EE was operating rubber tire Mack 8 personnel carrier when the front wheel hit a hole in mine floor causing the steering wheel to spin catching his right pointer finger, causing a dislocation. Cause of this injury was awareness. Compliance with rules/regs not a factor, mining equip/systems not a factor, miner proficiency a factor.

An un-intentional roof fall occurred in the intersection of number 6 entry on the working section mmu-001, one break inby the section loading point. The roof fall measured approximately 30'feet in length x 18'feet wide x 5' to 5 1/2' feet high.

A roof fall occured in the active working section, mmu number 001-0. The fall occured one break inby survey spad 6+96 in the intersection of number 6 entry and into the crosscut from 6 entry to 5 entry.

The accident was a roof fall in the #3 Entry of the active working section MMU-001. The 12-12 Remote Continious Miner was being operated at the time of the roof fall. The miner was taking his second cut out of X-cut 3 to 4.
