The generator overheated so the employee released the pressure then as EE took off the radiator cap the antifreeze spewed up causing burns to EE's face, neck, right shoulder and right arm.

Mostrando los 8The employee was unloading a gas cylinder from the back of a truck. The employee knew that they were supposed to report an accident/injury, but failed to do so until 1/23/2020. The employee went to a Hospital in Hazard, KY complaining of back pain and was then transferred to another Hospital in Lexington, KY.
The employee stepped off of the miner and turned around striking EE's elbow on the miner machine. The employee later received medical attention at the Pikeville Medical Center on 7/11/2019. EE was diagnosed with a fractured bone.
The employee shut down the generator when the high coolant temperature light came on, EE then removed the radiator cap while the generator was still hot. The hot antifreeze come out onto the employee burning EE's hands, arms, chest, abdomen, side and underarm.

Employee was installing a shear cylinder in cutter head and the chisel dislodged while spreading yoke on shear cylinder striking employee is the left leg.
Employee bent over to pick up a crib block to move out of the way. Shortly after throwing block out of the way, the employee started complaining with neck pain.
He accidently fell off belt because CB mike fell and hit mouse causing belt to turn on. It knocked him off causing him to fall about 12 feet to the ground. He landed on his side. He refused medical treatment.
