When climbing back on dozer after filling up with hydraulic oil, operator slipped on the tracks and fell onto the dozer/ground. Conditions were wet and muddy. Leg and ankle were hurt during the fall. Was treated and sent to hospital for X-rays.
Was prying metal with pry bar, when it slipped and jarred his body.
Walking on uneven ground and twisted his ankle which resulted in a fracture.

Employee driving company truck, patrolling property, wiring in truck caught fire. As employee tried to put out fire, truck went into ditch. His left vehicle and started walking down hill, he fell on pavement and hit his head. He was found in roadway to approx. 5:30 am 2/25/10. Transported to hospital by ambulance.

D11 Operator was pushing shot and backed over a rock. The Tractor broke center and sat down backwards with a hard jolt jarring operators back & neck.
Hanging ventilation curtain when 30" X 12" x 1" rock fell from top striking ee in right wrist.

Cold day, ground was frozen. EE was packing oil to dozer when he tripped on some frozen dirt falling in a low dip area injuring wrist. At the time EE refused medical attention. Swelling would not subside after a couple of days so he sought medical treatment for his wrist.
Employee finished refueling equipment spread & was rolling up the fuel hose in the rear of grease truck. A wind gust caught the rear door of the truck slamming it into employee's head knocking him down. The door was fastened at the time of accident/injury, per accident witnes. Weather was very windy, with gusts ranging upwards up to 40-50+ MPH.
EE was mounting the shuttle car anchor when the shuttle car operator turned the corner the EE was on wrong side and buggy cable pulled against the rib hitting the EE's knee area.
Dozer operator wasa pushing shot when dozer slide sideways jerking the operator causing him to strain his neck and upper back muscles.
Ignition of gases while hooking up cutting torches. Employee was taken to Plateau Medical Center in Oak Hill. Employee complained of pain in Abdomen and left ear.

Employee was trying to get a fan hub off of a fan motor with a hammer and punch. When he was striking punch with hammer a piece of hub came off and struck him in the hand.

Employee was climbing off service truck at side entrance when he lost his hold and fell approx. 5 feet to the ground landing on his rear side-lower back. Was helped up but was able to walk uner his own power.
Emp. was dismounting D9T Doz. when his Rt. foot slipped off a step & fell back on the track. Step was wet due to early rain fall. He bruised his Rt. hip.
Lifting belt structure and experienced back strain.
Operator was backing up when Dozer hit a rock jarring his lower back.

Employee drilled hole in top and steel lodged in hole. As he let down drill pot steel fell, from the hole landing on his left index finger smasheing it against drill chuck.
Employee was returning from break. He was remounting the Dozer when his back gave out which caused him to fall back twisting his left leg and left arm.
Coal truck driver was coming out of pit where there was a rut, driver straddled rut and overturned. Driver said he was dizzy got emt to him they looked at him ,gave him oxygen, called 911 for ambulance.
Employee was fueling up the coal truck he was driving and all employees are told not to fuel up truck after dark. He lit his cigarette lighter to see if the tank was full and he got burnt. They are always told in safety meetings not to smoke or use anything that could cause a fire.
Rock Truck Operator was climbing up and onto his rock truck when he lost his grip and fell approx. 12 feet to the ground landing on his left leg and ankle. Weather conditions were rainy, causing the ground to be muddy.
Left rear step had been welded on the right side, when operator steeped on ladder the weld broke on right side causing the left shoulder to be pulled more than normal
Hanging miner cable to the #2 miner at the #1 crosscut in front of power center when a piece of rib came out between the timber and the mine roof, catching injury on the left knee pinning him to the bottom
Two EEs were injured in an accident involving a pickup truck & a highwall drill. The truck traveled down the drill bench slope & struck the drill. The two EEs were apparently outside the drill, both on foot at the time of the accident & were struck by the truck. The exact location of these 2 EEs have not been determined at this time & the accident is stil under investigaton.

Spot bolting between spad #1425-1426. In 3 Lt one break inby feeder rib started to roll out causing co-worker to move to avoid rib hitting inch trap causing bolter to move and run over employee's left foot.
Drill operator stated that he stepped in a crack on the bench at night and injured his knee.
Fell down steps leading down from scale house, cut under arm and hurt shoulder.
Employee was taking last clamp off hose on a D11 Dozer when the metal end of the hose popped off and struck the mechanic in the forehead
Bolt crew was pulling slack when wire holding bolter cable on roof bolt plates began to break resulting cable hitting employee in back of neck.
Putting wet brakes on 488 Scoop on 6/3/09, stained back (lower) and came in to work on 6/4/09, loaded shear hub for feeder with help and felt burning in neck and sharp pain in low back.
Mechanic was cutting back rubber on 777 transmission harness to put in new plug. He was using a utility knife when it cut through the rubber and sliced into his left pinky finger.
Operator was pushing shot rock out of shot with D11 dozer. As he was backing up a rock rolled in behind him and he didn't see it. When he backed up on the rock the dozer slide off and jarred him and the machine and hurt his back.
After excessive rainfall and flooding the # 3 belt entry of the mine was blocked & canopy was crushed when materials fell from the highwall. Also, fan was hit but no damage to it except alarm & chart system.
Installing lifting eyes in a D11 converter to set in hole. One of the lifting eyes broke and the other eye was unable to hold the weight and torque fell on left arm causing laceration.

Operator started feeling slight pain after Dozer jarred him while pushing shot on April 21. Towards the end of his shift, he was experiencing moderate-severe pain in lower back. Went to Doctor on April 23 when pain would not ease up.

Dozer was tramming out of shot. Got to the bottom of shot and dropped over a ledge approx 4 feet in height. Operator had his seat belt on. Said it twisted him.
Employee was changing a 992G Cat wheel loader tooth. When picking up the loader tooth he strained/pulled a muscle in his lower left stomach area.
