Employee was performing a maintenance operation to replace a damaged conveyor trough. While lifting the new roller assembly, the employee lost their balance and started to fall. They braced themselves against a concrete block adjacent to them and the roller assembly fell striking their right middle finger causing injury to the tendon of the finger.

Mostrando los 6Injured Employee had driven a cold chisel into a gap on a mill journal to spread the pinch flange to remove a shaft from the journal. While attempting to remove the chisel from the gap it was dislodged and came free unexpectedly and with high force hitting ee in the tibia of right leg at about halfway down from the knee.
A contract welder was using an angle grinder to cut some material on an impact crusher. Upon completing EE's cut EE proceeded to set the grinder down and it twisted such that the grinding wheel cut through EE's glove on EE's left index finger causing a laceration of about 1" in length.
The injured employee was moving an oxy/acetylene torch cart by pulling it away from an area where a conveyor was going to be laid down for repair. As the miner was pulling the cart ee stepped on a rock approximately 4" in size when ee said ee felt left foot "pop."
Passenger vehicle was parked on east end of crushing bench in an area away from normal equipment operations. During work on the crushing bench the WA-470 loader required fuel. Operator drove loader to fuel tank situated east of the passenger vehicle and fueled. After fueling the operator backed up striking the hood and grill of the vehicle.
On 11/29/16 at approximately 16:00 two miners had powered down the equipment and locked and tagged out the equipment. They were attempting to rotate the belt to bring the belt part number into view. While rotating the belt by hand the injured employee grasped the belt to attempt to bring it to a stop near the drive sheave pinching thumb between the drive sheave and the belt.
