Employee was hand-salting an icy area on the haul road. EE was spreading calcium chloride when EE slipped on the ice and fell catching self with EE's left hand. After an x-ray at the local emergency room it was determined to be a fractured left wrist. Also, the next morning EE's left knee swelled. Received another x-ray, no fractures.

Mechanic was installing access door on the BMR when trolley beam moved striking mechanic on the right side of head.
Employee was using brake cleaner to clean a fly wheel housing to install a bearing. Employee began to heat the clean part with a torch and the cleaner fumes in the housing caused an explosion. Employee was taken to the Emergency Room and treated for First Degree Burns on arms, neck and face.
Employee was exiting a CAT 740 Articulated Truck when he missed the bottom step and fell on his left knee.
A mechanic was working on a waste coal fuel reclaimer chain and flights. the work involved using a 12lb sledge hammer to pound in flight pins. The repetitive motion of the day contributed to an elbow injury. The mechanic reported elbow pain to the plant EHS Manager.
The fuel handling Tabor Screen reject chute contains a door for access and cleaning. While removing the chute door, the door slid down toward the floor. The operator grabbed the door with his left hand and felt a pain in his shoulder area. Medical treatment was provided which included a prescription and scheduled physical therapy.
Employee was changing a fuel filter on a pump. Employee grabbed the fuel filter with his right hand and the fuel line with his left hand to remove the fuel filter. While twisting and pulling the fuel filter the fuel line released suddenly and his left hand hit the frame of the pump. Employee's left third finger was cut on the outside of the first knuckle and required five stitches.
EE was assigned to calibrate the B Ramsey belt scale load cell. The load cell includes a pivot arm which fluctuates up and down. As the arm is displaced, a signal is transmitted to a meter where the weight of the waste coal is determined. The arm is manually lifted to relieve any tension. EE improperly lifted the pivot arm while using only his right arm and improper body position.
EE was cleaning ash underneath the belt.EE attempted to use a fox tail brush,the belt idler caught his glove pulling his hand into a pinch point.EE pulled his hand out,had received severe compression trauma.EE called control room 911 called,EE's crew responded and first aid given to stop the bleeding.At 5:24 a.m.the ambulance arrived,EE was transported to E.R.See Attached
Employee was walking to water treatment pond after it rained when he slipped and fell in the mud, twisting his ankle. This caused his left ankle to be dislocated and left leg to break.
Employee slipped and twisted back while going over berm to start and fuel pump. Heavy rains overnight.
A mechanic was a member of a team of mechanics installing a drive chain on a waste coal crushing machine. The chain was suspended using a choker cable and chain fall. As the mechanic adjusted the chain fall, the load/weight of the drive chain shifted resulting in the choker cable pinching and trapping the mechanic's left 4th finger in the chain fall hook.
EE was working on a BMR(crusher) liner bolt replacement. Crew was placing the top lid on the BMR. 2 cranes/hoists are used to move the lid. The first positions the lid approx 3' short of its final placemen. The lid was placed on top of the BMR while the second chain hoist was rigged. While resting on the machine, the lid teetered and fell approx 18" on a mechanics foot.
By using a hoist, the EE was placing 500lb rods into a coal crusher machine. He placed his left hand on the trolley beam between the trolley and the beam stop. The trolley pinched & lacerated his left little finger.
On 1/25/06 it was reported to us by Cougar Trucking that one of their drivers had fallen off of his truck at our Ernest site on 1/16/06. The employee was fixing his tarp outside of his truck & slipped and fell to the ground (bruised groin/tailbone). He continued to work the rest of the day & did not report this incident to anyone at the site.
