One employee was attempting to dislodge a rock in the top part of a cone crusher. the employee failed to lockout the feeder feeding the crusher. when the plant operator attempted to start the plant the feeder started spilling rock. The other employees noticed what happened and had the feeder turned off. A rock however did fall and strike the employee in the shoulder.

Operator was running an open cab stand on skid steer and was not paying attention to surroundings and hit their hard hat on a guard of a conveyor. The operator went to the emergency room and was cleared by doctor to return on 3/2/26 missing one day of work.
Conditions were dry and no adverse conditions were noted. EE was ascending the ladder on HT-29 (777g CAT HAUL TRUCK) in order to enter the cab. EE felt a pain in EE's rt knee while climbing. EE notified pit foreman via text.
Employee was moving a mechanical lever to change products and felt a strain in the gut area. No action was taken at this time as felt it was a strain from exertion. Currently employee is working at full capacity with some discomfort.
As Employee was stepping up onto a 8 inch platform EE pulled a muscle on EE's right calf
Employee was assisting in changing screens stepped wrong and knee popped. Was taken to urgent care, treated as sprain, Ice with no restrictions was prognosis.
EMPLOYEE WAS WALKING UNDER CONVEYOR MATERIAL FELL OFF STRIKING EMPLOYEE IN THE HEAD AND SHOULDER
Clear dry day, employee went to exit the loader and missed the first step coming down. EE fell back landing on butt, back, & hitting head. EE didn't lose conciousness, got up and walked to where others were at. Went to an urgent care for eval and was sent to ER for CT. Only severe bruising was noted. Light Duty for three days.
While climbing down from a screen the employee slipped on some loose material and started to fall catching self. In catching self felt a pop in neck and shoulder area. No medical treatment was sought at this time.
Work was being done on a conveyor approx 8ft above ground. The work done was overhead for several hours. While replacing a roller the employee felt a pop in EE's right shoulder while prying roller in place. EE immediately reported it at the time. EE let me know 2 wks later that there was still discomfort and received medical evaluation. results: limited use till see specialist.
While removing an impact bar from the crusher and placing it on a pallet, the bar shifted and rolled onto EE's left metatarsal. This bruised at point of impact. Site tenderness was noted but motion was good. We treated as a bruise injury and EE finished the workweek. It was observed Monday to still be tender and EE went to see a doctor Tuesday. Xray revealed a bone fracture.
Employee was in the process of going to shut off impactor at ground level. When they heard a pop in their left leg which caused pain and loss of motion in it. Employee was unable to put any body weight on leg after this. Employee was taken to hospital by management.
Miner was walking across GP7 catwalk in one direction, but then turned abruptly into another. The quick pivot caused injury to right ankle
While removing a flat rock that was wedged in rock box above jaw belt. Employee was swinging down on rock with a sledge hammer when they heard a pop from lower back. Which in turn caused them extreme pain and discomfort when trying to move. Employee was transferred by EMS to hospital.
Miner climbed a platform to grease feeder bearings. Area is inaccessible. Miner was not wearing a safety harness. Miner finished greasing bearings, miner stepped back and fell through an opening in the handrail of the platform. Miner fell approximately 15 feet onto a pile of rock. County Emergency personnel called to treat Miner.
The Loader loading the haul truck lowered the boom and suddenly hitting the bed side causing the truck to suddenly shake.
Employee was preparing to wash spillage from under a conveyor. A new nozzle that was being used was 1.5" thread and so was the mating threads. The threads were close enough to engage and tighten. However, the unknowing different thread type did not hold pressure resulting in the hose blowing free and striking the employee in the upper lip.
While lifting a battery out of a pickup to install in a haul truck, worker strained right shoulder.
While swinging a sledge hammer to un choke a rock chute the worker hurt left shoulder on 10/18/2017. On 10/30/17 went to doctor. On 12/4/17 started therapy. On 1/30/2018 first day lost to have surgery.
While servicing a vibrating screen at Dupo Quarry, the worker struck right knee against the corner of employee's parts crate fracturing patella.
EE was trimming metal off of head pulley with a cut off wheel when debris got past EE's eye protection. First aid treatment was administered and EE was monitored. Swelling occurred and EE was taken to receive medical treatment on 8/23/2017.
While adjusting the setting on jaw crusher a mechanical failure occurred on the adjusting ram hydraulic hose. The failure allowed the jaw adjusting mechanism to close on the employee hands in the adjusting shim area. Employee was transferred by EMS to hospital trauma center with crushing injuries to both hands.
Employee was lifting nitrogen charging service tool box from the side box of service truck. The tool box weighs about 30 pounds and the side box shelf was about chest high. Employee felt a strain in right shoulder. Follow up with same day medical evaluation indicating strain of muscle and tendons of the rotator cuff.
Employee was walking by the rear of welding service truck and contact a piece of sharp metal protruding from the bed. Employee's left upper arm was lacerated requiring sutures.
Employee was replacing bolt-on cutting edge on CAT 980H bucket. The worn plow bolt twisted under the power of removal with air impact. The bolt head caught and cut the employee leather glove lacerating left index finger
EE(equipment operator) was assisting another ee(mechanic) on equipment maintenance. A frozen dirt clod fell on ee left leg and left hip knocking him to the ground. Other ee called 911 and ee was taken to the local hospital 3 miles away. The hospital is small ultimately ee was transferred to St. Johns Mercy St. Louis for further testing and care.
