Mostrando 50 de 510Employee was loading supplies into EE's scoop. The employee's foot got caught and EE fell. As EE was falling, EE reached EE's arm out and fell onto a pallet. A nail from the pallet poked EE in the right forearm requiring one stich to close the wound.
Employee was roof bolting and dropped wrench on EE's left pinky toe. This resulted in a fracture of left pinky toe.
Employee was installing the center pin while roof bolting and a rock fell striking the back of the employee's left leg below the knee. The injury resulted in a fracture of the employee's left leg. The employee was taken off work the following day (4-23-2026) due to doctor's restrictions.
Employee was hooking up a charging nip to a coal hauler battery. While tightening the nip the employee received burns on EE's left hand and left ring finger. Some of the burns are possible 2nd degree burns. The employee works for Jennmar services and is under Knight Hawk Coal, Prairie Eagle Underground Mine's supervision.
Upon discovery MSHA hotline was called immediately. The current E01 inspector was called and personnel were dispatched to operate machinery to begin cutting out heated material to help extinguish.
The IE slipped while entering the cab of the D5 dozer and struck EE's forehead on the door frame resulting in sutures.
Employee was bolting when a piece of rock fell and struck the left side of the employee's face as well as the bridge of their nose. This resulted in a laceration on the employee's left side of their face requiring 6 stiches. Employee has not missed any work due to the injury.

I.E was welding a new steering weldment on coal hauler 142. Some slag fell into EE's glove causing the employee to burn EE's left forearm. 4 days later on 2/17/2026 EE began to have redness and swelling in the area. EE was taken to the doctor and prescribed medicine to treat infection in the arm.
I.E. was cleaning around the wet duster slurry pump and EE's glove was caught. This resulted in a avulsion of the left index fingernail, nail bed, and also broke the bone in the end of this left index finger.
Employee was watching coal dump onto the slope belt. They turned to walk to the dump end of the magnet landing. When they did, they felt their left knee pop.
Employee was installing a roof bolt. When they fully extended their arm and pushed the bolt up, they felt a pop and a burning sensation in their left shoulder.

Employee was pulling up miner cable for a pull back and was moving cable from the middle of the entry to the rib. They then felt a sharp pain shoot down left calf muscle. Doctor's restrictions took them off work on 11/13/2025.
For the past few months, the employee has been having trouble with both hands going numb and loosing EE's grip on EE's tools. EE states that throughout the day EE's hands go to sleep and EE experiences pain. EE was taken off work for surgery on 3/11/2026.

Employee was attempting to change a filter housing on a diesel scoop. EE was using a pipe wrench when EE felt a pop in EE's right shoulder. The employee continued to work until EE was taken off for surgery on 5-5-2026.
EE was hitching the rib to set timbers for the unit tail piece. EE's right hand, right elbow, and left hand were going numb as EE was swinging a dutch head. EE later began to train the belt and was hitting rollers and experienced the same sensations. Employee said EE has had issues with this condition for the past 12 months. EE worked everyday up until 4/29/26, off for surgery.
After an employee hooked a come-along up to a drag, EE stood up and felt a sharp pain in EE's left hip. The employee was taken off work by a doctor's restrictions on 10/06/2025.
Employee was attempting to dump a loaded haul truck. EE raised the bed, the tailgate was not opened, EE then moved the truck while turning left (downhill direction) and rolled the truck onto it's right side. EE sustained 2 stiches in the right side of EE's head and broke EE's right collarbone.
Over the past few months, employee has had a lot of pain in elbows and loss of grip in hands along with numbness in both hands. Employee was taken off work for surgery on 2/25/2026.
Employee was walking towards EE's work partner. EE's partner was tearing down a curtain just outby the unit tail piece. As the employee turned the corner something struck EE in the right eye.

Employee was lancing a shear jack pin when the rod shot out causing burns to their left thumb, index finger, middle fingers and palm of their left hand.
Employee was putting up a 6ft bolt when it caught on the hog panel. They went to push up harder on the bolt and felt a pop in their right shoulder. The employee continued to work full duty until they went off for surgery on 12/9/2025.

Employee was changing a BO top roller. As EE was working a come-along to raise the belt, EE felt a sharp pain in EE's right shoulder. The employee continued to work until 04-23-2026 when EE went off for surgery.
I.E. was changing a tire on a coal hauler. While loosening the lugs with a ratchet and pipe, the head of the ratchet slipped causing the I.E. to smash their right ring finger. The finger was between the rim of the tire and the pipe. I.E. received a fracture in the end of the ring finger.
I.E. was handling cables during a belt move. They started feeling tingling in both hands along with numbness and elbow pain. Doctor took the I.E. off work on 7-30-2025.
I.E. and others were attempting to pull a stuck roof bolter out with a battery scoop. As they were pulling the bolter out, the piece of the scoop the strap was connected to broke. When it broke it contacted the I.E. across both knees. This caused contusions and ligament issues. I.E. was taken off by a doctor.
I.E. was roofbolting at the face. As they turned to get some supplies they stepped in a hole and twisted their left knee. Doctors restrictions took them off work on 5-22-2025.
I.E. was cutting coal at the face with a continuous miner. While operating the miner, a piece of material came back from the head and struck the I.E. in the right arm. The piece of material had to be removed by a doctor and the wound received 1 stitch.
I.E. was unloading 8" aluminum pipe on a belt move. They caught their right foot and fell down. When falling the pipe dropped and hit the inside of their right leg just above the knee. Doctor's restrictions took them off work on 5-20-2025.
A hot spot was discovered on the PEEP raw coal pad. No one was injured. When a dozer was freed from being stuck material started being moved from the area.
Dozer was pushing material into a feeder, when the blade of the dozer dipped forward, causing the dozer to get stuck in the hole of the feeder. No one was injured or trapped.
I.E. was driving empty coal hauler to the miner to get a load of coal. As they were driving they ran over a rock in the haul route. This caused them to be jarred in the seat. They bit down on their teeth causing the left front tooth to crack in half.
I.E. was hooking up cable strap to the tail of the CM. While lifting the cable and the strap, they felt a pain in their right shoulder. Doctors restrictions took them off work on 4-3-2025.
I.E. and another employee were attempting to chain up a drive shaft on a haul truck to be able to pull the down equipment out of the way. While holding the driveshaft up, the weight shifted and the driveshaft fell on the I.E.'s left hand bending back their wrist causing a fracture.
