while entering the cab of the D5 dozer and struck EE's forehead on the door frame resulting in sutures.

Employee was walking outby on the 2nd South Belt line when they tripped and fell landing on left shoulder. Employee continued to work until they went off for surgery on 03/02/2026.

I.E. was roofbolting at the face. As they turned to get some supplies they stepped in a hole and twisted their left knee. Doctors restrictions took them off work on 5-22-2025.

I.E. was unloading 8" aluminum pipe on a belt move. They caught their right foot and fell down. When falling the pipe dropped and hit the inside of their right leg just above the knee. Doctor's restrictions took them off work on 5-20-2025.

I.E. was putting a 6' bolt and glue into a hole. As they were pushing the bolt into the hole, they slipped causing them to lose grip of the bolt. The bolt fell from the roof hit the wrench and struck the I.E. above the eye. The laceration required stitches and the I.E. did not miss any work.

I.E. was cleaning mud, snow, and ice off of highwall miner beams. They stepped wrong and lost their balance falling down. When they fell they felt a sharp pain in their right foot. Doctors restrictions took to the I.E. off work.

I.E. was carrying a 5 gallon bucket to get a coal sample off a beltline at the wash plant. While walking they slipped on some ice and fell to the ground landing on the bucket. This caused a fracture in their right arm.

I.E. was walking around roof bolter to get screenwire ready for advancement. As they were walking they felt a pop in their right knee. I.E. was unable to come back to work due to the injury on 5-27-2024.

I.E. was loading belt rail frames on a sled. They tripped and fell. As they caught themselves, they felt a burning sensation and lump in their lower abdomen. I.E. continued to work full duty until surgery on 2-8-2024.

I.E. was getting out of the passenger side of pickup truck at warehouse. As they were getting out of the truck, their foot caught a CB cord. They fell out of the cab on their right shoulder. I.E. continued to work until doctor took them off for surgery on the shoulder on 10-19-2023.

OTR truck driver was walking around their truck checking lights. While walking the driver stepped in a hole causing them to trip and fall. When falling I.E. fractured two fingers on their right hand when hitting the ground.

I.E. was carrying a box of glue to their roofbolter tray. As they were walking they slipped on a rock and landed on their left arm catching themselves with their hand. I.E. has continued to work until doctor's restrictions put them on restricted duty. I.E. is still working. I.E. went on restricted duty on 5-9-2022. I.E. began missing work for the injury on 8-3-2022.

I.E. was moving a roof bolter from the side controls. As they were walking with the machine they stepped off the end of a rock and felt a strain in the back of their right ankle. Two days later while preparing to go underground the ankle popped and caused them discomfort. Doctors restrictions took them off work on 1-7-2022.

I.E. was getting curtain drops from the tool sled crosscut. While walking back out into the travelway, they tripped over a piece of metal stuck in the mine floor. When they fell they struck their elbow on the mine floor causing a hairline fracture in their elbow. I.E. is working on restricted duty.

I.E. was standing on the side of the roof bolter trying to move a crooked bundle of screenwire on the bolter. They took a step back and tripped over a bolt divider on the bolter and fell to the ground landing on their neck and upper back. Specialist deemed the I.E. unable to work on 5-4-2021.

IE was pulling cable off of the roofbolter reel for an inspection of the roof bolter. IE pulled the cable through a set of curtains and tripped over a loop of cable that had been previously pulled off. As IE tripped another miner pulled more cable off and caused the IE to fall and contact their right shoulder on the ground. Restrictions have taken the IE off work on 3-4-2021.

Walking along examiners route in the return. I.E. stepped in a rut along the route and tripped and fell to the ground. When tripped I.E. heard a pop in their right knee along with pain. I.E. continued to work until doctor restrictions took them off work on 12-28-2020.

I.E was brooming dust along the 2N #2 beltline. While doing so I.E. got close to the ledge of an underpass cut underneath the beltline. I.E. fell from the ledge (approx 4') causing an injury to head. Doctor restrictions have the employee off work starting 03/23/2020.

While working on grizzly bars on raw coal pad, an opening was covered in coal and employees foot fell through causing EE to fall and injure EE's right shoulder. Employee continued to work until surgery on 11/14/2019.

Employee was placing buckets in diesel scoop bucket when EE slipped and fell causing left elbow to come in contact with the bucket stinger. Employee began missing work on 8-5-2019.

After starting DOZ 128, EE stepped back out of the cab and missed the step. EE fell to the push arm and strained right knee. EE continued working until EE had surgery on 12-20-18.

Cleaning up spill on backside of unit tailpiece. While preparing to cross over belt slipped and fell, straining left knee. Employee continued to work until surgery on 05-09-2019.

I.E. was bolting when a piece of shale fell from the roof. The I.E. saw it coming and twisted to get away from the rock. While twisting to get away they fell into the side of the bolter and felt a pop in their lower back. Comp deemed this injury as pre-existing, reporting due to lack of evidence to support.

Employee slipped and strained right shoulder while climbing down off of the screen. Employee continued to work. Employee started missing work on 9-19-2018 after shoulder surgery.

I.E. was attempting to climb out of a scoop cab to start loading supplies. While exiting the scoop they tripped on the bottom lip of the cab and trip, falling head first into a coal rib. I.E. received pain injections for treatment.

Contractor was moving pipe and fell, causing the pipe to land on left ring finger. EE finger required sutures.

Contractor was cleaning the area around the slope/Stacker MCC room. EE fell down the embankment on the north side of the MCC Room, receiving bruises/strains to left arm, shoulder, and leg.

Employee was getting off of the dozer and slipped, striking employee's left shin on the push arm of the dozer. Employee continued working until 5-31-2017.

Employee was helping wet dust when EE fell backwards and struck back on the wet duster. EE fractured 2 ribs.

While stocking parts, the employee tripped on a pallet and twisted left ankle.

Twisted lower back while stepping out of a mud hole. Continued working regular duty till 7/21/16 when restricted duty began.

Employee stepped in hole and twisted left knee. EE continued working until ee had surgery on August 25, 2016. EE returned to work on August 29, 2016 on restricted duty.

Employee slipped and fell, hitting his head on truck bumper.

While moving a file cabinet in office, he fell causing a hairline fracture to his right ankle.

Employee was pulling a piece of line curtain when ee tripped. EE twisted left knee. EE worked until he had surgery on 6/4/15.

Employee was stepping off a bus to get on a man-trip and rolled his ankle.

Employee slipped and fell, hitting his face on a pipe. He continued to work until 1-20-2015. He returned to work on 1-23-2015.

While waking to the scoop employee stepped in a hole twisting his ankle.

Employee was walking in the #5 portal. He slipped on ice and fell on his left shoulder. He started missing work on 4-21-14.

Employee was walking to the face, to reset a pump. He stepped on a pile of gob sinking to his knees. He fell sideways, twisting his left knee.

Hanging phone cable, when he stepped in a hole causing him to fall. He caught himself with his right arm, and felt pain in his shoulder. He continued to work until 8/27/13.

Employee was closing a door on the no. 1 dryer in the preparation plant. He fell backward and caught himself, causing injury to his left shoulder. The first day of work missed was 6/6/2013.

Employee was hanging line curtain, when a rock 26" x 12" x 4" fell from the roof striking him in the head and shoulder and knocking him to the ground. When he fell he landed on his right elbow. Employee continued to work until he had surgery on his elbow on 5/14/13.

Employee was helping the miner man back out of a cut. He stepped in a hole twisting his right ankle.

Employee was doing permissibility on a pump. While wading through the mud he twisted his knee. This became reportable on 7/26/12. He is going for therapy.

Employee was getting out of battery scoop #397. he had his left foot on the ground, as he was pulling his right foot out of the scoop it got caught on the seat in the cap, causing him to fall. he caught himself with his hands. This caused pain in left shoulder.

While moving a new miner to the section the employee and another mechanic was pulling up the cable slack when his foot got entangled in the loop of the cable causing him to fall.

While loading belt material into a coal hauler the miner stepped out of the bake of the car (aprox. 6" step) and he rolled his left ankle.

Employee was walking to retrieve tools from tool sled when he stepped in a hole and rolled his left ankle.
