EE was walking on grating that had been installed in slurry bridge. Crew was in the process of lifting the slurry tank agitator gearbox to do a test fit for the millwrights. Piece was overhead, EE was watching the piece and not watching where they were in relation to the open hole. EE stepped off the grating, there is a clip on the beam that EE struck shin on causing cut about 3".

Employee was cleaning out the thickener tanks on the south end. While cleaning, employee slipped on mud. Employee fell and twisted left knee.

Employee lost control of grinder and cut EE's leg.

EE and crew were turning a floor brace using a sleever bar in one of the bolt holes. Brace was on flat side. EE standing on E side of the N/S aligned brace pulling on the bar toward EE. In the line of fire when piece reached tipping point where crew needed it for rigging to put in place. Piece reached tipping point, EE lost footing. The sleever bar struck EE's leg causing cut.

EE and partner were setting up a jig to assist in fit up of the ring to the duct work. EE was holding the piece low to assist in getting the jig welded to the duct. EE's partner stuck EE's weld rod to tack up the piece and rod got stuck. When EE went to remove the rod, the piece fell and landed on EE's pinky fingertip which caused a laceration. EE did have gloves on.

While moving a skirt that goes on to a conveyor, weighing approx. 150-200 pounds, while on a skate it slid off and pinched left thumb. EE was rushing and did not wait for other staff to secure it before moving it.

EE was assisting spreading a telehandler forks, while they tilted mast down to facilitate tine adjustment, the right tine was moved, after EE began sliding the L tine outward. During this process, the R tine slid back unexpectedly. EE attempted to reposition the R tine w/EE's gloved L hand, EE L middle finger got pinched between tine and mast's lower bar. Skin tear on L finger.

Employee was using an oxygen/acetylene torch to heat and shape rebar material. The worker inadvertently tripped over the torch hose, which resulted in a loss of balance. As a result of the trip, the worker unintentionally made contact with the active torch flame, causing a burn to the left wrist, just above the cuff of the glove.

Cable pull, picking up cable for refeeding into vault. Cable was being fed into vault and pulled out by hand to get it prepped for next pull into building from this vault.

Employee was holding a bull pin with their right/dominant hand and missed, hitting it with a hammer with their left hand and instead hit their right hand with the hammer.

EE first said EE had been experiencing soreness for sometime but no specific event occurred, then EE changed EE's story and now claims EE lost EE's footing a bit while climbing a column, and noticed soreness later that day. However, EE failed to inform anyone until Sunday, March 29th.

EE hit EE's left index finger with a hammer while aligning a piece of steel.

EE was positioned beneath the outlet duct assisting coworker with placement of a stiffener ring onto duct. Two manual chain falls were used to maneuver the ring. Ring weighed about 1000 lbs. Each chain fall was rated for 2200 lbs. A chain fall load chain broke, causing the ring to drop. EE states they felt the stiffener ring brush their forearm before landing on their thumb.

EE preparing to remove rigging from pad eye. EE was lowering hook using overhead crane. EE was detaching the shackle from the pad eye with gloved hands. The chain from the hoist had accumulated above the shackle and became caught on structure above pad eye. The hook from hoist rotated downward, EE's L thumb caught between the hook/housing and shackle. Contusion to thumb pad.

EE was moving a pallet jack loaded with iron when EE encountered a piece of 1/2 inch by 4 inch plywood covering an expansion joint on the floor. The pallet jack became stuck and as EE pulled hard to get it over the plywood, EE slipped and fell to the floor. EE also noted that there was salt and sand in the area where EE was maneuvering the iron.

The injured worker states that EE has right elbow pain. Today, while adjusting the pipe bender with a coworker, EE felt/heard a "pop" in EE's right elbow. There is no appreciable bruising at this time. There is no numbness/tingling into EE's hand or fingers. There are no open wounds

EE in a seated position had completed grinding and set grinder down. They then placed a hand on the duct next to the grinder that had not completely stopped spinning. As they applied weight to their hand to assist in standing up. The weight caused the grinder to slide toward their hand. The spinning wheel contacted the back of left hand resulting in a burn and laceration.

Two ironworkers were working inside a cylindrical hopper at the rail pellet loading area. The task involved welding steel plate sections together as part of the hopper construction. A come along was being used to align steel plates. The come a long was attached to a welded lug. The lug broke and the come along struck the apprentice in the elbow fracturing their arm.

Snowing & cold day while outside installing a fire suppression system on a Hydraulic excavator. Employee was wearing insulated bib overalls, as EE was descending the stairs off excavator EE's bibs caught the stairwell causing EE to trip and miss the last 2 stairs landing on the ground on EE's right shoulder, elbow and hand.

Was bending over and twisting when serving a telehandler to install a plastic cover that allows access to the HVAC box as EE had just finished servicing the blower motor. Felt pain in EE's lower back, very similar to a back injury a year or so ago. called triage, ice and ibuprofen. but EE elected to see doc. EE went to Do con 11-12-25 and was taken off work through 11-19-25.

Employee tripped and impact caused cut bleeding on little finger on right hand. After employee left for the day the employee decided to go in to get finger looked at and it was found that little finger was fractured. Return to Work report brough back by employee next morning stated to avoid heavy lifting with right hand which will not be an issue with their assigned work tasks.

EE cutting off a stiffener inside a piece of ductwork on the fab table with a 6" cut off wheel/grinder. As the cut was getting complete the piece had some stored energy and sprung in toward the cut, causing their right thumb to get pinched between the sheet and the stiffener.

Removing bolts from larger bolt, 45# bolt slid down onto EE's left thumb/hand.

Employee jumped up on a 3' platform, missed, landing back on ground.

Employee tripped while walking on path, knee made contact with ground causing laceration.

Employee tripped while walking on steel. Chin made contact with steel during fall.

Doubler plate became dislodged from it's rigging and dropped, pinching the workers fingers between the doubler plate and the steel under it.

Forklift brought in a piece of 3'x15'x1' rubber, 3 people lifting together, EE bent down to lift it up and felt pain in back while starting to lift.

EE states EE was lifting a 3"x3"x8' piece of dunnage that wasn't heavy. EE twisted wrong and felt immediate pain.

While worker was flying in a section of steel into place, the steel slipped down catching the workers right thumb between another piece of steel.

Employee was holding a large wrench while assisting coworker tightening a nut. When coworker tapped the end of wrench with hammer, wrench fell off the ring and landed on EE's right pointer finger.

Carrying a 1' X 1' X 3/4" piece of steel when the employee tripped and fell, landing on the right hand that the employee was carrying the piece of steel with.

Employee was descending down an A-frame ladder and missed placed footing from 3rd rung to 2nd rung on ladder and lost balance.

11/1/17 of a deceased person on proper of the mine. The deceased person was a trespasser on the property, and had no connection to any mining activity.

While on mine property, a security guard working for Guardco Security LLC shot and fatally injured a trespasser. This accident is being investigated by WV State Police.

A roof/highwall fall occurred in the number 4 intake fan entry between the end of the fan house and highwall. Fall/slip was approximately 14' wide x 3' thick x 5' high. Fall was not above anchorage.

Employee stepped on D11R track and opened right side engine compartment door in order to repair air filter housing lid, employee stated that he does not know if he stepped back or lost balance when he opened the door, but he fell between the push arm and the track and landed on the tilt cylinder mount bracket on his left side.

Dozer slide on ice covered slope after stopping which resulted in the dozer rolling over Damage has not been estimated as of the time.

EE was putting a new ball stud in a sterring jack, While striking it with a hammer a piece of metal flew off and hit him in the left eye. EE was taken to PMC Hospital immediately after the accident, and then was transported to Hospital in Morgantown.

Employee was dismounting the D9 dozer with both hands carrying tools. He stepped onto the push arm & slipped striking both shins on the push arm and subsequently falling to the ground striking his head. He declined first aid medical treatment on 4-9-13. He went to Boon Memorial Hospital on 4-10-13 where he was treated by the E.R. physician and released to return to work 4-17-13.

Employee was working on a loader and the employee bent over at the waist to pick up a socket that was laying on the ground. When he stood up he felt a sharp pain in his back.

NOTIFIED OFFICE ON 11/30/12 OF ACCIDENT THAT WAS CLOSING GATE AT SITE ON 11/15/12 AND STEPPED IN A RUT AND KRILLED ANKLE AND KNEE HIT GATE AND HURT LEG

While employee was changing bits in the cutter head, the hammer he was using hit the chisel and slipped off and struck the back of his hand. He was holding the chisel in his left hand, the hammer was in his right.

The employee was dismounting the D-75 Cat dozer via the right side push arm. The employee had inadvertently stepped on a rock with his left foot, at which time he fell to the ground and felt pain in his left knee. Employee stated he maintained 3 points of contact while dismounting.

Operator was dismounting WT-10 Caterpillar water truck when he slipped on step and fell backwards striking his right hand on the ground breaking the fifth bone in his hand was treated and released from hospital.

EE fell from a Caterpillar 777-D Rock Truck. It appears that the employee was transferring his dinner bucket from one rock truck to another from the operator's deck to the next rock truck off side fender that was 4ft. away. He over extended himself and slipped and fell between the two parked trucks to the ground. EE has 3 broke ribs and brusing on his left side.

The employee was operating a 777 cat trk on the haul rd when he stated he hit a bump in the rd. At which time he felt a pinch in his lower back and tingling. The employee had stated that he had previously had problems with his back when in the military in the 1990's

Employee twisted his right knee getting on man trip while checking the deep mine pumps.

While working on the raw coal belt skirt board EE stepped off the belt structure onto a lump of coal twisting his right knee. He told the supervisor around 11:30 during lunch. He finished out the shift. He went to the ER at Montgomery General Hospital around 8:00 that evening, he was told that he had torn ligaments in the knee and to see his family Dr for a follow up.

Trespasser was electrocuted while stealing copper. There was no active mining in the vicinity of the accident.
