Electricians were changing a fuse on the pressure washer. The electrician was at the back of the truck when the wash bay tech got out of the forklift to talk to the electrician. The wash bay tech failed to set the parking brake or to put the bucket down. The forklift freewheeled, pinning the EE between the forklift and the truck.

EE was walking from parking a haul truck on the ready line and slipped and fell, landing on EE's right shoulder and elbow. EE reported the incident upon occurrence. EE refused medical treatment and thought the strain would heal. After a period of time with no improvement EE sought medical attention.
Employee was carrying a 12' bolt to the front of the bolter when EE lost footing and fell. When EE fell EE's finger was smashed between the bolt EE was carrying and a rock.
Employee was walking from truck to the administration building when employee became dizzy and fell. When employee fell employee struck face on the front of a bumper on a parked ATV.
Employee was closing the rock guard on the front windshield with EE's left hand and had EE's right hand placed on a pinch point. When the guard shut it cut EE's thumb causing a laceration.
Employee went to get a water hose to water the ribs. EE turned the hose on and underestimated the pressure of the hose causing EE to lose grip. The hose whipped around and struck the employee on the head.
Employee put a break out box on the drill, it rotated unexpectedly pulling employee to knees.
Employee was hanging a 480 volt cable with a cab hook. The cable slipped pulling the cab hook off the wire causing the cab hook to come into contact with the employees' right hand causing a laceration.
Employee was checking for a hydraulic leak when the hood came down and struck EE's right hand that was between the hood and the cab.
The employee was plugging in the power cable from the bolter to the power source. There was resistance when EE was trying to plug it in so EE pushed the cable up and felt a pop in the abdomen.
A rock fell from the highwall and struck the side of the portal steel sets.
Employee tipped a underground haul truck while making a turn. Employee hit EE's head while exiting the vehicle resulting in a laceration that was stapled at the hospital.
Fall of the roof or back resulting in a fatal injury.
While repairing a broken bracket on air door, employee was using a grinder and welder. At some point in the evening the employee thought EE had some dust in EE's right eye and reported it to EE's supervisor, who took EE into the doctor. The doctor at ER removed foreign object.
Mechanics were removing a hoist cylinder from a bolter. As they were removing a collet one mechanic was tapping on the boss with a hammer and the collet relieved itself and flew approximately 10 feet and hit the mechanic standing on the opposite side in the face. As a result the individual received (5) five stitches above EE's left eye at the eyebrow area.
While the employees where traveling out of the mine the floor mat wadded up in such a way that it caused the throttle to stick and maintain speed when braking was necessary. The vehicle collided with the rib and resulted in a shoulder injury.
EE was struck by a rock from the face while installing color pipe in the lifters. EE was bent over and was struck in the back of the head and neck. EE received 9 stiches on the top of head
Employee was hanging electrical cable from a man basket off of the forks on a forklift. Employee bent over to get under a 42" ventilation bag and twisted employee's left knee.
Mechanic was standing on a ladder leaning on a tire wielding. When he shifted his weight on the ladder he slipped and fell against the tire sliding down the tire on his left side. The mechanic was standing on the second step of the ladder.
Employee was operating a jumbo on an incline in the main haulage. The operator drifted to the right rib and the spare drill steel in the drill steel rack came in contact with the right rib. The drill steel gained pressure breaking the drill steel rack striking employee with the drill steel in the head causing a fatal blow.
While tramming the mucker (L-35) the brakes set up. This caused his head to slam into front window causing his glasses to push into his eyebrow. It cut his eyebrow and required 4 stitches.
Employee was operating a haul truck and strained his back. EE went to a specialist on 2/4/15 and was giving a light duty release for his strained back.
Employee was connecting a 2" drisco line. As he was lifting the line in place to install the clamp, he felt a burning sensation in his left biceps area.
Miner does not know exactly how the injury happened; he stepped off his bolter to load the carousal with bolts. Said he stepped off the bolter took 3-4 steps when he felt the pain in his back at the left shoulder. When questioned, the mine stated he did not twist or trip. He was not lifting anything at the time.
The mechanic was changing flushing head on Bolter 16. While putting the last piece on something caused the inner piece to shoot out. The mechanics finger was caught in between the two pieces. As a result, his finger was cut and so was the tendon. He had to get surgery to fix it.
Miner removing a reamer bit from the cubex; had his hand on the bit reached up to the controls to release the bit. Pulled the wrong lever and the brakeout tool slid forward, instead of the bit being released.
While driving the pin from a ripper shank, the pin punch splintered resulting in a small laceration to the left thumb.
The mechanic's wrench slipped off a bolt while tightening, striking his right cheek which resulted in a laceration. 5 stitches were required.
He had his right arm hanging out of cab smashing his arm between the truck and side of drift.
Removing jammed bolt from bolter when the bolt plate slid down the bolt striking his right hand requiring sutures.
Employee was working on a set of multi media filter that were on a utility trailer. He was standing on the piping that connected the filters together, slipped and fell hurting his right knee, neck and back.

Worker was pulling liner when the built in handles ripped and employee fell backwards landing on left arm
While unloading supplies he pulled on bundle of rock bolts and felt a pop in his left arm. He had surgery to repair a bicep ligament separation at the elbow.
Removing re-mix lid when it slipped cutting his index and middle finger requiring sutures.
Employee was cutting liner with hook blade knife. Liner was cold and harder to cut than normal. Employee slipped and cut his left thigh.
After cleaning shot Crete re-mix truck he was replacing the lid, when he caught his thumb between the truck and the lid, cutting his thumb. His received sutures.
A stack of wire (13) was lying against the rib. When employee removed one wire, the others fell on him, knocking him unconscious for a short time. He received a concussion.
